Performance Work Statement (PWS) For

Blanket Purchase Agreement Flooring Replacement for B120, Fitness Center

Schriever Air Force Base, Colorado 15 July 2020

1. General:

a. Scope of Work:

The work requested in this project consists of the Contractor furnishing all layout, survey, labor, supervision, quality control, materials, equipment, machines, tools, appliances, services, supplies, and incidentals necessary to perform all of the work outlined below

i. Blanket Purchase Agreement – provide demolition and installation services of carpet, tile, and resilient flooring (to include base cove, thresholds and all materials incidental to flooring replacement) when called for by the contracting officer or representative.

ii. Requirements Type Contract – provide flooring installation and demolition services for a specified number of facilities.

b. Site Descriptions Schriever Air Force Base is located approximately 15 miles East of Colorado Springs, with a majority of facilities and infrastructure having been built in the 1980s and 1990s.

Much of the existing flooring is original to the facilities and does not meet updated Installation Facilities Standards (IFS).

c. Project Description The contractor shall provide flooring installation and demolition services, all flooring and incidental materials, dumpsters as well as any materials required for demolition, and materials and services required for cleanup.

d. Period of Performance Contractor shall have 90 days from contract award to purchase and install flooring

a. Carpet:

Purchasing: Contractor shall order carpet using the following Tier I contractors, as is mandated through the Air Force Mandatory Use Policy. Please reference attachments 5a and 5b for additional guidance on the Air Force’s carpet program.

Color Scheme: The color scheme to base flooring purchases on for the B120, gymnasium requirement is the Blue/Grey color scheme.

1. Carpet – Contractor shall install Area Ecoworx tile, Mystical Azure color from Shaw Contract Group.

2. Tile – Contractor shall propose via the form 66, a 12”x12” tile similar to the Daltile listed under the blue/grey color scheme for CE approval.

3. Resilient Flooring – Contractor shall propose via the form 66, a resilient flooring alternative similar to color and style of ceramic tile listed under the blue/grey color scheme for CE approval.

4. Cove Base – Contractor shall propose via the form 66, a cove base, size same as existing, and color matched to “Dovetail,” listed under the blue/grey color scheme.

5. Threshold – Contractor shall propose via the form 66, thresholds/flooring transitions for installation.

3. Contractor Furnished Services:

a. Work Equipment and Materials Staging

All work, equipment and materials required for this contract shall be staged within the area at or near the facilities where work is being performed. Areas where staging of material and equipment shall be coordinated with the Contracting Officer and/or the 50

CES PM. These staging areas will not interfere with building operations or access to work spaces by building personnel. Staging areas will be cordoned off appropriately per applicable safety requirements.

b. Furniture Movement Contractor shall move furniture items to perform the work. All furniture shall be connected and repositioned to its original state after the removal/installation process.

c. B120, Gymnasium Installation and Demolition Reference attachment 6, for areas of demolition and installation of flooring materials.

d. Demolition

i. Contractor shall dispose of, in contractor provided dumpster, all refuse resulting from the execution of this requirement, to include carpet tiles, cove base (as required), etc. Dumpster size and proposed staging location must be approved by government PM prior to placement onsite. Refuse disposal shall comply with the current disposal environmental requirements. All required manifests shall be obtained prior to the transportation of refuse off-site. Contractor shall provide their own dumpster for use during the project. The government shall provide a location near the worksite where contractor may place their dumpster.

ii. Asbestos: If the possibility exists for Asbestos Containing Material (ACM) in the area to be worked it will be identified prior to any work. If there is ACM in the area the Contractor will not use any mechanical means to remove the ACM. In the event during carpet removal it is determined that the ACM must be removed the Contractor must stop and notify the 50 CES PM. Removal of ACM is accomplished by the Government through the use of a separate Asbestos Removal Team.

iii. Removal: The most suitable removal method of floor coverings and associated items will be determined by the contractor. No additional cost will be incurred by the Government for the contractor’s choice of removal methods.

iv. Cleanup: Remove any residual glue, oil, grease, paint, plaster, and other substances detrimental to proper performance of adhesive and carpet or flooring materials. Allow floors to thoroughly dry before laying flooring material.

Floors shall be free of any foreign materials that would affect the proper installation of new floor covering.

e. Installation

i. Prepare floor for installation of carpet, ceramic tile, linoleum flooring, flooring transitions, and new cove base. Flooring adhesive shall meet manufacturer’s specifications and must use low VOC adhesive. Adhesives shall comply with applicable regulations regarding toxic and hazardous materials. Provide water resistant, mildew resistant, nonflammable, and non-staining adhesives and concrete primers for carpet installation as required by the carpet manufacturer.

Install carpet moldings where floor covering material changes or carpet edge does not about a vertical surface. Provide vinyl molding designed for the type of carpet being installed.

ii. Ventilation: If working in an office environment that is inhabited, contractors need to provide ventilation to mitigate fumes from adhesives and other required chemicals.

iii. Patching and Preparation of Surfaces: Inspect all areas before installation.

Repair holes, cracks, depressions or rough areas using polymer fortified patching compounds recommended by the flooring or adhesive manufacturer compatible with existing surfaces to ensure a smooth, finished appearance.

Minimum floor temperature before, during and after installation and requirements for conditioning adhesive shall comply with the carpet and adhesive manufacturer's instructions; however, in no case shall floor temperature be less than 60 degrees F. for 24 hours prior to, during and after installation.

f. Cleanup All plumbing fixtures used to clean equipment shall be cleaned, and restored to their prior state of cleanliness immediately following use. All clean-up activities will be performed in accordance with applicable standards and procedures. Hazardous and waste items will be disposed of properly as outlined in this document’s waste disposal policy. Mineral spirits and other flammable liquids shall not be poured into plumbing fixtures, on grass, on plants, or on asphalt. Flammable paints, liquids and materials will not be stored overnight at facilities other than contractor storage area.

g. Materials and Supplies Acquisition The contractor shall acquire all materials and supplies required for the performance of this contract. The contractor shall bear the cost of all materials and supplies. The items supplied shall be standard products of manufacturers regularly engaged in the production of such supplies or materials. Sign blanks acquired shall meet the requirements of the Base Sign Plan, and the prior approval of the Contracting Officer or his/her designated representative.

h. Vehicles Each Contractor vehicle (cars and trucks) shall be in safe and satisfactory mechanical condition and shall present a neat and clean appearance. Vehicles shall be operated in compliance with base traffic regulations.

i. Safety Equipment and Clothing The Contractor shall provide Contractor personnel with necessary safety equipment and clothing, which shall be worn as applicable while performing work under this contract.

Necessary safety equipment includes items such as steel-toed safety shoes, protective eyewear, protective ear-wear, etc.

j. Telephone Service The contractor is responsible for securing commercial telephone service for making and receiving service calls. Please note that when working within the installation’s restricted area, contractors will not have use of their cellphones.

k. QC Report The contractor shall provide a written report showing that all rework found has been cleared up and the area is ready for inspection.

4. Invoicing and Payment

a. Invoicing will be IAW DFARS clause 252.232-7006, Wide Area WorkFlow Payment

Instructions and PGI 232.70 - Electronic Submission and Processing of Payment Requests and Receiving Reports.

5. Project Management and Quality Assurance

a. Performance Evaluation Meetings: The contracting officer may require the contract manager to meet with the contracting officer, contract administrator, contract inspector, and other Government personnel as deemed necessary. The contractor may request a meeting with the contracting officer when he or she believes such a meeting is necessary. Written minutes of any such meetings shall be recorded in the contract and signed by the contract manager and the contracting officer or contract administrator. If the contractor does not concur with any portion of the minutes, such non-concurrence shall be provided in writing to the contracting officer within 10 calendar days following receipt of the minutes.

b. Quality Control: In compliance with the contract, the contractor shall provide a Quality Control Plan that contains, as a minimum, the items listed in paragraph 6.3, to the contracting officer for acceptance not later than the pre-performance conference.

The contracting officer will notify the contractor of acceptance or required modifications to the plan before the contract start date. The contractor shall make all appropriate modifications and obtain acceptance of the plan by the contracting officer before the contract start date.

i. The Plan Shall Include:

1. A description of the methods to be used for identifying and preventing defects in the quality of service performed.

2. A description of the records to be kept to document inspections and corrective or preventive actions taken.

3. The Records of Inspections: Shall be kept and made available to the

Government throughout the contract performance period and for the period after contract completion until final settlement of any claims under this contract.

c. Submittals: The contractor shall submit the following items for approval by the Contracting Officer:

i. Manufacturer’s Descriptive Data: Submit complete descriptive data for each type of material. Clearly mark data to indicate the type the contractor intends to provide. Data shall state conformance to specified requirements.

ii. Material Safety Data Sheets (MSDS): The contractor shall submit Material Safety Data Sheets for all chemicals proposed for use under this contract to the Contracting Officer.

6. Work Hours

a. The Contractor shall perform services required under this contract during the following hours: 7:00 a.m. through 4:00 p.m., Monday through Friday. Call may require the contractor to perform services at outside of normal hours of operation (to include nights, weekends, and holidays). If the contractor is required to work outside of normal hours of operation, any additional costs over shall be negotiated under each applicable call on a case by case basis. Weekends and federal holidays shall not be worked except by agreement with the CO.
