Performance Work Statement (PWS)

Maintenance and Service of Audio Visual (AV) and Control System Equipment

Little Rock Air Force Base, Arkansas

13 May 2021

1. The primary objective of this PWS is to provide an outline of Maintenance and Service for Audio Visual and Control System equipment in for Building 314, room 100 (Primary CAT Conference Room) and

Building 160, room 121 (Alternate CAT).

2. Vendor must be provide a certification of authority to maintain and repair any Crestron products mentioned below.

3. Contractor shall provide 24/7 on-call maintenance and services for the entire system and sub-components with a 4-hr response time and routine preventive maintenance.

1.1. The vendor shall provide supervision, personnel, maintenance equipment, transportation, tools, and labor necessary to maintain and service the equipment installed in the current system configuration. The vendor will provide all required safety equipment. All equipment used under this contract shall meet all

Occupational Safety and Health Administration (OSHA), Environmental Protection Agency (EPA), and or any other applicable guidelines. All safety equipment must be used for its intended and designed purpose.

Unsafe acts by the vendor will be reported to vendor supervision and 19th Contracting Squadron (19

CONS) and may be grounds for contract termination.

1.1.1. The vendor will not use any government equipment or personnel in the performance of their duties unless section supervision determines it is required for the completion of this PWS.

1.2. Maintenance and Service during warranty period includes the following:

1.2.1. Ensuring system is free from defects in material and workmanship, in accordance with the contract, drawings, specifications, alterations and additions thereto.

1.2.2. All system components regardless of age will be covered by vendor and will be replaced at vendor’s expense in the case of component failure not related to customer misuse/abuse.

1.2.3. 4 hour (or less) on-site response time via phone or on-site service; based on 19th Wing Plans and

Programs (19 AW/XP) determination of resolve.

1.2.4. For the purpose of this PWS, warranty maintenance and service of this system is defined as those tasks to keep the system in original working order.

1.3. The contactor will provide at no expense to the government the repair or replacement of components and equipment due to abuse, misuse, mishandling or neglected maintenance of the vendor or by those employed by the vendor.

1.4. If equipment is down for maintenance for an extended period due to parts back order or building maintenance during the scheduled maintenance and service, the maintenance and service will be completed on that item at another scheduled time that shall be coordinated with the customer.

2. Basic Services – The vendor shall provide the basic services as described in this PWS

2.1. Vendor Check-in –

Prior to the start of any maintenance, service, part installation, parts deliveries, or site inspections, the vendor must check in with the appropriate office to gain access to the area where maintenance is being performed. The vendor will be escorted at all times when working in room 100. Depending on mission requirement, vendor may be required to remove all electronic devices i.e. Smart Phones, Smart Watches, pagers, etc. Due to USAF mission requirements and needs of the government, there may be a delay for supervision availability. There will be no additional costs to the government. Access for installation is assumed Monday through Friday 8:00am to 5:00pm CST.

2.2. Vendor actions at worksite –

Vendor technicians will maintain control of all vendor tools and equipment at the jobsite. At no time should the government be responsible for lost, damaged, or missing tools, equipment, or parts that are property of the vendor on the jobsite. Lost tools and equipment will immediately be brought to the attention of section supervision. Any debris or trash created by the vendor or replaced parts on the jobsite will be disposed of in accordance with paragraph 2.3 of this PWS. Contractor personnel are only to be in their assigned areas and are not allowed in other secure areas without approval and/or escort.

2.3. Vendor Checkout –

After finishing any maintenance and/or service, part installations, part deliveries, or site inspections, the vendor must check out with the applicable office. Inspect of all work performed will ensure work has been accomplished IAW this PWS and meets requirements necessary for the government.

2.3.1. Any dust, dirt, debris on floor that is caused by the vendor will be swept up and cleaned before the vendor leaves and will be disposed of. Unless otherwise stated in this PWS, the contractor will remove all debris from area and will not dispose of items in the section’s trash or trash dumpsters unless inspected and approved by section supervision. The areas will be checked at vendor checkout and vendor will not be released until area is clean.

2.3.2. Cardboard boxes may be disposed of in section cardboard dumpster once inspected and approved by section supervision.

3. General Information:

3.1. Recognized Holidays:

The contractor may be required to perform service on holidays including but not limited to:

3.1.5. Independence Day

3.1.6. Labor Day

3.1.7. Columbus Day

3.1.8. Veteran's Day

3.1.10. Christmas Day

3.2. Hours of Operation:

The contractor is responsible for conducting business with the individual purchased maintenance plans, except when government facility is closed due to local or national emergencies, administrative closing, or similar government directed facility closing. The contractor shall not be reimbursed when the government facility is closed for the above reasons.

3.3. Worker's Health and Safety:

The contractor shall comply with all applicable Federal, State and local legal requirements regarding workers' health and safety. The contractor is solely responsible for determining the legal and safety requirements that apply to his/her activities. These include but are not limited to all OSHA safety rules and regulations, arc flash training, personal protection training and equipment to protect themselves and others from unsafe work conditions. The contractor shall report any accidents or OSHA violations immediately to the appropriate authorities and within 24 hours to the Procuring Contracting Officer

(PCO).

3.4. Security Requirements:

The contractor shall comply with all security controls directed by the Government Site POC (i.e.

Facility Manager), or Program Manager and 19th Security Forces (19 SFS) personnel while on base.

Failure to do so may result in the immediate removal of contractor personnel from the base and denied reentry. In this case, the Government is not responsible for costs accrued by the contractor.

3.5. Government Property Damage:

The contractor shall record and report, within 24 hours, to the Power Conditioning and

Continuation Interfacing Equipment (PCCIE) Program Manager, PCO and designated government site

POC, all available facts relating to each instance of accidental damage to government property or injury to either contractor or government personnel. The contractor shall not perform any work on the damaged equipment/property until released by an authorized government representative. If the government elects to conduct an investigation of the accident, the contractor shall cooperate fully and assist the government personnel until the investigation is completed.

3.6. Damage to Property or Equipment:

The Contractor, at their own expense, shall repair any damage to Government property or equipment that can be attributed to the Contractor. Said repairs shall be made by a skilled workman, and to the satisfaction of the Contracting Officer or a designated representative.

3.7. System Software and programming Code:

The Contractor will provide the 19 Communication Squadron with full access to system passwords, encryption, software and programming under this contract.
