Contract Title: Fitness Equipment Maintenance
1. DESCRIPTION OF SERVICES.
1.1. General Description. The Contractor shall provide all labor personnel, equipment tools, materials, supervision, other required items, and non-personal services necessary to perform routine inspections, preventative maintenance, and repairs of cardio and strength equipment as defined in this Performance Work Statement (PWS) within the Freedom 1 Fitness Center
Complex/Building #404 & Annex/Building #400 at Moody AFB, Georgia.
1.1.1. Contractor tasks include inspect, maintain, clean, and repair gym equipment in accordance with original equipment manufacturers (OEM) specifications, procedures, industry standards, and/or best practices, (in that order). The estimated quantity of equipment is listed in Appendix
B.
1.1.2. The Contractor shall submit reports and documentation as identified throughout this PWS.
Requests for such reports and documentation and responses shall be coordinated through the
Fitness Center Director (FCD), or the FCD Designee.
1.2. Location. Service will be performed at the Freedom 1 Fitness, 4851 Burrell St., Building
04 and the Annex Fitness Center Building 400, (both) on Moody AFB, GA 31699.
1.3. Hours of Operation. All work will be performed between the hours of 0500-2200 Monday through Thursday, 0500-2000 Friday. Federal holidays will be observed at the Freedom 1
Fitness Center as specified in section 4.4.1. of this PWS.
1.4. Operational Requirements and Specific Tasks
1.4.1. Initial Inspection. An initial inspection shall be conducted by the contractor within 7 calendar days of contract award. The initial inspection is to allow the contractor to develop a maintenance plan.
1.4.1.2. The initial inspection will allow the contractor to inspect all equipment listed in
Appendix B and to research the corresponding equipment manuals for manufacturer recommended maintenance requirements. The manufacturers recommended preventative maintenance requirements for each piece of equipment shall be identified in the maintenance plan.
1.4.2. Maintenance Plan. Within 5 business days following the initial inspection, the contractor shall submit a maintenance plan to FCD for approval.
1.4.2.1. The maintenance schedule shall include all manufacturer recommended maintenance for each piece of equipment listed in Appendix B.

1.4.2.2. The maintenance schedule shall include all contractor recommended maintenance and inspections additional to manufacturers recommendations.
1.4.2.3. The maintenance schedule shall include all repairs identified in the initial inspection, the dates they can be repaired, and parts required.
1.4.2.4. The maintenance schedule shall include inspections and preventative maintenance at least monthly.
1.4.2.5. A maintenance schedule shall be updated, re-accomplished, and approved by the FCD for each fiscal year, by the beginning of that fiscal year, for the duration of the contract.
1.4.2.6. All preventive maintenance and repairs will be documented on a maintenance log. An example maintenance log can be found in Appendix D. All logs, guides, specifications, and manuals used to perform this contract will be maintained by the Fitness Center staff at the front counter.
1.4.3. Preventative Maintenance/Inspections. All inspections and preventative maintenance shall be in accordance with the maintenance schedule submitted to the FCD or designee in accordance with paragraph 1.4.2 of this PWS.
1.4.3.1. Inspections and preventative maintenance work shall be in accordance with manufacturer’s recommendations for each piece of equipment listed in appendix B and shall include, but is not limited to, lubrication, adjustments, functional checks, preventive maintenance based upon the specific needs of the individual equipment, and replacement of all parts deemed necessary by the original equipment manufacturer recommendations.
1.4.3.2. The contractor shall be responsible for validating through the FCD or designated representative all warranty documentation to ensure times of maintenance, service, replacement parts, and equipment service is documented correctly. Contractor shall comply with all requirements to maintain equipment warranty in good standing.
1.4.3.3. The contractor shall make all repairs possible during preventative maintenance and inspections in an effort to limit unnecessary cost to the government.
1.4.3.4. When equipment requires additional parts, and/or a follow on visit from the contractor, an anticipated time for repair, and parts required to complete the repar shall be written on the form in Appendix C. The form shall be delivered to the FCD, or designee for approval at the end of the visit. The government shall order the parts and provide them to the contractor for the follow on Non-Routine Service Call.
1.4.4. Non-Routine Service Calls. Maintenance and repair is required on any piece of equipment listed in appendix B when the equipment no longer meets the requirements of

Contract Title: Fitness Equipment Maintenance manufacturer’s intended use. All repairs shall be accomplished in accordance with the manufacturer’s recommendations.
1.4.4.1. The FCD or a designated representative is authorized to make calls to the contractor for non-routine services.
1.4.4.2. The contractor shall complete an on-site inspection of unscheduled maintenance or non-routine service calls within 3 business days. An anticipated time for repair, and parts required to complete the repair shall be written on the form in Appendix C. The form shall be delivered to the FCD, or designee for approval at the end of the visit.
1.4.4.3. The FCD or designee shall notify the contractor when parts are ready for a repair identified through preventative maintenance or an non-routine service call. The contractor shall complete on-site repairs within 3 business days after receipt of notification that the parts are ready. Following the repair the Contractor shall log the relevant information into the corresponding Maintenance Log Form. See Appendix D for an example Maintenance Log.
1.4.5. Emergency Repairs. Emergency Repairs are repairs designated by the FCD or designee on equipment vital to the operations of the Fitness Center or a Fitness Center Program.
1.4.5.1. The FCD or designee is authorized to make calls for emergency repairs.
1.4.5.2. The contractor shall respond with an on-site inspection within 24 hours to inspect the equipment, make repairs if immediately possible, or determine the parts needed to make repairs.
An anticipated time for repair, and parts required shall be delivered to the FCD, or designee for approval on the form in Appendix C at the end of the call.
1.4.5.3. The FCD or designee shall notify the contractor when parts are ready for a repair identified through an emergency service call. The contractor shall complete on-site repairs within 48 hours after receipt of notification that the parts are ready. Following the repair the
Contractor shall log the relevant information into the corresponding Maintenance Log Form.
See Appendix D for an example Maintenance Log.
1.4.6. Re-performance. Re-performance is required, at contractor cost, for any repairs that do not return equipment to fully operational status or manufacturer’s intended use. See Section 4.3.
for additional information on Re-Performance.
1.4.6.1. If cost of repair exceeds cost of equipment replacement, or the FCD or CO determine that replacement is of greater value to the government than repair, than the contractor is released from re-performance obligation. Contractor may recommend replacement of equipment, but contractor’s recommendation alone does not release contractor from reperformance. Only a

Contract Title: Fitness Equipment Maintenance written determination from the FCD or CO can release contractor from contractor’s re-performance obligation.
1.4.7. Payment.
1.4.7.1. Invoicing. Contract shall invoice only once a month for all fees associated with the contract performance of that month.
1.4.7.2. Initial Inspection Fee. Contractor will be paid a one-time initial inspection fee. This fee will include contractor’s initial inspection, and the development of the maintenance schedule per PWS 1.4.1.
1.4.7.3. Preventative Maintenance/Inspections Monthly Fee. The monthly fee shall include preventative maintenance and inspections as identified in the PWS and the Contractor’s maintenance plan. In addition, the fee shall include parts required for preventative maintenance/repair and items (such as lubricants, etc.) used in performance of the maintenance/repair. This fee does not cover minor or major repairs the contractor may encounter during preventative maintenance/inspections.
1.4.7.4. Equipment Replacement Parts. Replacement parts shall be ordered by the contractor for the government for minor and major repairs and paid for by Government Purchase Card.
Replacement parts shall be invoiced separately from all other repairs and must be approved by the FCD before ordering.
1.4.7.5. Minor and Major Repairs Fee. Minor repairs are classified as those repairs requiring less than $500 in replacement parts per individual piece of equipment. Major repairs are classified as those repairs requiring greater than $500 in replacement parts per individual piece of equipment. Minor and major repair fees shall be paid only as a result of non-routine service calls, and emergency service calls. Repairs associated with preventative maintenance, and the contractor developed maintenance plan in PWS 1.4.2., do not qualify for the Minor and Major repairs fee. The contractor must meet the qualifying requirements in PWS 1.4.4. for both the
Non-Routine service call and Non-Routine repair in order to get the fee.
1.4.7.6. Emergency Repair Fee. In addition to the minor or major repair fee, an emergency repair fee shall be paid to the contractor for emergency repair responses complying with PWS
1.4.5. The contractor must meet the qualifying requirements for both the emergency inspection and emergency repairs in order to get the fee. Only one emergency repair fee shall be invoiced per qualifying emergency visit, regardless of number of pieces of equipment serviced.
1.4.7.7. Payment Examples.
1.4.7.7.1. FCD makes a call for emergency repairs on 3 pieces of equipment, contractor responds within 24 hours in accordance with the PWS 1.4.5., repair parts total is less than $500 for two pieces of equipment and greater than $500 for one piece of equipment; Contractor responds within 48 hours after notification the government has the repair parts and completes the repairs on all three pieces of equipment; the contractor will be paid the following: Two Minor
Repair fees, One Major Repair fee and One Emergency Response fee.
1.4.7.7.2. Contractor identifies a repair requiring parts greater than $500 on one piece of equipment during a monthly inspection and submits form in appendix C for approval.
Contractor will be paid monthly maintenance and inspection fee for the monthly maintenance and inspection visit. FCD approves the Contractor’s recommended repairs and orders parts.
FCD notifies Contractor when parts are in. Contractor responds in 3 business days as identified in PWS 1.4.4.3. Contractor repairs equipment with government provided parts. The Contractor will be paid the following: One Major Repair fee.
1.4.7.7.3. FCD makes a call for a non-routine service call on four pieces of equipment.
Contractor arrives during the time specified in PWS 1.4.4.2. and inspects equipment. Contractor can make repairs on the spot with a parts total less than $500. Contractor will be paid the following: Four Minor Repair Fees.
1.4.8. Contractor Requirements. At a minimum, the contractor shall be certified by Life
Fitness, Cybex, and Hammer Strength prior to period of performance start date. All certifications must be provided with contractor’s quote. If Contractor does not currently have the appropriate certification, contractor shall submit a plan to achieve certification prior to the period performance start date with the contractor’s quote. All plans to achieve certification provided with the quote shall be reviewed and approved by the CO to ensure certification occurs before the period of performance start date.
2. SERVICE SUMMARY (SS).
2.1. General. The purpose of this paragraph is to state the performance objectives, the paragraph where located in the PWS or SOW, and identify the performance thresholds
(standards) to determine the overall effectiveness of services delivered to the Government.

3.0. Government-Furnished Services.
3.1. Refuse Collection. The base refuse and recycling services Contractor is responsible for the collection and removal of all trash and refuse. However, when the Contractor finds it necessary, at times, to dispose of refuse and trash generated as a result of this contract requirement, the Contractor may use the base dumpsters with approval of FCD or designee.
3.2. Emergency Medical Service. Moody Air Force Base Emergency Medical Technicians
(EMTs) will provide emergency medical services, if required, on a reimbursable basis. The
Contractor is responsible to reimburse the Government.
3.3. Utilities. The Government will furnish reasonable amounts of utilities to include electricity, water, sewage, and heating. The Contractor shall exercise responsible use of all utilities.
3.4. Smoking. The Contractor and/or his employees shall not smoke inside Government facilities and shall ensure that their work areas are neat, clean, and orderly. Contractor must smoke at approved locations.
3.5. Original Equipment Manufacturers Service Manuals. The Original equipment manufacturer service manuals will be kept at the Fitness Center Front Desk and will be available for Contractor use upon request.
4. GENERAL INFORMATION.
4.1. Contractor Personnel.
4.1.1. Contract Manager. The Contractor shall provide a contract manager who shall be responsible for the performance of work. The name of the contract manager and an alternate(s), who shall act for the Contractor when the manager is absent, shall be designated in writing to the
CO prior to the contract start date.
4.1.1.1. The contract manager or alternate(s) shall have full authority to act for the Contractor on all contract matters relating to the daily operation of this contract.
4.1.1.2. The contract manager or alternate(s) shall be available during standard duty hours within sixty (60) minutes to meet on the installation with Government personnel designated by the CO to discuss problem areas. After standard duty hours the manager or designated alternate shall be available within two (2) hours.
4.1.1.3. The contract manager and alternate(s) shall be able to read, write, speak, and understand
English.
4.1.2. Contractor Employees. The CO may restrict the employment under this contract of any
Contractor employee(s), or prospective contract employee(s) who is identified as a potential threat to the health, safety, security, general well-being or the operational mission of the installation and its population.
4.1.2.1. Contract personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing clothing bearing the name of the company (uniforms) or by wearing appropriate badges, which contain the company name and employee’s name. The Contractor is responsible for acquiring an appropriate number of badges to meet their needs at their own expense.
4.1.2.2. The Contractor shall ensure that employees have current and valid professional certifications before starting work. Vehicle operators shall have a valid Georgia driver’s license.
4.1.2.3. The Contractor shall not employ any person who is an employee of the United States
Government if the employment of that person could create a conflict of interest, nor shall the
Contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DD
5500.7-R Joint ethics Regulation (JER), Chapter 2. In addition, the Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106, Air Force Industrial Labor Relations Activities, paragraph 3.
4.1.2.4. The Contractor is cautioned that off-duty active military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours, or deployment. Military Reservists and National Guard members may be subject to recall or active duty. The abrupt absence of these personnel could adversely affect the Contractor’s ability to perform. Their absence at any time shall not constitute an excuse for nonperformance under this contract.
4.2. Quality Assurance.
4.2.1. The Government shall rely on the contractor’s existing quality assurance system(s). The
Government relies on and enforces the inspection/acceptance procedures stated in FAR 52.212-4(a).
Contractor shall possess an acceptable inspection or program for quality control, utilizing the industry standard commercial practices and procedures to ensure that only conforming services are tendered for
Government acceptance. This system or program for quality control shall be validated by the
Government to ensure the process or processes fulfill the requirement of the PWS. The plan shall be submitted no later than the pre-performance conference
4.2.2. The Government will periodically evaluate the Contractor’s performance by appointing representatives to monitor Contractor performance to ensure services are received in accordance with the contract requirements. The CORs will evaluate the Contractor’s performance through intermittent on-site inspections of the Contractor’s quality control QCP program, receipt of complaints from base personnel, and/or COR surveillance and evaluation. The Government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints. The Government may inspect contract services using a variety of methods.
4.3. Re-performance of Nonconforming Contractor Services.
4.3.1. Re-performance of Service. When service performed does not conform to contract requirements, the Contractor shall re-perform the service in conformity with the contract specifications at no additional cost to the Government. Re-performance of the service(s) shall commence immediately upon notification by the program manager; and the Contractor shall have a sufficient workforce dedicated to ensure corrections are accomplished within 3 business days without degradation to standard services.
4.3.2. Acceptable Performance. Acceptable performance is achieved when the Contractor’s performance meets the minimum contract standard(s).
4.3.3. Unacceptable Performance. Unacceptable performance exists when the Contractor does not meet the minimum contract performance standard(s).
4.3.4. Performance that does not meet contract standards shall be dealt with as cited in accordance with 52.212-4, Contract Terms and Conditions—Commercial Items—(a) Inspection and Acceptance. The Contractor shall only tender for acceptance those items that conform to the requirements of the contract. The Government reserves the right to inspect or test any supplies or services that have been tendered for acceptance. The Government may require repair or replacement of nonconforming supplies or re-performance of nonconforming services at no increase in contract price. If repair/replacement or re-performance will not correct the defects or is not possible, the Government may seek an equitable price reduction or adequate consideration for acceptance of nonconforming supplies or services.
4.4. Hours of Operation.
4.4.1. Holidays. The Contractor is not required to provide services on the following days, in accordance with the DOL Wage Decision:

4.4.2. Good Friday. The Contractor may substitute the “Good Friday” holiday time-off for the day after Thanksgiving, which is usually considered a “family day” for the general base population. If the Contractor takes advantage of this option, this arrangement must be finalized between the CO, Contractor, and COR two (2) weeks prior to the holiday
4.4.2.1. On work days that have been declared “family days,” the Base may be minimally manned with many offices and facilities closed. On these days, the COR shall ensure that the
Contractor has access to the work site.
NOTE: Any of the above holidays falling on a Saturday will be observed on the preceding
Friday. Holidays falling on a Sunday will be observed on the following Monday.
4.4.3. Holiday Services. Services scheduled for a certain day of the week, but cannot be performed due to a holiday, shall be performed either the day before or the day after the holiday.
If a service is scheduled on Monday and that Monday is a holiday, then the service shall be performed on the day after the holiday (Tuesday). Services scheduled for Friday and a holiday is on Friday, then the service shall be performed on the Thursday before the holiday.
4.5. Security Requirements.
4.5.1. All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering the Government installation, shall abide by all security regulations of the installation.
4.5.1. Pass and Identification. Pass and identification items required for contract performance shall be obtained for Contractor personnel and non-Government-provided vehicles. Procurement of the appropriate passes and identification items are the responsibility of the Contractor.
4.5.1.2. The Contractor shall obtain personnel pass application forms from their Contract
Administrator. The Contractor employee shall complete and carry the forms to the Security
Forces personnel at the Visitors’ Center; vehicle registration, proof of insurance and a valid driver’s license must be presented for all vehicles to be registered.
4.5.1.3. Contractor shall be responsible for retrieving pass and identification items from
Contractor employees upon their departure from employment, prior to contract expiration date;
these items shall be turned in to the personnel at the Visitors’ Center.
4.5.2. Listing of Employees. The Contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number, and type of investigation if contract work involves access to Government-automated-information systems (AIS), unescorted entry to Air Force restricted areas, or other sensitive areas designated by the installation commander, or for access to unclassified sensitive information or sensitive equipment. The list shall be provided to the CO and the sponsoring agency’s Security Manager. An updated listing shall be provided when an employee's status or information changes.
4.5.3. Security Training. Contractor employees must receive initial and recurring security training from the Contractor’s representative concerning the “Security Requirements” associated with the contract on which the employee performs. The Government may provide additional security information to the Contractor for dissemination to employees as world events and security requirements change.
4.5.3.1. Key Control. The Contractor shall establish and implement methods of ensuring all keys and vault combinations issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons.
4.5.3.1.1. The Contractor shall immediately report to the COR or Program Manager any occurrences of lost or duplicated keys.
4.5.3.1.2. In the event keys, other than master keys, are lost or duplicated, the Contractor may be required, upon written direction of the CO, to re-key or replace the affected lock or locks without cost to the Government. The Government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the Contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government, and the total cost deducted from the monthly payment due the Contractor.
4.5.3.1.3. The Contractor shall prohibit the use of keys, issued by the Government, by any persons other than the Contractor’s employees and the opening of locked areas by Contractor employees to permit entrance of persons other than Contractor employees engaged in performance of contract work requirements in those areas.
4.5.3.2. Lock Combinations. The Contractor shall control access to all Government-provided lock combinations to preclude unauthorized entry. The Contractor is not authorized to record lock combinations without written approval by the Government Program Manager.
4.5.4. Traffic Laws. The Contractor and its employees shall comply with base traffic regulations.
4.5.5. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their Contractor-owned or privately-owned vehicle(s) while on Moody AFB GA.
4.5.6 Physical Security. The Contractor shall be responsible for safeguarding all
Government property and controlled forms provided for Contractor use. The Contractor shall be responsible for safeguarding Government funds and monies received for services or products provided in performance of this contract. At the end of each work period, all Government facilities, equipment, and materials shall be secured.
4.5.6.1. Fire Prevention. The Contractor is responsible for following Fire Prevention practices and procedures as set forth in AFI 32-2001, Fire Emergency Services Program.
4.5.7. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of
DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.
4.5.7.1. Controlled Areas. The Contractor shall implement local base procedures for entry to
Air Force controlled areas where contractor personnel will work.
4.5.8. Retrieving Identification Media. The Contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires;
e.g., retirement, etc.
4.5.9. For Official Use Only (FOUO). The Contractor shall comply with DoD 5400-2R, Appendix C, Section 2, Information Security Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material
4.5.10. Additional Security Requirements. Additional security requirements, including
Communications Security (COMSEC) User Requirements; Command, Control, Communications, and Computer (C4) Systems; Information Assurance (IA) Program; and
Operations Security (OPSEC), shall be complied with by Contractor personnel.
4.6. Safety Requirements.
4.6.1. Safety of personnel and performance shall conform to Office of Safety and Health
Administration (OSHA) requirements as specified in AFI 91-301, Air Force Occupational and
Environmental Safety, Fire Protection, and Health (AFOSH) Program.
4.6.2. Cell Phone Usage. Cell phone usage while driving on base is prohibited, unless one is using a completely “hands-free” device.
4.8. Government Performance of Services during Labor Strike.
4.8.1. The services called for under this PWS and at the direction of the CO, the Contractor agrees to remove its non-striking force from the performance site and not to interfere in any way with Government performance.
4.8.2. Contractor further agrees under such circumstances to permit the Government to use any essential Contractor-furnished property. The Government will equitably compensate the
Contractor for use of such property.
4.8.3. Contractor shall not be entitled to payment for any performance period or parts thereof during which the Government assumes performance.
4.9. Records Management.
4.9.1. All records, files, documents, regardless of media (e.g., paper, electronic, etc.), as described in this PWS, are the responsibility of the Contractor and are the property of the
Government and shall remain so upon termination or completion of the contract.
4.9.2. The Contractor shall keep these items current and maintain and dispose of them in accordance with the requirements established in Air Force Instruction (AFI) 33-322, Records
Management Program, paragraph 5, Air Force Manual (AFMAN) 33-363, paragraph 6.4, Air
Force Instruction (AFI) 33-364, paragraph 2.10, and the Air Force Records Disposition Schedule which may be accessed from the web address cited below:

4.9.3. Records shall be turned over to the Government upon completion of the contract, unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act. The
Moody AFB Records Management Office (23 CS/SCXKR), 257-3500, will provide assistance and training to the Contractor to ensure compliance with Federal record-keeping requirements.
4.10. Health and Safety on Government Installations.
4.11.1. In performing work under this contract on a Government installation, the contractor shall:
4.10.2. Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and
4.12.3 Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes.

APPENDIX A
DEFINITIONS
General Definitions.
CO - Contracting Officer
Contract Management. Implementation of the Performance Plan (PP)/Quality Assurance
Surveillance Plan (QASP) after contract award by the multi-functional team (MFT). It integrates the goals and objectives identified by the multi-functional team (MFT), oversight and performance management, and performance measurement in accordance with the Performance
Plan or Quality Assurance Surveillance Plan.
Contracting Officer’s Representative (COR). Individuals designated to perform quality assessment functions, and oversee performance in accordance with the quality assurance surveillance plan (QASP). They serve as on-site technical managers assessing Contractor performance against contract performance standards.
Defective Service. Any service output that does not conform to contract requirements as stipulated in the Services Summary.
Functional Commander/Functional Director (FC/FD). The person responsible for a functional area at the Squadron level. For example, the Civil Engineer Squadron Commander is the Functional Commander for the Civil Engineering performance-based work statements.
Market Research. A continuous process for gathering data on service characteristics, suppliers’ capabilities, and current business practices, in addition to the analysis of that data to make acquisition decisions.
Multi-Functional Team (MFT). A team of stakeholders responsible for a service acquisition.
Includes not only representatives of the technical and procurement communities, but also stakeholders of the service acquisition and the Contractor(s) who provide the service. This team could be a Business Requirements Advisory Group (BRAG), a Mission Area Acquisition Team
(MAAT), an Integrated Product Team (IPT), working group, and A-76 IPT or Steering Group, etc. At Moody AFB GA this team is called the Multi-Functional Team (MFT).
Performance Assessment/Quality Assurance. A process that measures success towards achieving defined performance goals or objectives defined within the performance thresholds in the services summary or the process of assessing progress towards achieving the objectives/goals developed in a Performance Plan.
Performance Management. The use of performance measurement information to effect positive change in organizational culture, systems, and processes, by helping to set agreed upon performance goals, allocating and prioritizing resources, informing managers to either confirm or change current policy or program direction(s) to meet those goals and sharing results of performance in pursuing those goals.
Performance Management Assessment Program/Quality Assurance Program. A quality assurance program that is developed, planned, and managed to carry out cost-effectively all efforts to affect the quality of services. The Performance Management Assessment Program includes performance measurement and performance management by the multi-functional team
(MFT).
Performance Objective. The outcome associated with successful contract performance in a specific area. This is a critical success factor in achieving the organization’s mission, vision and strategy which, if not achieved, would likely result in a significant decrease in customer satisfaction or risk mission failure. Obtaining multi-services/sub-services performed at a certain measurable standard and consistently ensures success in achieving the objectives critical to the mission.
Performance Threshold. The minimum standard of a performance objective required by the
Government.
Quality Assurance Program Coordinator (QAPC). Mission support group individual, normally from the contracting activity, selected to coordinate and manage the Performance
Management Assessment Program (Quality Assurance Program), in addition to the training of the CORs, COR Supervisors, and Functional Commanders when necessary.
Quality Control. Those actions taken by the Contractor to ensure the standards of performance of services as stated in the contract and PWS or SOW are obtained.
Quality Assurance. Those actions taken by the Government to ensure services meet the requirements of the Performance Work Statement (PWS) or Statement of Work (SOW) and contract.
Services Summary (SS). A summary of the performance objective(s) and performance threshold(s) required by the Government in contractor performance, along with the paragraph where the particular objective and threshold are located in the PWS or SOW.
