FAR Provision 52.212-2, Evaluation -- Commercial Items, applies to this acquisition. As prescribed in FAR 12.30 l(c), the following addendum is provided for this solicitation and hereby amends any language therein:

The Government intends to award a Firm-Fixed Price purchase order resulting from this solicitation to the responsible quoter whose quote provides the best value to the Government, in accordance with FAR 13.106-l(a)(2)(ii) and FAR 2.101. There is no ranking or assigned importance to evaluation factor IAW FAR 13.106-l(a)(2)(iii). The use of FAR 15.3 and DoD Source Selection Procedures are not applicable to this solicitation, or any award resulting from this solicitation.

Evaluation Factors and Sub-factors: The following evaluation factors and subfactors will be used to evaluate each quoter. Award will be made to the offeror that provides the best value to the Government determined by using a Subjective Tradeoff as outlined below. Final quotations will be rated against the following factors and sub-factors described below.

Factor 1: Price Factor 2: Technical

Sub-factor 1: Quality Control Plan Sub-factor 2: Management/Manpower Plan

Factor 3: Past Performance

Quoter's are required to meet all material solicitation requirements, such as terms and conditions, representations and certifications, deliverables, Performance Work Statement (PWS), etc. Failure to comply with the terms and conditions of the solicitation may result in Quoter being ineligible for award. Quoter's must clearly identify any exception to the solicitation terms and conditions and must provide a complete supporting rationale. The Government reserves the right to determine any such exceptions unacceptable, and the quote, therefore, ineligible for award.

For a quote to be eligible for award, the quoter must have a current registration in the System for Award Management (SAM):

https://www.sam.gov/SAM, prior to submitting a quote, IA W FAR 4.1102. SAM will be checked to verify the quoter's status before any evaluation is conducted.

Quoter's are advised that their quotation may exceed the requirements of the PWS, but the Government is not requesting or accepting alternate quotes, and each response must respond to the solicitation requirement stated in the RFQ. The Government intends to evaluate quotes and issue a purchase order based on the initial quotes received. Therefore, the contractor's initial quote should contain the contractor's best terms from a pricing standpoint. The Government reserves the right to request revised quotes from, or discuss final terms with, one or more contractors if later determined by the contracting officer to be necessary.

Additionally, the contracting officer will not establish a competitive range, conduct discussions, or otherwise use the procedures described at FAR 15.306.

Two copies of the quote are required: (1) original quote and (2) redacted and sanitized copy of the quote removing all company or personally identifiable information (e.g., company name, logo, individual names, etc.). Failure to submit redacted copy may result in disqualification.

Factor 1 - PRICE

Total Evaluated Price:

Only the total evaluated price (TEP) submitted in the quote will be evaluated in Attachment 3 - Quote sheet. The Government will evaluate quotes for award purposes by adding the total price for all options to the total price for the basic requirement.

Evaluation of options shall not obligate the Government to exercise the option(s). In accordance with FAR 52.217-8, the Government may require continued performance of services within the limits and rates specified in the contract. The rates shall be determined to be the unit price of each applicable CLIN, as stated in the Schedule of the award of the CLIN, which corresponds with the most recently entered period of performance. Evaluation of CLIN prices constitutes evaluation of the rates

Evaluation Criteria http://www.sam.gov/SAM used for options exercised under this clause.

Factor 2 - TECHNICAL

The Government’s technical evaluation team shall evaluate the technical quotations on an acceptable or unacceptable basis, assigning either acceptable or unacceptable ratings for each sub- factor. Offerors that fail to clearly address any of the following requirements may receive an Unacceptable rating for this subfactor. Any sub-factor evaluated as “Unacceptable” will render the entire quote unacceptable and, therefore, not awardable. Only those quoters determined to be technically acceptable, either initially or because of discussions, will be considered for award. However, the quoters are reminded that the Government reserves the right to award this effort based on the initial quote, as received.

Rating Description Acceptable Submission meets the requirements of the solicitation with a low to moderate risk of unsuccessful performance Unacceptable Submission does not meet the requirements of the solicitation and there is a high risk of unsuccessful performance

Sub-factor 1: Quality Control Plan

DESCRIPTION: Offeror shall submit a Quality Control Plan that addresses all requirements of the Performance Work Statement (PWS). The plan must address methods and processes to be used for performing monthly, quarterly, semi-annual, and annual preventative maintenance. The plan shall also include procedures to identify potential areas of risk or degradation to the DOME and various items of equipment meant to support, as well as methods for correcting deficiencies.

Measure of Merit: This requirement is met when the QCP demonstrates an effective performance/quality monitoring system that addresses all requirements of the PWS and will enable the contractor to identify and correct their own errors/discrepancies.

Sub-factor 2: Management/ Manpower Plan

DESCRIPTION: The offeror shall provide a management staffing approach that adequately provides personnel qualified to meet the requirements of the PWS. This should include retention of personnel, organization structure, and related commercial and/or government experience supporting The DOME and various items of equipment that support DOME. The offeror shall submit a realistic approach to their hiring and retention process that enables contract performance at Minot AFB, ND.

Measure of Merit: An acceptable rating is met when the plan identifies individuals with a minimum of three (3) years of experience in projects of similar size and scope to fulfill the key positions. Provide information on the individual’s name, title, duty, years of experience and experience level relating to similar projects.

Factor 3 - PAST PERFORMANCE

DESCRIPTION: Past Performance record history will be assessed as one of the multiple standards required for a successful quote to be determined responsible as described in FAR 9.104. Past performance can be evaluated based upon the contracting officer's knowledge of and previous experience with the service being acquired, customer surveys, past performance questionnaires and government-wide contract performance assessments rating system (CPARS). Quoters may submit up to three past performance documents, to display the quality of work performed. A Past Performance Information Form (PPIF), Attachment 5, is provided with the RFQ for quoters to send to a maximum of three (3) references of their choice. PPIF references who have returned the questionnaires will be used in the past performance evaluation. Please note that the PPIF will only be accepted if it’s received directly from the reference, and the PPIF must also be received by the quoter due date and time.

For purposes of this evaluation, recency is defined as active or completed efforts performed within the last three (3) years from the date of issue of this solicitation. If the past performance is not considered recent, it will not be evaluated any further. Data independently obtained from other Government and commercial sources may also be used. Relevant past performance demonstrates Dome experience and experience on various items of equipment meant to support Dome as defined by the North American Industry System code (NAICS Code) 811310 with a total contract dollar value of at least $250,000.00 and having similar complexity and function, utilizing trades relevant to those required by this solicitation.

The Government may use a combination of the following methods to evaluate past performance:
