PERFORMANCE WORK STATEMENT (PWS)
FOR
COOLING TOWER CLEANING
1. DESCRIPTION OF SERVICES. This contract is for non-personal services to the Government. The Contractor shall provide all management, tools, supplies, equipment, transportation, personnel and labor necessary to ensure tower chemical testing and cleaning services are performed at Altus AFB, Oklahoma in a manner that will ensure continuous and safe operation of the cooling towers
1.1. BASIC SERVICES. The Contractor shall provide on-site monthly chemical testing of cooling tower cleaning chemicals and annual cleaning Altus AFB, OK. Coolings covered under this contract are large-sized towers located at building 88, 172 and 179. Contractor shall provide all required testing equipment and supply items at no additional cost to the Government.
1.1.1. REGULATIONS. All services under this PWS shall be done in accordance with the Unified Facilities Criteria (UFC) 3-240-13FN dated 25 may 2005 or newer. This regulation is available to the public at the Whole Building Design Guide website http://dod.wbdg.org/.
1.1.2. TECHNICIAN CHECK-IN. The Contractor’s Technician shall check in with the COR Office at building 358, room W123 upon arrival at Altus AFB.
1.2. MONTHLY CHEMICAL TESTING. The Contractor shall perform monthly quality control chemical testing for each cooling tower building serviced. Chemical dosage rate and pump adjustments shall be made by the Contractor as determined by testing results. (UFC Chapter 6).
1.2.1. MONTHLY VISITS. The Contractor shall schedule monthly recurring visits at least one
(1) week in advance of arrival.
1.3. ANNUAL COOLING TOWER CLEANING SERVICES. The Contractor shall perform annual cleaning of the cooling towers. Annual cleaning shall include, but is not limited to: run tests and diagnostics as required, check for leaks, ensure all gauges and indicator lights function properly. Clean towers to remove organic materials. Operational and safety checks shall be performed by the Contractor. Reference UFC 3-240-13FN Chapter 4 for additional information.
1.3.1. COOLING DEFECTS. If the Contractor finds leaks, broken gauges, lights, etc., the contractor personnel shall report these findings, by building number, to the COR.
1.3.2. ANNUAL VISIT. The Contractor shall perform annual scheduled cleaning during the last week of December and the first week of January. The systems can be worked on during this time period without interruption to the training mission schedule.
1.3.3. INCLEMENT WEATHER. Occassionally, inclement weather may occur during the last week of December and the first week of January. If this occurs, the COR shall contact the http://dod.wbdg.org/
Contractor to reschedule the services. Attempts will be made to ensure that the training mission is impacted as little as possible.
1.4. APPROVED CHEMICALS. The Contractor shall furnish and use only approved chemicals during the performance of this contract. Contractor must get approval from the COR prior to ordering chemicals. Chemicals that are not approved, may not be reimbursed.
1.4.1. The following chemicals are reimbursable under the contract.
- Sodium Polyacrylate, NCH Chemsearch, CWT-92, M/M, 0633, Product # 12061536
- Aqueous Solution, Chemsearch, 15000LTT, C777, Product # 12069063
- Biocidal Solution, Chemsearch, Check-Mark 40224, CM, C807, Product # 12043738

1.5. CHEMICAL STORAGE. During the first month of the contract, the Contractor shall order six (6) storage pail vaults (Product # 1065742) and three (3) 20 gallon storage tanks (Product # 12055190) to store excess chemicals. This exact quantity of each storage container is reimbursable under the contract. Any quantities in excess of this amount MUST be requested through the COR, including replacements. Excess quantities that are not approved by the COR shall not be reimbursable.
1.6. SERVICE REPORTS. Service reports shall be sent to the COR office no later than five
(5) business days after completion of the services. Reports must include test results accomplished during the monthly visits and any defects noted during the annual cleaning service.
Payment on the contract shall not be approved until service reports are received.
2. SERVICE SUMMARY (SS). The Contractor service requirements are summarized into performance thresholds that relate directly to mission essential items. The stated performance thresholds are the minimum acceptable levels of service for each stated requirement. The SS provides information on contract requirements, the expected level of Contractor performance and the expected method of Government validation and confirmation of services provided. These thresholds are critical to mission success. Procedures as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies.

2.2. QUALITY ASSURANCE. The COR will assess the Contractor’s performance and adherence to standards using Table 1. The COR shall use random sampling, customer complaints and 100% inspection methods.. If inspection reveals unacceptable performance, the CO will then notify the Contractor and require re-performance of the service (referred to as “call back”). The Contractor shall be allotted time as agreed to by all parties, but no longer than 72 hours, to return to Altus AFB and accomplish corrective call back work.
3. GOVERNMENT FURNISHED PROPERTY AND SERVICES.
3.1. GOVERNMENT FURNISHED PROPERTY. The Government will furnish, without cost to the Contractor, all reasonable amounts of electricity and water in connection with the contract performance while on Altus AFB. The Contractor shall practice energy and water conservation in Government facilities.
3.2. GOVERNMENT FURNISHED SERVICES. The Government will provide police and fire protection while on Altus AFB. In the event of a medical emergency, call “911” for transportation of an injured employee by civilian ambulance to a local hospital. The Government shall provide emergency medical treatment for life-threatening emergencies only until civilian ambulance arrives.
4. GENERAL INFORMATION.
4.1. CONTRACTOR QUALIFICATIONS. The Contractor must provide written evidence of their ability to obtain approved chemicals in the quantities required and with sufficient time to meet the terms of this contract.
4.1.2. The Contractor shall provide evidence that service personnel to be utilized on this contract have a minimum of 3 years’ experience servicing commercial HVAC systems, unless accompanied by a second qualified technician.
4.2. CONTRACTOR PERSONNEL. While on Altus AFB, the Contractor personnel shall present a neat appearance and be easily recognized as Contractor employee. This shall be accomplished through the wear of distinctive clothing, overcoats or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection. The management and control of Contractor employees in the performance of this contract shall be the responsibility and prerogative of the Contractor. However, the Contractor shall comply with the contract requirements and Altus AFB regulations concerning conduct of employees.
4.3. SECURITY REQUIREMENTS. While on Altus AFB, the Contractor shall comply with Altus AFB security measures. These regulations are available for review in the contracting office. Security and base access requirements are contained in clause 5352.242-9000 “Contractor Access to Air Force Installations.” Contract employees will be required to obtain and display identification badges.
4.3.1. Contractor employees shall provide information to assist in the completion of background checks for employees requiring access to Altus AFB. This information shall consist of, but is not limited to: full name (first, middle, last), social security number, date of birth, address, driver’s license number and state of issue, number of years residency in the state of Oklahoma and Jackson County. This information will be protected under the procedures listed in the Privacy Act of 1974. A background check must be completed prior to access to Altus AFB. If the background check reveals a Contractor employee or prospective employee with an adverse criminal history or as a potential threat to the health, safety, operational mission of the installation, then the Government reserves the right to restrict or deny access to Altus AFB for that individual.
4.3.2. CONTRACTOR BADGES. The Contractor is responsible for tracking and retrieving badges from employees upon their separation, termination and/or completion of business on Altus AFB, OK. Badges from employees who are no longer employed on the contract must be retrieved and turned in to the COR within 30 days of the individual’s separation or termination. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access. Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Failure to control or turn in badges may result in contract termination or a decision not to exercise the option periods of this contract.
4.3.3. ANTI-TERRORISM. Altus Air Force Base has an Antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces, and adhere to all security measures implemented during Force Protection Condition (FPCON) changes and Random Antiterrorism Measures (RAM’s). Security Forces members may conduct spot checks and inspections of you and your sub-contractors to ensure you are in authorized areas.
4.3.3.1. Altus Air Force Base is currently in FPCON Bravo, meaning an increased and more predictable threat of terrorist activity exists. You and your sub-contractors are expected to report any suspicious persons, activities, packages, or items you see while conducting work on Altus Air Force Base to base Security Forces. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation, or somebody asking many questions about the base.
4.3.3.2. In the event of an emergency, there are multiple mass warning notification systems on Altus Air Force Base (AtHoc, weather radios, telephone notification, and giant voice system).
4.3.3.3. Any Contractor personnel or sub-contractor obtaining a Common Access Card (CAC) must complete Antiterrorism Level I training within 30 days of receiving your CAC, and on an annual basis thereafter. This training is conducted via Joint Knowledge Online (https://jkodirect.jten.mil), course #JS-US007. In addition, Altus Air Force Base has an Antiterrorism binder available for review at the Pass and Registration Office located at the Visitor Control Center, building #428. 4.3.4. VEHICLES. The Contractor shall provide and maintain vehicles for transportation to meet the requirements of this contract. All Contractor vehicles used in the performance of this contract shall be readily identified as property of the company and be maintained to present a professional appearance at all times. Contractor personnel shall operate vehicles in compliance with base traffic regulations. Any violations of base traffic regulations are considered federal offenses and are adjudicated at federal court in Oklahoma City, OK. The current policy for Contractor personnel receiving a Driving Under the Influence (DUI) charge, on base, shall result in that person having their driving privileges on base revoked for a minimum of one year. See clause FAR 5352.242-9000, entitled “Contractor access to Air Force Installations.” All vehicles shall be registered, insured, and operated by a licensed operator at all times.
4.4. SAFETY. While on Altus AFB, the Contractor shall comply with all safety and health requirements necessary for the protection of personnel, facilities and equipment including, but not limited to, all applicable Occupational Safety and Health Administration (OSHA) standards.
4.4.1. Provide a verbal report to the COR as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO within 5 (five) calendar days of each occurrence.
4.4.2. The Contractor shall notify the COR of any equipment that, when used, would violate fire or safety regulations or that would pose a risk to life or property. The COR shall lock-out and tag-out the equipment. The tag shall indicate the problem, corrective action required, date condition was noted and the anticipated repair date. When the Contractor returns to the site for repairs, they shall notify the COR to unlock the equipment.
4.5. ENVIRONMENTAL AND HAZARDOUS MATERIAL/WASTE (HM/W)
REQUIREMENTS. Hazardous Materials (HM) are defined in 49 CFR 172.101 and 40 CFR 301-304. Absolutely no HM shall be brought onto Altus AFB until that material is authorized for use by the Civil Engineer Environmental Element at (580) 481-7156. The Contractor shall submit a list of chemicals with attached Safety Data Sheets (SDS) and other supporting documentation for each HM prior to their use to the Environmental Element and COR. The Contractor shall take appropriate actions to comply with waste minimization and pollution prevention practices and policies.
4.5.1. HAZARDOUS MATERIALS. The Contractor shall submit their request for hazardous materials to the COR in accordance with installation procedures. The COR shall work with the CE Environmental Element for approval of all HM brought on-site by the Contractor prior to use. The Contractor shall request approval by submitting a list of chemicals with attached Safety Data Sheet (SDS) and other supporting documentation for each HM prior to their use to the COR. The Environmental Element has the right to prohibit the use of hazardous materials it deems to be especially hazardous to human health and environment.
4.5.2. SPILL CONTROL. The Contractor shall maintain spill control material on hand at all times sufficient to contain a worst-case spill, both volume and hazard level. Contractor is responsible for disposal of all material used during spill clean-up. Materials must be disposed of IAW local, state and/or federal regulations. If the Contractor spills or releases any hazardous substances/ hazardous waste into the environment, the Contractor shall immediately report the incident to the Fire Department and the COR. The Contractor shall be liable for the costs of clean up and remediation of any spills or the release of such substances into the environment.
4.5.3. HAZARDOUS MATERIALS INVENTORY. The Contractor shall maintain an up-to-date list of hazardous material inventory with copies of SDS(s) for all materials used on the job site.
4.5.4. Hazardous Materials Management. The Contractor shall store excess chemicals in the vaults and tanks listed in paragraph 1.6. Contractor is responsible for proper storage, labeling and security of each vault/tank.
4.5.5. ENVIRONMENTAL INPSECTIONS. The Contractor is subject to inspection by federal, state or local agencies.
4.5.6. CONTRACTOR ENVIRONMENTAL AWARENESS TRAINING. The Contractor shall ensure all employees that work on Altus AFB complete the Altus AFB Environmental Management System (EMS) Awareness training IAW DAFI 32-7001, , paragraph 4.3, Risk Management-Competence, Training, and Awareness, within 30 days of starting work.
4.5.6.1. The Altus AFB EMS Site Specific Training (Course# EMS111AGGN00099) can be accessed on The Environmental Awareness Course Hub (TEACH) at the following link https://usaf.learningbuilder.com/. This training is accessible from any computer with internet access. The Contractor shall submit the employee training certificate to the COR within 30 days of employee start date. This is a one-time requirement for all current and new Contractor employees working on Altus AFB.
4.6. SERVICE CONTRACT REPORTING (SCR). The Contractor shall report all Contractor labor hours (including sub-Contractor labor hours) required for performance of services provided under this contract for “U.S. Air Force” via the www.Sam.gov website.
Reporting will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30.

4.7. WARRANTY OF WORK. Contractor warrants that work performed under this contract, including work performed by any subcontractor, shall conform to the contract requirements and shall be free of defects for 30 days after completion of work. The Contractor shall remedy, at the Contractor’s expense, any damage to Government owned or controlled real or personal property. The terms of this warranty shall continue for (30) days after the completion of this contract.
4.8. HOURS OF OPERATION. The Contractor shall perform services during normal duty hours which are 0730 to1600, Mondays through Fridays, excluding federal holidays. The Contractor must request/coordinate to work weekends or holidays through the COR.

4.8.1. RECOGNIZED HOLIDAYS. The Contractor is not required to provide service on federal holidays. The holidays are:

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first day post the holiday observance.
4.8.2. BASE CLOSURES. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.
4.8.3. PERFORMANCE OF SERVICE DURING CRISIS OR HEIGHTENED SECURITY. Services at Altus AFB are not considered mission essential and limited service will not continue in the event of a crisis or heightened security caused by a national emergency, natural disaster or other cause, as necessary, in support of the Air Force mission. Contractor employees with approved background checks will be allowed access to the base for performance of work, except during “Charlie” conditions. If the “Charlie” condition exists for an extended duration, some services may be required. The CO may direct that services continue on a case-by-case basis.
4.9. CONTRACT MODIFICATIONS. Periodically, during the contract performance period, the Government may have equipment decommissioned from use or new equipment installed. If this occurs, a modification to the contract shall be initiated to incorporate changes to requirements, if warranted.
