About this file
This document is a Signature of Bidder's Authorized Representative Form for a state contract opportunity with the Oregon Department of Forestry (ODF). The form requires potential bidders to provide comprehensive information about their business, including legal name, business designation, federal tax ID, contact information, and residency status. Bidders must specify whether they are a resident or non-resident bidder in Oregon, and disclose any preferential treatment they might receive in their home state's government contracting.
The form includes multiple certification statements where the bidder acknowledges critical compliance requirements, such as preparing the bid independently, understanding all solicitation provisions, committing to non-discrimination practices, having a written policy preventing sexual harassment, and confirming they are not in violation of Oregon tax laws. The document is designed to ensure transparency, legal compliance, and ethical standards in the bidding process, with the bidder's authorized representative required to sign and date the form, attesting to the accuracy of all provided information.
