SAMPLE RFTOP #1 – For Solicitation Purposes ONLY
Title: Scheduled Deployment of C-C19ERT in Tandem with CDC to Nebraska Nursing Home Facilities
I. BACKGROUND:
As the COVID-19 pandemic continues, a growing number of individuals in healthcare who have persistently and intermittently positive SARS-CoV-2 PCR tests during their convalescent phase of illness. This phenomenon is prominent among residents of long-term care facilities who not only live in congregate settings but are also living among older persons who are more vulnerable to severe COVID-19. More data is needed to inform management of persistently positive SARS-CoV-2 PCR tests in long-term care residents during the outbreak. There is an urgent need for additional data on early markers for persistent PCR-positivity to inform management and better estimate the infectiousness of patients at different time points during convalescence and determine surrogates for viral culture positivity (i.e. infectivity) in PCR-positive specimens.
II. PROJECT OBJECTIVE:
The CDC COVID-19 Response is contracting the C-C19ERT in tandem with CDC emergency response teams to provide assessment (which includes the collection of specimens, testing, and reporting) of nursing home residents in Nebraska. The information gathered from this assessment will provide insight into persistence of viral shedding in SARS-CoV-2 infections.
III. DESCRIPTION OF WORK
CDC anticipates sampling of 50 residents at four Nebraska nursing home facilities over a time schedule of 100 days at 25-day sampling increments. The first sampling will occur 2 days after task order award and then at 25 days, 50 days, 75 days, and 100 days following the first sampling day. Samples to be collected will include one each of orange-top blood tubes, saliva swabs, and nasal swabs. Samples will be packaged and shipped to the CDC following the base contract shipping instructions. While on-site, the C-C19ERT will report to the CDC Dr. Jane Doe (XYZ@cdc.gov, 555-555-5555). If any difficulties or concerns arise in fulfilling this task order, contact CDC, Atlanta Dr. John Doe (ABC@cdc.gov, 777-777-7777).
The offeror should include in their proposal the training plan/completion record of C-C19ERT staff according to the base contract technical requirements. The offeror should include their proposed protocols and materials to be used in the above sample collection work.

IV. DELIVERABLES:
Offeror will provide C-C19ERT services as described in Section III. Description of Work. At the end of the task order performance period, the Offeror will submit a final report to include: 1) the samples collected, 2) sample shipping manifests, tracking documentation, and proof of arrival, and 3) C-C19ERT staff lists and training records. A draft of the final report will be submitted to the COR four weeks prior to the end of the period of performance. The period of performance will end 30 days after the 100-day sampling date to allow time for final reports to be drafted, reviewed by CDC, and submitted following the last sampling date.

XII. INSTRUCTIONS AND EVALUATION CRITERIA:
A. Instructions: Offeror must propose on Tasks 1 and 2 to fulfill this task order. It is the responsibility of the offeror to make clear their intentions upon submission of the proposal.
B. Format:
Offeror technical proposal may be up to 10 pages in length and additionally include appendices of relevant protocols or other documents. Offeror will include in their technical proposal an appendix of all training plans/completions for the C-C19ERT, any federal protections for human subjects, and an anticipated travel plan for deployment.
C. Award: This task order will be awarded to the offeror whose proposal is considered to be the most advantageous to the government, price and other factors identified below considered. Technical factors will be more important in the evaluation than price. The government will not make an award at a significantly higher overall cost to the government to achieve only slightly superior performance.
D. Notice of Intent to Award Contract without Discussions: The government reserves the right to evaluate proposals and award a task order without conducting discussions with the potential offerors.
E. Technical Evaluation:
Technical evaluation criteria for this request for task order proposal (RFTOP) is as follows in descending order of importance:

3. Staffing Plan
Technical Approach:
The offeror’s proposal will be evaluated, as acceptable or unacceptable, on the basis of the soundness and feasibility of the technical approach and the degree to which it demonstrates an understanding of CDC’s requirements and challenges. The offeror should clearly describe the proposed approach to comply with each of the requirements specified in the Performance Work Statement (PWS). The approach should be clear, succinct and demonstrate a complete understanding of the intent and requirements of the PWS in the Offeror’s own words. CDC is seeking the offerors’ input to provide a high quality service at a controlled cost.
The criteria will be evaluated according to the extent that it reflects a clear understanding of the subject areas to be accomplished and on the soundness, practicality, and feasibility of the offeror’s technical approach for providing the services required for this task order.
Management Plan:
Offerors will provide a management plan that describes their approach for managing the work outlined in this task order by demonstrating their understanding of the labor requirements listed in this RFTOQ. Offerors will provide a 1-2 page proposed marketing plan for communicating with potential laboratory recipients at conferences, meetings, their website, and by email.
The criteria will be evaluated according to the soundness, practicality, and feasibility of the offeror’s management plan for this task order.
Staffing Plan:
Offerors will provide a detailed statement of staffing proposed for this task order, including (1) resumes from key personnel (limited to 1 page per resume) outlining the credentials and background of key management, professional, and technical personnel to be used for this task order; (2) a plan that describes current staff available for this task order and how the team will interface with CDC.
The criteria will be evaluated according to the soundness, practicality, and feasibility of the offeror’s staffing plan for this task order.
The following adjectival rating table shall be used to rate technical proposals. The evaluator will use the appropriate table and select one of the adjectival ratings that most clearly reflect the assessment of the proposal’s based on the established evaluation criteria. Price proposals will not receive an adjectival rating.

F. Price Evaluation: A price analysis of the proposal may be conducted to determine the reasonableness of the offeror’s proposal.
G. Price Realism: The government reserves the right to conduct a price realism analysis of the proposal for such purposes as determining an offeror’s understanding of the solicitation’s requirements or assessing risk associated with an offeror’s proposal.
H. Past Performance: The CDC reserves the right to evaluate the past performance of the offeror, the offeror’s proposed employees, and subcontrators.
