PERFORMANCE WORK STATEMENT
LANDSCAPING REQUIREMENTS FOR BUILDING 228 AND BUILDING 296
1. SCOPE: The contractor shall provide all legally required licenses, labor, tools, equipment, travel, material, parts, supervision and administration necessary to perform the landscaping requirements outlined in the description of work.
2. AREA OF WORK: Contractor shall work only in the areas designated by Contracting Officer Representative
(COR). The work area will be accomplished within the immediate areas around building 228 and building 296.
3. PERIOD OF PERFORMANCE: Work must be completed within 8 calendar weeks of contract award date.
Work will be accomplished during the work week, Monday-Friday 0700-1900, reference Section 9, Government
Working Hours. Work will cease if Altus AFB Mission requirements deem necessary.
4. DESCRIPTION OF WORK: The work to be accomplished consists, in general, of the following principal items. This does not mean the work consists only of these items, but that it consists of these principal items along with all incidental minor work which becomes necessary to complete to accomplish the overall project in satisfactory and workman like manner meeting the approval of the customer and the Contracting Officer.
• Contractor will furnish: all labor, tools, materials, personnel, and equipment to fully accomplish all requirements as listed below:
- Completely remove all shrubbery (to include stumps) on the northeast and southwest corner of building 228
- Completely remove all concrete pavers from the west (main) entrance of building 228
- Completely remove all current landscaping features (wooden retaining walls and rock on the northeast corner and southwest corner of building 228.
- Prepare area for new landscaping, to include commercial grade weed barrier fabric and pre-emergent granular for weed prevention.
- Level the areas on the northeast, southwest, and west sides of building 228 with new, nutrient rich soil
- Construct stone edging/retaining wall in areas that will house new landscaping. Stone edging (bricks) will be mortared to ensure longevity and stability.
- Plant new, native shrubbery in intervals that create a clean, professional appearance on the northeast corner, southwest corner, and west side (main entrance) of building 228.
- Fill landscaped area with an appropriate amount of river rock.
- Professionally trim six trees on the west side and nine trees on the east side of building 228.
- Professionally trim one tree on the west corner and one tree on the north corner of building 296.
- Haul off all debris, rock, and trimmings to off-base debris collection site.
5. GENERAL CONTRACT MANAGEMENT
5.1. Employees must be eligible for clearance badges for on-base access.
5.2. Contractor must check in with the Facility Manager for building 228 prior to performance. The Facility Manager must be contacted before the contractor leaves the area, to make sure all required work has been satisfactorily completed. POCs for building 228 and 296 are Mr. Christopher A. York at 580-481-7639 or Mr. Michael S. Speer at 580-481-6014.
6. PASS AND IDENTIFICATION ITEMS The Contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non- government owned vehicles requiring access to Altus AFB to perform contract requirements:
6.1. The contractor is responsible for providing information to assist in completing background checks for those employees requiring access to Altus AFB. This information shall consist of, but not limited to, the following: full name to include first, middle, and last name, social security number, date of birth, address, driver’s license number and state of issue, number of years resident of the State of Oklahoma and Jackson County. This information will be protected under the procedures listed under the Privacy Act of 1974. A background check must be completed prior to unescorted access. Those contractor employees whose background checks reveal adverse criminal history may be denied access to Altus AFB.
6.2. The contractor is responsible for tracking and retrieving badges from employees upon separation, termination and/or completion of business on Altus AFB, Oklahoma. Badges from employees no longer working on the contract must be turned in to the Contracting Office within 30 days of leaving.
6.3. Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Excessive abuse may result in termination or a decision not to exercise option periods for this contract. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access.
6.4. Altus Air Force Base has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes).
Additionally, you and your sub-contractors are expected to report to base security forces any suspicious activities, packages, or items you see while conducting work on Altus Air Force Base. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation, or somebody asking a lot of questions about the base. In an effort to help familiarize you, your employees, and sub-contractors about antiterrorism, Altus Air Force Base has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate (building #428) of Altus Air Force Base, Oklahoma.
7. ANTITERRORISM CLAUSE:
7.1. Altus Air Force Base has an Antiterrorism program that is responsible for securing and protecting the base and its personnel from terrorist attacks. All contractor personnel and sub-contractors are required to obey all orders from Security Forces and adhere to all security measures implemented during Force Protection Conditions (FPCON’s) and Random Antiterrorism Measures (RAM’s). Security Forces members may conduct inspections of your company as well as your sub-contractors while operating on the installation. Unescorted entry into any area marked as a “CONTROLLED AREA” or “RESTRICTED AREA” is strictly prohibited.
Compliance with Unified Facilities Criteria (UFC) 4-010-01, DoD Minimum Antiterrorism Standards for Buildings, is required for all applicable construction projects.
Altus Air Force Base is currently in FPCON Bravo, meaning an increased and more predictable threat of terrorist activity exists. All contractor personnel and sub-contractors are expected to report any suspicious persons, activities, packages, or items you see while conducting work on Altus Air Force Base to base Security Forces. Suspicious activities could include personnel conducting surveillance, unauthorized personnel requesting access, or information probing pertaining to security operations or assets on the installation.
In the event of an emergency, there are multiple mass warning notification systems on Altus Air Force Base (AtHoc, weather radios, telephone notification, and giant voice system).
Any contractor personnel or sub-contractor that obtains a Common Access Card (CAC) MUST complete Antiterrorism Level I training within 30 days of receiving your CAC (https:// jko.jten.mil/courses/AT-level1/launch.html) Altus AFB has an Antiterrorism binder available for review at the Visitor Control Center, building 428. 8. IP-SECURITY CLAUSE:

9. ENVIRONMENTAL STATEMENT:
9.1. Contractors must comply with all environmental requirements to include completion of initial Environmental Management Systems (EMS) awareness level training within 10 days of contract award or with a new contract employee beginning work on the Air Force Installation and the appropriate documentation of training completion is to be maintained. Training is available at http://www.altus.af.mil/shared/media/document/AFD-130214- 023.pdf. Questions regarding this requirement can be directed to 97 CES/CEIEC at 580- 481- 6221. All chemicals brought onto Altus AFB must be reviewed through the Environmental, Safety and Occupational Health Offices prior to use on the installation.
10. Contractor personnel shall present a neat and clean appearance and be easily recognized as contractor employees. The contractor shall provide contractor personnel working at Altus AFB with a method of identifying the company name, employee’s first and last name, and job title. This may be accomplished by a name badge or a company shirt. Employees must always wear identification.
11. Contractor personnel shall always present professional conduct while on Altus AFB. Loud volumes and use of profanity are strictly prohibited.
