Assistant Director’s Intent
Dr. Stewart Smith ERO Assistant
Director, IHSC
The ICE Health Service Corps (IHSC) mission is to deliver safe and high‐quality health care to noncitizens in ICE custody. IHSC fulfills this mission daily, as the only entity within ICE responsible for providing direct medical care to detained noncitizens. IHSC is committed to delivering optimal health care services to reduce global disease, and supporting the safe apprehension, enforcement, and removal of detained noncitizens involved in immigration proceedings.
As ICE missions evolve, the ability to provide the safe delivery of high‐quality health care is essential. The construction of new facilities with efficient IHSC medical and administrative workspaces is critical to fulfill the ICE mission.
The intent of the ICE Health Services Design Standards is to offer development and design criteria for the building, sustainment, renovation, and upgrade of ICE medical facilities. Its goal is to simplify and regulate the construction and design development, while adhering to health service requirements for a detention facility. Stakeholders responsible for the planning, design, and construction of new and renovated health services facilities, must use ICE Health Services Design Standards as a tool during the planning process, from commencement to supplying the facility.
This revision is the result of the IHSC Health Operations Plans and Programs (HOPPs) Unit leadership and collaboration with the ICE Facility Support Unit (FSU) and other IHSC units. I expect these standards will be invaluable to IHSC, in anticipating and preparing for future ICE detention facility needs.
I personally thank you for choosing IHSC. Together, we provide world‐class health care to noncitizens in ICE custody, help protect national security and public safety, and safeguard the American people and our homeland.

Executive Summary
U.S. Immigration and Customs Enforcement (ICE) is responsible for the health and welfare of noncitizens in its custody. Medical staff address the health care needs of detained noncitizens in accordance with national detention and accreditation standards. ICE Health Service Corps (IHSC) staff provide the safe delivery of high‐quality health care to detained noncitizens in ICE custody, in accordance with national standards at IHSC‐staffed facilities.
This document provides a standard facility design framework for all IHSC‐managed facilities. Its purpose is to simplify and standardize the construction and design process, while meeting the requirements for immigration health services delivered within a detention center.
ICE Health Services Design Standards, herein referred to as “Design Standards,” detail the technical requirements for administrative and medical spaces in which IHSC cares for noncitizens in ICE custody.
The layout enables IHSC health staff to deliver quality care and ensure the overall health and safety of the ICE detained population.
Individuals involved in the planning, design, and construction of new and renovated health services facilities should use this document as a tool throughout the process, from conception to furnishing the facility. These individuals include ICE and IHSC personnel.
This Design Standards document supersedes the previous guidance titled, Department of Homeland Security (DHS) Public Health Service (PHS) Design Standards, published in 2015.
ICE Health Service Corps FY 2023 Page 6 of 156
ICE Health Services Design Standards
This Design Standards document provides planning and design criteria for the construction, sustainment, restoration, and modernization of IHSC‐staffed medical facilities within ICE detention facilities. Contractors should use the Design Standards for all IHSC projects and work for other customers, where appropriate.
The IHSC Health Operations Plans and Programs (HOPPs) Unit, in consultation with ICE Enforcement and
Removal Operations Facility Support Unit (FSU), are responsible for administering the Design Standards.
The HOPPs and FSU working group are responsible for the technical content of the IHSC Design
Standards (Reference Appendix B for Design standards). Staff may send recommended changes with supporting rationale to the ICE ERO FSU unit chief, who will coordinate internally with the HOPPs unit chief.
IHSC Design Standards are effective upon issuance, and IHSC Headquarters or ICE ERO Facilities Support
Unit, will distribute the Design Standards only in electronic media from ICE.
Staff should compare hard copies of the Design Standards against the current electronic version, which is posted online, to ensure they are using the most current standards.
Purpose
The Design Standards contain design criteria and requirements staff must use to develop new construction, additions, or alterations to ICE medical facilities. The purpose of the Design Standards is to convey the design intent and performance specification criteria for IHSC construction projects.
The IHSC Installation includes the local IHSC team, IHSC Headquarters’ supervisor, and ICE ERO Facility Support Unit project manager.
The design agency may develop project designs using their in‐house design personnel or may contract with private‐sector architecture and engineering firms (A/Es) to provide design services. The design agency can be an Army Corp of Engineers or private architectural and/or engineering service provider.
The design agency must use these construction criteria as a benchmark of acceptable quality for IHSC full facility restoration (FFR), minor maintenance and repair (MMR), and other projects.
The design agency must consider these criteria and provide guidance representing a 60% solution; the information should apply at least 60% of the time and address at least 60% of the issues. The design agency should always obtain written sign off from the agency for any design that departs from the criteria detailed in the Design Standards.
Scope
The guidelines found in the Design Standards apply to all new construction projects for ICE medical facilities and serve as a general guide in the modernization or restoration of existing facilities. These guidelines address only the more common or typical features associated with IHSC facilities. The Design Standards deal primarily with IHSC administration, intake, medical clinics, and medical housing units.
ICE Health Service Corps FY 2023 Page 7 of 156
The intent of the Design Standards is to provide a portion of the general information and guidance required for the successful preparation of project designs. IHSC and the ICE ERO FSU team must provide additional information and guidance, all applicable building codes, and designer investigation on such matters as project scope, local codes, and site constraints.
ICE Detention Health Care System
IHSC is a component within ICE Enforcement and Removal Operations (ERO) and the only entity within
ICE responsible for providing direct health care to detained noncitizens. IHSC is committed to providing the safe delivery of high‐quality health care to those in ICE custody and strives to be the best health care delivery system in detention and correctional health care. Individuals in IHSC‐staffed and non‐IHSC‐ staffed facilities receive a health screening within 12 hours of arrival, and a complete health assessment within 14 days of admission.
IHSC has a high‐performing, results‐producing health care workforce made up of a multi‐sector, multidisciplinary staff of approximately 1,650 authorized positions. The positions include U.S. Public
Health Service Commissioned Corps officers, federal General Schedule (GS) civil servants, and contract health professionals. IHSC staff comprise physicians, psychiatrists, psychologists, social workers, psychiatric nurses, dentists, dental assistants, dental hygienists, pharmacists, pharmacy technicians, radiology technicians, registered nurses, advanced practice providers, licensed vocational nurses, medical assistants, medical records technicians, other health professionals, and administrative support personnel.
IHSC administers a detention health system that provides direct health care in ICE‐owned facilities;
oversees compliance with health care related detention standards in contracted facilities; reimburses for off‐site health care services detained noncitizens receive; and supports special operations missions.
IHSC’s structure is divided into five divisions which work together to support the organization’s commitment to being the best health care delivery system in detention and correctional health care.
• The Administration Division supports the activities and the systems utilized by the IHSC personnel responsible for sustaining the IHSC health care system.
• The Clinical Services Division provides medical oversight, guidance, and instruction to IHSC providers and allied health professionals who deliver medical, mental, and dental health care to individuals in ICE custody.
• The Health Care Compliance Division has detention standards compliance oversight for non‐
IHSC‐staffed detention facilities; coordinates continuous quality health care to detained noncitizens housed in IHSC‐staffed facilities; ensures all IHSC‐staffed facilities comply with accrediting body standards; oversees the agency's quality improvement, risk management, and patient education programs; and reimburses clinicians who provide care for individuals in ICE custody outside the detention health system.
ICE Health Service Corps FY 2023 Page 8 of 156
• IHSC’s Health Systems Support Division oversees medical and administrative staff in IHSC‐staffed facilities; facilitates facility openings, transitions and closures; and supports special missions and special operations.
• The Office of the Chief of Staff coordinates across IHSC’s divisions to support the agency’s communications, investigations, policy, external information, and emergency preparedness functions.
In FY 2022, IHSC Headquarters (HQ) comprised 20 units that provided strategic and operational guidance to 19 IHSC‐staffed medical facilities/clinics; oversaw compliance with health‐related standards in non‐
IHSC‐staffed detention facilities; initiated program and process improvements; monitored health care expenditures and the medical claims billing process; and managed the human and fiscal resources necessary that sustained the health care system.
IHSC provides medical support for ERO removals and special operations. Officers from the IHSC Special
Operations Unit serve as medical service providers for ICE missions, both domestically and internationally. IHSC’s Special Operations Unit supports ERO by delivering medical services during domestic transfers and international removals. It also provides medical services to migrants rescued or interdicted at sea on U.S. Coast Guard cutters in support of noncitizen migrant interdiction operations.
Facility Staffing Minimum Requirements
Generally, IHSC’s staffing model and services are proportional to the facility’s noncitizen population.
However, the Health Services Unit’s configuration may vary depending on facility operations and clinical staff composition. The following table presents IHSC’s minimum staffing requirements by facility size, based on Department of Justice, National Institute of Corrections, and National Commission on Correctional Health Care recommendations (2001).
