INDICATOR DESCRIPTION

De-duplicated number of Veterans receiving Grant and Per Diem (GPD), Health Care for Homeless Veterans (HCHV) Contract Emergency Residential Services (CERS), HCHV Low- Demand Safe Haven (LDSH), HUD-VASH, and Supportive Services for Veteran Families (SSVF) Rapid Re-Housing (RRH) homeless services who obtain permanent housing (PH).

POPULATION

For this metric, PH placements are captured for three possible cohorts:

1. Veterans who move into PH through HUD-VASH in FY 2022.

2. Veterans with a HOMES program exit to PH in FY 2022 from GPD, HCHV CERS, or

HCHV LDSH programs.

3. Veterans who move into housing or exit SSVF RRH services to PH in FY 2022.

TARGET AND BUSINESS RULES

Targets will be set at the VAMC level and are based on a 5% increase from the FY 2021 PH placement total. In extraordinary circumstances, target adjustments will be considered for VAMCs that do not have a sufficient unhoused Veteran population to meet the target.

Adjustments will be made by the Homeless Program Office based on an analysis of the Point-in-Time (PIT) Count, By-Name List (BNL), estimated inflow data in the Gap Analysis tool, and Homeless Operations Management and Evaluation System (HOMES) data. All target modification requests must be submitted to HPO’s Business Intelligence Office by the VAMC by December 1, 2021.

ESCALATING TARGETS AND STOP LIGHT FORMATTING

Q1: 25% or above (Green), 24% or below (Red) Q2: 50% or above (Green), 49% to 25% (Yellow), 24% or below (Red) Q3: 75% or above (Green), 74% to 50% (Yellow), 49 or below (Red) Q4: 100% or above (Green), 99% to 75% (Yellow), 74 or below (Red)

NUMERATOR

Number of permanent housing placements in FY 2022.

1. Number of Veterans with a HUD-VASH move-in date between 10/1/21 and 9/30/22.

o HOMES data source: HUD-VASH Housing Progress Form (HPF), item 6 has a date entered.

2. Number of Veterans with an exit from GPD, HCHV CERS, or HCHV LDSH programs to

PH between 10/1/21 and 9/30/22.

o HOMES data source: Residential Treatment Exit Form item 3, options 1 – 7 or 9.

3. Number of Veterans with an SSVF RRH housing move-in date between 10/1/21 and 9/30/22. If no move-in data are available, the Veteran will be counted in the measure if their SSVF RRH program exit is to a permanent destination type.

o HUD HMIS data source: Field 3.20, Housing Move-In Date and Field ID 3.12, Destination options 3, 10, 11, 19-23, 26, 28, and 31-34.

NUMERATOR EXCLUSIONS

Veterans receiving services from multiple programs with duplicate placements to PH will be excluded from the numerator. De-duplication will occur in the following cases:

1. Veteran moves into PH through HUD-VASH and also has an exit from GPD, HCHV CERS, or HCHV LDSH programs, or has an SSVF RRH move-in/exit to PH within 30

PHP1: Increased Permanent Housing Placements

Page: 4 of 76 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2021 days of the HUD-VASH move-in. The GPD, HCHV CERS, HCHV LDSH, or SSVF RRH placement will be excluded from the numerator.

2. Veteran has a program exit from GPD, HCHV CERS, or HCHV LDSH to PH and an SSVF RRH move-in/exit to PH within 14 days of the HOMES program exit. The SSVF RRH placement to PH will be excluded from the numerator.

DENOMINATOR

The denominator for this measure is set at the VAMC level and is calculated based on a 5% increase of the permanent housing placement total from FY 2021.

RATIONALE

VA and community PH placements are key to ending Veteran homelessness. This metric establishes a methodology to identify how many Veterans exit homelessness to permanent housing, thereby ending homelessness for those Veterans, and it establishes targets for each VAMC to increase PH placements and collectively achieve a national de-duplicated permanent housing placement goal.

Page: 5 of 76 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2021

Number of Veterans with a HOMES Assessment who were unsheltered in the preceding 30 days.

POPULATION

Veterans engaged by VA Homeless Programs.

TARGET AND BUSINESS RULES

Targets will be set at the VAMC level. Targets will remain consistent with the targets established in FY 2020 (based on FY 2019 assessments completed), except in communities where there is a gap of more than 10% in the FY 2020 denominator and the three-year average unsheltered Point-in-Time (PIT) Count. In those cases, an increase of 10% will be applied to the FY2020 target. Adjustments to the FY 2022 targets may be requested by VAMCs that do not have a sufficient unsheltered Veteran population to meet the target.

Adjustments will be made by the Homeless Program Office based on an analysis of the PIT Count, By-Name List (BNL), and estimated inflow data in the Gap Analysis tool. In addition to target adjustments, full exemptions from the measure will also continue to be applied for sites that do not have the unsheltered Veteran population to meet the target.

NUMERATOR

Number of assessments conducted during FY 2022 in which Veterans spent at least 1 night in a place not meant for habitation in the 30 days prior to assessment.

o HOMES data source: Intake Assessment Form, item 20, “Where did the Veteran sleep last night?”, answer “Place not meant for habitation.”

o HOMES data source: Intake Assessment Form, item 21, “Over the past 30 days, did you spend at least one night in a place not meant for habitation?”, is option 1, yes.

The denominator for this measure is set at the VAMC level. Targets will be based on FY 2019 assessment totals due to COVID-related factors influencing assessments in FY 2020-2021.

For VAMCs that exceeded the FY 2019 target, the denominator for this measure is the total number of Veterans assessed during FY 2019 who spent at least 1 night in a place not meant for habitation in the 30 days prior to assessment.

The denominator for sites that did not achieve the FY 2019 target will not change, except for VAMCs with a 10% gap in the denominator and the three-year average unsheltered PIT count, which will have an increase of 10% to the FY 2020 target.

The denominator values for each VAMC included in the metric are as follows:

DENOMINATOR EXCLUSIONS

The HCHV Program Office will continue to review VAMC requests for exemption if there is evidence of an insufficient unsheltered Veteran population to meet the target. Please review the HCHV5 Exemption Guidance for additional information.

RATIONALE

Though the VA is making progress in ending Veteran homelessness, much of the gain has come in reducing the number of sheltered homeless Veterans, while the number of unsheltered homeless Veterans has remained relatively stable. Reducing the unsheltered homeless Veteran population is a very high priority for VA and other federal agencies, such as HUD and the United States Interagency Council on Homelessness. This measure is intended to prioritize

Page: 8 of 76 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2021 outreach to unsheltered homeless Veterans, engaging them in the care needed to ultimately obtain and maintain permanent housing.

ESCALATING TARGETS AND STOP LIGHT FORMATTING

90% or above (Green), 89% or below (Red)

NUMERATOR

Total number of positions indicating a “filled” status in the Homeless Staffing Database (HSD).

DENOMINATOR

Number of positions in the HSD during FY 2022.

o Data source: HSD.

DENOMINATOR EXCLUSIONS

The following positions are not included in the denominator:

1. Positions that are not included in the HSD, due to being VAMC-funded or VISN-funded.

2. Positions listed with a “0” FTE in the database (contract positions).

3. Positions that have not been active for at least 90 days.

RATIONALE

The hiring of homeless program positions is essential to VHA’s initiative to end homelessness among Veterans. Position vacancies result in reduced services for homeless Veterans and inefficient use of resources. This measure is intended to provide VACO, VISNs, and VAMCs feedback on hiring rates within their homeless programs and to reinforce hiring efforts.

OP1: Percent Hired

Page: 10 of 76 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2021

Point-in-time measurement of the percentage of Housing and Urban Development-VA Supportive Housing (HUD-VASH) vouchers issued to the VAMC that result in housing.

Veterans currently in HUD-VASH housing.

TARGET AND BUSINESS RULES

The target for September 30, 2022 is 92%.

ESCALATING TARGETS AND STOP LIGHT FORMATTING

92% or above (Green), 91% or below (Red)

Number of Veterans currently in HUD-VASH housing. Veterans who have signed a lease and moved into an apartment unit or house resulting in a homeless Veteran achieving resident status and no longer being counted as homeless. This includes Veterans or their family members who are graduated from HUD-VASH case management services and have retained a HUD-VASH voucher. This is a point-in-time measure. Therefore, if a Veteran is housed at the end of a reporting period, the Veteran will be included on the Veteran-level report in the numerator for the reporting period.

o HOMES data source: Most recent HUD-VASH Housing Progress Form (HPF), item 6 has a date entered.

o ODC data source: Legacy Housing Progress Report 2 (HPR2) item 15 has a date entered and Legacy Case Management Progress Report (CMR) items 6 and 8 are entered as option 1 and item 20a is entered as option 3.

Veterans who have been discharged from HUD-VASH case management with a HUD-VASH voucher or have a family member who continues to use the voucher after discharge are included in the numerator:

o HOMES data source: HUD-VASH Exit Form, item 6 has option 1 or 2 entered, or item 5 has option 12 and 5a has option 1.

o HOMES data source: HET, Section III Voucher Status, item 2 = 9.

Veterans housed at the end of each reporting period are included on the Veteran-level report and in the numerator the reporting period. If the Veteran leaves housing during the reporting period, the Veteran is not included during the month the Veteran was not housed and is removed from the YTD numerator. In all cases, date of move out must be considered. When a subsequent move-in date is documented the Veteran is considered re-housed and will be counted during the move-in month. Veterans are not housed if the following is documented in

HOMES:

o HOMES data source: HUD-VASH Exit Form, item 4 has a date entered and item 6 is entered as option 3 – 9.

o HOMES data source: HUD-VASH HPF Form, item 14 = 1, 2, 3, 5, 6, 7, or 8 and 14a has a date entered.

o HOMES data source: HET, Section III Voucher Status, item 1 has a date entered and item 2 = 1, 2, 5, 6, 7, or 8.

1. Veterans are excluded from the numerator if the HUD-VASH Exit form or VA enrollment file indicates the Veteran is deceased, unless the voucher remains in use by another family member.

HMLS3: Percent Housed in HUD-VASH

Page: 11 of 76 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2021 o HOMES data source: HUD-VASH Exit Form item 5, option 12, or the VA enrollment file, unless exit Form item 5a indicates that the voucher will be used by another household member (option 1).

2. Re-wording suggestion: Legacy records are excluded for Veterans that are included in the numerator based on HOMES data sources.

3. Using a matching procedure between VA and HUD databases, Veterans previously counted in the HMLS3 numerator who had been discharged from case management services with a HUD-VASH voucher and are no longer housed in HUD-VASH according to HUD’s data systems are removed from the numerator. This matching procedure occurs on a monthly basis.

4. Veterans previously counted in the numerator who are no longer housed according to HOMES data.

o HOMES data source: Housing Progress Form (HPF), item 14 = 1, 2, 3, 5, 6, 7, or

8 on the most recently submitted HPF, indicating that the Veteran is no longer housed in HUD-VASH as result of a change in voucher status, and item 14a has a date entered.

o HOMES data source: HUD-VASH Exit Form indicates that the Veteran or family member exited without a voucher, item 4 has a date entered, and item 6 = 3-9 or 5a=0.

o HOMES data source: Section III of the Housing & Employment Tracker (HET) = 1, 2, 5, 6, 7, or 8, indicating that the Veteran is no longer housed in HUD-VASH as result of a change in voucher status. This rule only applies if the Veteran does not have an open admission in HUD-VASH Case Management.

Number of vouchers allocated to the VAMC between FY 2008 – FY 2021. If FY 2021 HUD- VASH vouchers are allocated to PHAs after September 30, 2021, they will not be included in the denominator for this metric.

DENOMINATOR EXCLUSIONS

1. Vouchers that are set aside for project-based housing developments that are not yet activated are excluded from the denominator. These vouchers will be included 90 days after operations begin at the project-based site and will be reflected in the following month’s denominator. Note that in order for the denominator to accurately reflect these vouchers, VAMCs are responsible for updating the HUD-VASH Project-Based Voucher Registry.

2. Vouchers or Tribal HUD-VASH rental subsidies awarded after September 30, 2021 will not be included in the denominator for this metric.

3. Vouchers managed by a service provider designated by the VA Secretary to provide case management services in lieu of VA will be excluded from the denominator. Note that in order for the denominator to reflect this exclusion, VAMCs participating in the HUD- VASH Collaborative Case Management or HUD-VASH Continuum pilot are responsible for notifying the HUD-VASH Program Office as vouchers are released.

SCORECARD DISPLAY EXCLUSION CODES

1. HUD Table Exclusion

2. HPF Voucher Status Exclusion

3. Additional Legacy Exclusion

5. HPF Form Exclusion

6. HOMES Current Status Table Exclusion

7. Veteran Deceased at Program Exit

10. External Date of Death

The number of Veterans who are homeless will decrease with each Veteran who obtains permanent housing through HUD-VASH. Placement into permanent housing is critical to

Page: 12 of 76 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2021 ending Veteran homelessness, and HUD-VASH is VHA’s primary resource for accessing permanent supportive housing.

TARGET AND BUSINESS RULES

VAMCs will be considered “Fully Successful” if they achieve at least a 1.50% increase over the FY 2021 end of FY score. The baseline for this metric will be calculated using the September (end of FY 2021) HMLS3 numerator and the October (FY 2022) HMLS3 denominator. VAMCs will be excluded from the measure if they have successfully achieved the HMLS3 target.

The numerator for this metric is calculated by subtracting the end of FY 2021 HMLS3 score from the current FY 2022 reporting period HMLS3 score.

N/A

DENOMINATOR

Number of vouchers allocated to the VAMC between FY 2008 – FY 2021. If FY 2021 HUD- VASH vouchers are allocated to PHAs after September 30, 2021, they will not be included in the denominator for this metric.

DENOMINATOR EXCLUSIONS

The HUD-VASH Program Office has established a mechanism by which VAMCs can be exempted from this measure if there is evidence there are insufficient vouchers available to achieve the target. Questions regarding this exemption should be directed to the HUD-VASH Program Office at VACOHUD-VASHTeam@va.gov.

RATIONALE

Placement into permanent housing is critical to ending Veteran homelessness and achieving increases in the number of Veterans housed in HUD-VASH is an important step to achieving that goal. This measure encourages increases in performance across all HUD-VASH programs, including those who may experience specific challenges achieving the HMLS3 target or who have experienced plateaus in performance in prior fiscal years.

HMLS3-PI: Increased Housing in HUD-VASH

Page: 14 of 76 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2021

Percentage of Veterans whose process time from admission to move-in to Housing and Urban Development-VA Supportive Housing (HUD-VASH) is 90 days or less.

Veterans in the HUD-VASH program with a housing date in FY 2022.

The FY 2022 target for this measure is 65%.

65% and above (Green), 64% or below (Red)

Number of Veterans whose time from HUD-VASH Case Management entry to HUD-VASH housing move-in is 90 days or less.

o HOMES data source: HUD-VASH Housing Progress Form (HPF), item 6 (move-in date), and HUD-VASH Entry Form, item 2 (entry date). Move-in-entry is < or = 90.

Number of Veterans with an initial move-in date during FY 2022 for the current HUD-VASH case management episode. Note: If a Veteran has a move-in date documented as part of a pre- FY 2022 closed case management episode, the Veteran is included in the measure for the reporting period if a new move-in date takes place during FY 2022. Veterans whose process time crosses fiscal years will count in the year of the move-in date.

o HOMES data source: HUD-VASH Entry Form, item 1, option 1 = yes, and item 2 has a date entered (date of entry to HUD-VASH Case Management).

o HOMES data source: HUD-VASH HPF, item 6 (date Veteran moved into HUD- VASH housing) has a date entered that is during FY 2022.

1. Move-in dates occurring after the initial move-in date during FY 2022 for the same case management episode.

2. Veterans without an FY 2022 move-in date.

3. Veterans who have the same dates for program entry and move-in. These are most likely data errors and would inaccurately skew results for this measure.

4. Veterans with move-in dates during FY 2022 if housing was from a prior, pre-FY 2022 housing episode.

o HOMES data source: HUD-VASH HPF, “Does the Veteran’s current HUD-VASH case management episode retain housing from a previous HUD-VASH episode?” is answered yes, option 1.

1. Not an initial move-in date.

3. Veteran has the same dates for entry and move-in, or move-in prior to program entry.

4. Move-in date during current fiscal year but from prior housing episode.

As HUD-VASH is an integral part of the VHA’s initiative to end homelessness, it is key that efforts are made at the VAMC level to expediently and efficiently house homeless Veterans through the HUD-VASH program.

Percentage of Veterans in the Housing and Urban Development-VA Supportive Housing (HUD-VASH) housing who are employed at the end of each month.

All Veterans currently receiving case management and housed in the HUD-VASH program.

The FY 2022 target for this measure is 47%.

47% or above (Green), 46% or below (Red)

Number of employed Veterans housed in HUD-VASH each month during FY 2022.

o HOMES data source: HUD-VASH Housing Progress Form (HPF), item 6 has a date entered.

o HOMES data source: Current employment status displayed on the Veteran’s History Screen. Status is obtained from the most recent update documented on either the HOMES Exit Forms (employment item = option 3, Part-time or temporary employment, or 4, Full-time employment) or the HET (Section II = options 3, Part-time or temporary employment, or 4, Full-time employment).

Veterans are included in the denominator if the Veteran has a housed date and is active in case management during FY 2022.

o HOMES data source: HUD-VASH Housing Progress Form (HPF), item 6 has a date entered.

o HOMES data source: HUD-VASH Entry Form, item 1= yes and item 2 has a date entered.

Veterans who are disabled/retired, students, or indicate an unpaid volunteer position are excluded from the denominator.

o HOMES data source: Current employment status displayed on the Veteran’s History Screen. Status is obtained from the most recent update documented on the Residential Treatment Exit Form, item 7 = option 0 (Disabled or retired), 7 (Unpaid volunteer), or 8 (Student); or the HET Section II = option 0 (Disabled or retired), 7 (Unpaid volunteer) or 8 (Student).

9. Veteran is disabled/retired, student or unpaid volunteer.

Increasing employment rates among formerly homeless Veterans is an essential component of the VHA’s Plan to End Veteran Homelessness, as it increases likelihood that these Veterans will not return to homelessness.

ESCALATING TARGETS AND STOP LIGHT FORMATTING

90% or above (Green), 89% or below (Red)

NUMERATOR

Total number of HUD-VASH positions indicating a “filled” status in the Homeless Staffing Database (HSD).

DENOMINATOR

Number of HUD-VASH positions funded by the VHA Homeless Programs Office in FY 2022.

HOMES data source: Homeless Staffing Database (HSD).

DENOMINATOR EXCLUSIONS

The following positions are not included in the denominator:

1. HUD-VASH positions that are not included in the HSD, due to being VAMC-funded or VISN-funded.

2. HUD-VASH positions listed with a “0” FTE in the database (contract positions).

3. HUD-VASH positions that have not been active for at least 90 days.

The hiring of HUD-VASH positions is essential to VHA’s initiative to end homelessness among Veterans. Position vacancies result in reduced services for Veterans in HUD-VASH and inefficient use of resources. This measure is intended to provide VACO, VISNs, and VAMCs feedback on hiring rates within their HUD-VASH programs and to reinforce hiring efforts.

VASH4: Percent Hired in HUD-VASH

Page: 18 of 76 VHA Homeless Program Office ● Performance Measure & Metrics Technical Guide 10/01/2021
