About this file
This is a Cost Proposal Submittal Form for RFP 2027-1600-0043 issued by the Alaska Department of Health (DOH) for a SUPRT (Substance Use Problem Recognition Tool) data collection and management system. The project involves developing and maintaining a HIPAA-compliant online platform for DBH subrecipient agency grantees to submit client-level SUPRT data, with subsequent batch upload to the SPARS interface. The contract spans five years with seven deliverables: one-time security assessment and Authority to Operate (ATO) completion, hosting of the HIPAA-compliant solution, data management and formatting, ongoing platform maintenance and licensing, report development for batch upload feedback, compliance reporting and quality review, and technical assistance and provider support including a resource website and training materials. Offerors must account for all costs including personnel (salaries, wages, and fringe benefits), direct and indirect costs, materials, supplies, equipment, and administrative overhead within their pricing for each deliverable and contract year.
The maximum total evaluated cost across all five performance years cannot exceed $1,250,000.00, with any proposal exceeding this threshold deemed non-responsive. Offerors must submit all proposed costs in designated yellow-highlighted cells on the worksheet, which automatically calculates the five-year total. Costs should be allocated based on the provided Timeline and Activities table, which specifies when each deliverable occurs (Months 1-2, Month 3 and ongoing). The contract assumes 12-month performance years, though the initial and final terms may be prorated to align with a desired performance period end date if required by the state.

| INSTRUCTIONS FOR COMPLETING THE COST PROPOSAL WORKSHEET |
| Offerors must enter all proposed costs in the yellow-highlighted cells within the Cost Proposal below. Costs should be provided for each deliverable (RFP SEC. 3.04 Specific Deliverables to be Provided by the Contractor to the State) and for each Contract Year, as applicable. The worksheet will automatically sum all five performance years in the Total Evaluated Cost column. Offerors should assume 12 months for each contract year. Depending on start date of the contract, the initial term and final term could be prorated to align with any desired performance period end date if alignment to a specific end date is required. Costs proposed per deliverable must be all-inclusive. Offerors shall account for all anticipated expenses and assumptions necessary to complete each deliverable, including personnel costs (salaries, wages, and fringe benefits), direct and indirect costs, materials and supplies, equipment, and any administrative and other overhead expenses. |
The following Table 1. Timelines and Activities, is provided to clarify when each associated deliverable of RFP SEC. 3.04 is expected to occur. The “Associated Deliverables” columns identifies which deliverable numbers correspond to specific activities and time periods (e.g., Months 1–2, and Month 3 and Ongoing Timeline and Activites). These deliverable numbers directly cross-reference the deliverables listed in the Cost Proposal which are from RFP SEC. 3.04:
For example, activities marked with “3” in the Timeline table correspond to DEL.#3 – Data Management in the Cost Proposal. If multiple deliverable numbers are listed (e.g., “3, 4, 7”), those activities relate to multiple cost categories and should be reflected accordingly in the pricing. Total evaluated costs in excess of $1,250,000.00 referenced in RFP SEC. 1.02 - Budget - will be considered non-responsive.
Offerors should use the Timelines table as a guide to:
• Understand when work will occur, and
• Allocate costs appropriately across contract years for each deliverable.
