Addenda 01, Performance Work Statement (Basic Contract)

Performance Work Statement

Naval Special Warfare Basic Training Command (NSW BTC)

500 Gallon Waste Tank Pumping Service

1.0 BACKGROUND

1.1 Naval Special Warfare Command (NSWC) requires pumping services of a 500-gallon waste tank in support of Basic Training Command (BTC) facilities.

2.0 REQUIREMENTS

2.1 PRODUCTS AND SERVICES: The Contractor shall provide pumping services in accordance with the below and refill toilet paper, paper-towel, & handwashing soap.

2.1.1 Pumping of a 500 -gallon waste tank.

2.1.2 Removal and disposal of waste matter services shall be conducted once per week.

2.1.3 Refill toilet paper, paper-towel, & handwashing soap once a week.

2.1.3.1 Box of Toilet Paper (96 Rolls) – Monthly Supply

2.1.3.2 Box of Paper Towels (16x250 sheets) – Monthly Supply

2.1.3.3 1 Gallon of Liquid bottle soap -Monthly supply

2.1.4 The Contractor shall provide all personnel, equipment, tools, materials/supplies, transportation, supervision and other items and services necessary to perform services.

2.2 LOCATION: Building 220 at NAB Coronado, CA

2.2.1 The Contractor shall check-in and check-out with the BTC quarterdeck, or assigned point of contact, before and after each pumping service.

2.2.2 It is not anticipated pumping service will be required in between service iteration dates. However, if requested by the BTC quarterdeck, the Contractor may be required to service the tank. A modification will be issued should additional services be required.

3.0 EMERGENCY SERVICE CALLS

3.1 The Contractor shall have the capability to respond to emergency calls at all locations.

3.2 Contractor shall respond to Emergency Service Calls within 24 hours.

4.0 COMPLIANCE WITH ENVIRONMENTAL AND SAFETY LAWS

4.1 The Contractor shall comply, and assure that all subcontractors comply, with all applicable federal, state, and local laws, regulations, ordinances, policies and standards related to environmental matters. The Contractor must comply with all applicab1e federal, state, and local safety requirements. The Contractor shall also comply with the American General

Contractor's Safety Manual and the Occupational Safety and Health Act.

5.0 SPILL CLEANUP

5.1 The Contractor shall bear financial responsibility for cleanup, transportation, disposal and restoration cost in the event a spill or accident is caused by the Contractor.

6.0 HOURS OF OPERATION

6.1 Non-emergency service calls will be scheduled with the Technical Point of Contact for

7:30 AM – 3:00 PM, Monday through Friday, to exclude weekends and federal holidays.

6.2 RECOGNIZED HOLIDAYS: The following Federal legal holidays are observed:

New Year’s Day 1 January Martin Luther King’s Birthday Third Monday in January Presidents Day Third Monday in February Memorial Day Last Monday in May Juneteenth 19 June Independence Day 4 July Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day 11 November Thanksgiving Day Fourth Thursday in November Christmas Day 25 December

6.3 Performance shall conform to professional commercial and industry standards and be performed in a manner that will maintain a satisfactory condition and professional base appearance.

7.0 QUALITY ASSURANCE

7.1 The Government will inspect the Contractor's performance under this contract using surveillance methods appropriate for the nature and complexity of this requirement. The

Contractor will be given the opportunity to correct or address any adverse findings resulting from the Government's inspection consistent with the Inspection/Acceptance clause in the contract. Failure to correct any adverse findings or failure to deliver all products/services described in this contract may result in non-acceptance of the products/services by the

Government. Failure to meet the standards of this contract may result in a deduction of price at least equal to the value of the unperformed or poorly performed requirements.

Performance will be evaluated to determine whether or not it meets the performance standards of this contract. Re-performance at no cost to the Government is the preferred method of correcting any unacceptable performance. The Contractor shall provide the

Government written response why the performance standard was not met, how performance will be returned to acceptable levels, and how recurrence of the cause will be prevented in the future. Failure to perform in accordance with this contract could also result in termination of the contract for cause and/or adversely impact the Contractor's ability to be selected for future contracts.

9.0 INFORMATION USE

9.1 The contractor hereby unequivocally agrees that it shall not release to anyone outside the

Contractor’s organization any information, regardless of medium (e.g., film, tape, document, contractor’s external website, newspaper, magazine, journal, corporate annual report, etc.), pertaining to any part of this contract or any program related to this contract, unless the Contracting Officer has given prior written approval. Furthermore, the contractor is prohibited from using any Naval Special Warfare Command logos and /or other trademarked identifiers in any way without first receiving a specific license to do so.

9.2 Request for approval shall identify the specific information to be released, the medium to be used, and the purpose for the release. The contractor shall submit the request to the

Contracting Officer at least 45 days before the proposed date for release for approval. No release of any information related to this contract shall be made without specific written authorization by the Contracting Office, other than that required for performance of the requirement.

10.0 PERIOD OF PERFORMANCE:

10.1 Period of Performance: Period of performance will be for a total of five years. Resulting contract will involve five, one-year ordering periods. Date of ordering periods to be established upon award.

11.0 POINTS OF CONTACT: Provided at time of award.

Contractor provides the necessary management and oversight necessary to comply with the administrative requirements of the

PWS. This includes timely submission of invoicing.

Contractor complies with administrative requirements by submitting invoices in a timely manner.

*No more than two complaints relating to timely submission of deliverables.
