AOP Box Culvert FSR 665A

DESCRIPTION/SPECIFICATIONS/WORK STATEMENT

DESCRIPTION

A. SCOPE OF CONTRACT- Contractor shall furnish all labor, equipment, materials, tools, transportation, supplies (including safety), supervision, and perform all work necessary for the installation of one reinforced concrete box culvert, one reinforced concrete pipe (RCP) relief culvert, and incidental construction as specified herein.

Construction shall be performed at the Long Cane Ranger District on the Sumter National Forest in accordance with plans and the Standard Specifications for Construction of Roads and Bridges on Federal Highway Projects (FP14), and with the Forest Service Supplemental Specifications. In accordance with the contract provisions, the contractor is to provide 1 box culvert installation, 1 relief culvert installations, and improvements to the road surface.

B. PROJECT LOCATION – The project site is located on the Sumter National Forest, Saluda County, SC, as indicated on the enclosed maps.

C. PROTECTION OF ADJACENT AREA – The Contractor is responsible for protecting from damage all adjacent areas and existing improvements around or near the project sites. Equipment shall not be operated in the stream channel when and if there is live flow. The contractor shall be permitted one stream crossing to move necessary equipment to the opposite side of the box culvert replacement. No uncured concrete shall be allowed to come in contact with stream waters. Erosion control fencing shall be used to protect the stream channel from sedimentation. All damage to areas caused by the contractor’s construction activities or equipment shall be immediately reported to the Contracting Officer Representative (COR). The contractor shall be responsible for repairing all damaged areas.

D. APPLICABLE SPECIFICATIONS:

“Standard Specifications for Construction of Roads and Bridges on Federal Highway Projects” FP-14 and US Forest Service Supplemental Specifications.

E. SUBMITTALS:

CONSTRUCTION SCHEDULES

o A proposed schedule of work for the project construction shall be prepared by the Contractor and submitted to the COR.

o Contractor may submit “or Equal” precast concrete box culvert and footer configuration(s) that meet the specified load and flow capacities. One hard https://flh.fhwa.dot.gov/resources/specs/ copy and 1 digital copy of the submittal shall be provided to the COR prior to issuance of the Notice to Proceed.

o Contractor shall provide the manufacturers footing dimension and reinforcement design and shall submit 1 hard copy and 1 digital copy shop drawings to the COR for approval.

F. ACCESS ROAD:

o The primary road leading to FSR 665 is a county maintained gravel road.

o After construction operation has been completed, the contractor shall be required to repair all disturbed areas to the original condition or to the specified condition. Seed and mulch all disturbed areas (see seeding required on the drawings) o FSR-665A shall be closed at all times during the contract. The contractor shall be responsible for securing the road and preventing public vehicles from accessing the excavated areas.

G. BOX CULVERT AND RELIEF CULVERT:

o BOX CULVERTS: Culvert (FSR-665A): 10 foot span x 5 foot rise x 10 inch wall thickness four sided precast reinforced concrete box culvert.

o RELIEF CULVERT: 36 inch diameter by 36 foot length reinforced concrete pipe (RCP) culvert as shown in the plans.

o Box culvert and relief culvert shall meet or exceed applicable ASTM, AASHTO standards, for storm drain and highway loadings.

o Installation shall be made in accordance with the manufacturer’s recommendations and project plans.

H. HEADWALL/WINGWALLS:

o Headwalls and wingwalls shall be precast or site formed. In either case, the headwalls shall be designed by a registered P.E. If being site formed, the contractor shall follow all requirements for site poured concrete in the specifications to include: all submittals, approval of a lead technician, form design, materials testing and a pre-work conference. Headwall shall be minimum 6 inches higher than the top of box culvert. Wingwalls shall skew as indicated on plan sheets.

o Installation shall be made in accordance with the manufacturer’s recommendations.

I. CONSTRUCTION:

o General Notes:

• Motor Grader finish required on surface aggregate.

• A 4% Crown is the Forest Service standard.

• After construction operation has been completed, the contractor shall be required to repair all disturbed areas to the original condition or to the specified condition. Seed and mulch all disturbed areas (see seeding required on the drawings)

• Removed CMP culverts shall become property of the Contractor and shall be removed from government land.

• Use drawings and typical section for location, elevations, alignment, and other miscellaneous details.

• Technical questions shall be directed to the COR for clarification.
