PART 1 GENERAL

SCHEDULE OF ITEMS

This is a firm fixed price contract based on the Contractor Pricing provided in the schedule of items. Breakdown each lump-sum item into component parts of construction work for which progress payments may be requested. The total costs for the component parts of work shall equal the contract line item amount for that lump-sum item. The CO may request data to verify accuracy of dollar values. Include mobilization, general condition costs, overhead and profit in the total dollar value of unit price items and in the component parts of work for each lump-sum item, as described below. Do not include mobilization, general condition costs, general requirement costs, overhead, or profit as a separate item.

Do not break down unit price items. Use only the contract line item amount for unit price items.

The total cost of all items shall equal the contract sum. The Schedule of Items will form the basis for progress payments.

Other standard electronic forms will be considered in lieu of standard form specified; submit draft to the Contracting Officer for approval.

Forms filled out by hand will not be accepted.

Revise schedule to list approved Change Orders, with each Application For Payment.

APPLICATIONS FOR PROGRESS PAYMENTS

Payment Period: Submit at intervals stipulated in the Agreement.

Electronic media printout including equivalent information will be considered in lieu of standard form specified; submit sample to COR for approval.

Forms filled out by hand will not be accepted.

For each item, provide a column for listing each of the following:

Item Number (from schedule of items).

Description of work.

Previous Applications.

Work in Place and Stored Materials under this Application.

Authorized Change Orders.

Total Completed and Stored to Date of Application.

Percentage of Completion.

Balance to Finish.

Retainage.

Execute certification by signature of COR.

Use data from approved Schedule of Items. Provide dollar value in each column for each line item for portion of work performed and for stored products.

List each authorized Change Order as a separate line item, listing Change Order number and dollar amount as for an original item of work.

Include the following with the application:

Transmittal letter as specified for submittals in Section 013000.

Construction progress schedule, revised and current as specified in Section 013000.

Partial release of liens from major subcontractors and vendors.

Sustainable design documentation applicable to work for which application is being made; see Section 013329.

When the Contracting Officer requires substantiating information, submit data justifying dollar amounts in question. Provide one copy of data with cover letter for each copy of submittal. Show application number and date, and line item by number and description.

MODIFICATION PROCEDURES

Submit name of the individual authorized to receive change documents and who will be responsible for informing others in the Construction Contractor's employ or subcontractors of changes to Contract Documents.

For minor changes not involving an adjustment to the Contract Scope, Contract Price, or Contract Time, COR will issue instructions directly to Construction Contractor.

For other required changes, the Contracting Officer will issue a document instructing the Construction Contractor to proceed with the change, for subsequent inclusion in a Change Order.

The document will describe the required changes and will designate method of determining any change in Contract Scope, Contract Price, or Contract Time.

Promptly execute the change.

For changes for which advance pricing is desired, the Contracting Officer will issue a document that includes a detailed description of a proposed change with supplementary or revised drawings and specifications, a change in Contract Time for executing the change with a stipulation of any overtime work required and the period of time during which the requested price will be considered valid. Construction Contractor shall prepare and submit a fixed price quotation within 7 days.

Computation of Change in Contract Price:

For change requested by the Government for work falling under a fixed price contract, the amount will be based on Construction Contractor's price quotation.

For change requested by Construction Contractor, the amount will be based on the Construction Contractor's request for a Change Order as approved by Contracting Officer.

For pre-determined unit prices and quantities, the amount will based on the fixed unit prices.

Substantiation of Costs: Provide full information required for evaluation.

Provide the following data:

Quantities of products, labor, and equipment.

Taxes, insurance, and bonds.

Overhead and profit.

Justification for any change in Contract Time.

Credit for deletions from Contract, similarly documented.

Support each claim for additional costs with additional information:

Origin and date of claim.

Dates and times work was performed, and by whom.

Time records and wage rates paid.

Invoices and receipts for products, equipment, and subcontracts, similarly documented.

Execution of Change Orders: The Contracting Officer will issue Change Orders for signatures of parties.

After execution of Change Order, promptly revise Schedule of Values and Application for Payment forms to record each authorized Change Order as a separate line item and adjust the Contract Price.

Promptly revise progress schedules to reflect any change in Contract Time, revise sub-schedules to adjust times for other items of work affected by the change, and resubmit.

Promptly enter changes in Project Record Documents.

APPLICATION FOR FINAL PAYMENT

Prepare Application for Final Payment as specified for progress payments, identifying total adjusted Contract Price, previous payments, and sum remaining due.

Application for Final Payment will not be considered until the following have been accomplished:

All closeout procedures specified in Section 017000.

GENERAL ADMINISTRATIVE REQUIREMENTS

Comply with requirements of Section 017000 - Execution and Closeout Requirements for coordination of execution of administrative tasks with timing of construction activities.

Make the following types of submittals to COR in the PMIS:

Shop drawings, product data, and samples.

Test and inspection reports.

PART 2 PRODUCTS - Not Used

PART 3 EXECUTION

Project management information system (PMIS) All documents transmitted for purposes of administration of the contract are to be in electronic (PDF, MS Word, or MS Excel) format, as appropriate to the document, and transmitted via an Internet-based submittal service that receives, logs and stores documents, provides electronic stamping and signatures, and notifies addressees via email. COR is to be copied on all correspondence.

Besides submittals for review, information, and closeout, this procedure applies to Requests for Information (RFIs), progress documentation, contract modification documents (e.g. supplementary instructions, change proposals, change orders), applications for payment, field reports and meeting minutes, Construction Contractor's correction punchlist, and any other document any participant wishes to make part of the project record.

Construction Contractor and the Government are required to use this service.

It is Construction Contractor's responsibility to submit documents in allowable format.

Subcontractors, suppliers, the Government, and the Government's consultants are to be permitted to use the service at no extra charge.

Users of the service need an email address, internet access, and PDF review software that includes ability to mark up and apply electronic stamps, unless such software capability is provided by the service provider.

Paper document transmittals will not be reviewed; emailed electronic documents will not be reviewed.

All other specified submittal and document transmission procedures apply, except that electronic document requirements do not apply to samples or color selection charts.

PMIS Provider: The Construction Contractor’s system will be used throughout the project life.

Training: One, one-hour, web-based training session will be arranged for all participants, with representatives of COR and the Government participating including Government consultants; further training is the responsibility of the user of the service.

Project Closeout: The Contracting Officer will determine when to terminate the service for the project and is responsible for obtaining archive copies of files for the Government. All data documents and products of the PMIS will be the property of the Government.

The Government intends to use the system to manage the project and provide a common collaboration and interaction platform for all project team members, including Government staff, external partners and cooperators, architecture and engineering consultants, constructors and construction managers, suppliers, and subcontractors.

This system will be the USDA Forest Service’s system of record and be used for all infrastructure project and contract management functions. All information will be gathered, stored, routed, and used from within the system.

At each progress payment request and at project closeout, a snapshot in time will be taken and placed into Box, the Forest Service’s electronic file system and serve as the contract file of record.

Requirements:

External and Internal collaboration tool that can be used by both internal USDA Forest Service employees and external customers and vendors. Construction Contractor and A-E consultants are required to use this service.

Ability to track documents and correspondence with a history function or log through all project phases. Examples include, but are not limited to:

Shop Drawing Submittals Construction Submittals Requests for Information Change Requests Supplemental Instructions Field Reports Photos Contract Documents and Revisions Backup Materials for Invoices Master level scheduling that allows a broad overview of project progress. This is for major program and project milestones only. Constructors are responsible for providing detailed construction scheduling.

Mobile component for instantaneous communication and ability to make updates and take pictures on job sites using phones or tablets. Mobile apps need to include offline capacity and syncing when the device is near a signal again.

Centralized workflow and unlimited file capacity for the documents associated with the workflow.

Access to the application is based on role with fine grain permissions as well as separate log-in credentials for each user.

Robust and customizable reporting and dashboard features.

Audit trails for all actions in the system.

On-demand training for the system for all users.

Unlimited users.

Application will provide for management of user level access control to be administered by USDA Forest Service project leadership.

All data sets created, stored or manipulated by the solution design will be readily available for long term electronic storage and back up as part of official records requirements for government projects.

Project Closeout: Construction Contractor will determine when to terminate the service for the project after Substantial and is responsible for obtaining archive copies of files for the Government.

Record minutes and distribute copies within two days after meeting to participants, with two copies to COR, the Government, participants, and those affected by decisions made.

Project Schedule General: The Construction Contractor shall provide the project schedule after award and before the pre-construction meeting. The schedule shall include a detailed construction schedule with important milestones included for all phases.

Development and Updates: Fully develop the construction portion of the schedule and submit before the Preconstruction meeting or any construction begins. The project schedule shall be updated on a monthly basis throughout the entire contract period and until project substantial completion. The status date of each schedule update shall be 10 days before the progress payment request date.

Purpose: The purpose of the project schedule is to ensure adequate planning, coordination, scheduling, and reporting during execution of design and construction activities of the Construction Contractor. The project schedule will assist the Construction Contractor and the Government in monitoring the progress of the work, evaluating proposed changes, and processing the Construction Contractor's monthly progress payment requests.

Schedule Development: The project schedule shall cover the entire contract period. The late finish date of the project schedule shall be the same date as the established completion date of the contract.

The Construction Contractor shall use the Critical Path Method (CPM) with limited use of lead or lag durations between schedule activities. The Construction Contractor's project schedule shall consist of procurement activities (including mobilization, submittal, and the fabrication and delivery of key and long-lead procurement items) and construction activities.

The Construction Contractor's project schedule shall consist of, but not be limited to, the following for each activity:

Identify each and every activity number.

Concise description of the work represented by the activity (maximum 48 characters). Avoid the use of non-standard abbreviations. The work related to each activity shall be limited to one work trade.

Activity duration in whole working days, except for non-construction activities including mobilization, shop drawing and sample submittals, fabrication of materials, delivery of materials and equipment, and concrete curing.

In developing the project schedule, the Construction Contractor shall be responsible for ensuring that subcontractor work at all tiers, as well as its own work, is included in the project schedule.

The project schedule, as developed, shall show the sequence and interdependence of activities required for complete performance of the work. The Construction Contractor shall be responsible for ensuring all work sequences are logical and the project schedule shows a coordinated work plan. Proposed durations assigned to each activity shall be the Construction Contractor's best estimate of time required to complete the activity considering the scope and resources planned for the activity. Resource loading of each activity shall list all personnel by labor category and equipment type and capacity proposed to complete the activity in the duration shown. Include permit requirements and constraints. Seasonal weather conditions shall be considered and included in the planning and scheduling of all work influenced by high or low ambient temperatures, wind and/or precipitation to ensure completion of all work within the contract time.

Project Schedule Updates:

Monthly Updates: The monthly updating of the Project Schedule shall be an integral part and basic element of the estimate upon which progress payments shall be made under this contract. If, in the judgment of the CO, the Construction Contractor fails or refuses to provide a complete schedule update or revision as specified hereinafter, the Construction Contractor shall be deemed to have not provided the required information upon which the progress payment may be made, and shall be subject to retainage of a portion of the payment.

Time Impact Analysis for Contract Modifications, Changes, Delays, and Construction Contractor Requests Requirements: When contract modifications or changes are initiated, delays are experienced, or the Construction Contractor desires to revise the project schedule, the Construction Contractor shall submit to the CO a written time impact analysis illustrating the influence of each modification, change, delay, or Construction Contractor request on the contract time.

Time Extensions: Activity delays shall not automatically mean that an extension of the contract time is warranted or due the Construction Contractor. It is possible that a modification, change, or delay will not affect existing critical activities or cause non-critical activities to become critical. A modification, change, or delay may result in only absorbing a part of the available total float that may exist within an activity chain of the project schedule, thereby not causing any effect on the contract time. Time extensions will be granted in accordance with the terms of the contract.

Float: Float is not for the exclusive use or benefit of either the USDA Forest Service or the Construction Contractor. Extension of the contract time will be granted only to the extent the equitable time adjustments to the activity or activities affected by the modification, change, or delay exceeds the total (positive or zero) float available on a particular activity.

PROGRESS PHOTOGRAPHS

Photography Type: Digital; electronic files.

Provide photographs of site and construction throughout progress of work produced by an experienced photographer, acceptable to COR.

Photograph all elements and assemblies before enclosure.

Digital Photographs: 24 bit color, minimum resolution of 8 megapixels, in JPG format; provide files unaltered by photo editing software.

Delivery Medium: Upload to PMIS.

File Naming: Include project identification, date and time of view, and view identification.

Point of View Sketch: Include digital copy of point of view sketch with each electronic submittal; include point of view identification in each photo file name.

PDF File: Assemble all photos into printable pages in PDF format, with 2 to 3 photos per page, each photo labeled with file name; one PDF file per submittal.

COORDINATION DRAWINGS

Provide information required for preparation of coordination drawings.

Review drawings prior to submission to COR.

REQUESTS FOR INTERPRETATION (RFI)

Definition: A request seeking one of the following:

An interpretation, amplification, or clarification of some requirement of Contract Documents arising from inability to determine from them the exact material, process, or system to be installed; or when the elements of construction are required to occupy the same space (interference); or when an item of work is described differently at more than one place in Contract Documents.

A resolution to an issue which has arisen due to field conditions and affects design intent.

Whenever possible, request clarifications at the next appropriate project progress meeting, with response entered into meeting minutes, rendering unnecessary the issuance of a formal RFI.

Preparation: Prepare an RFI immediately upon discovery of a need for interpretation of Contract Documents. Failure to submit a RFI in a timely manner is not a legitimate cause for claiming additional costs or delays in execution of the work.

Prepare a separate RFI for each specific item.

Review, coordinate, and comment on requests originating with subcontractors and/or materials suppliers.

Do not forward requests which solely require internal coordination between subcontractors.

Prepare in a format and with content acceptable to the Government.

Prepare using software provided by the PMIS.

Combine RFI and its attachments into a single electronic file. PDF format is preferred.

Reason for the RFI: Prior to initiation of an RFI, carefully study all Contract Documents to confirm that information sufficient for their interpretation is definitely not included.

Include in each request Construction Contractor's signature attesting to good faith effort to determine from Contract Documents information requiring interpretation.

Unacceptable Uses for RFIs: Do not use RFIs to request the following::

Approval of submittals (use procedures specified elsewhere in this section).

Approval of substitutions (see Section - 016000 - Product Requirements) Changes that entail change in Contract Time and Contract Price (comply with provisions of the Conditions of the Contract).

Different methods of performing work than those indicated in the Contract Drawings and Specifications (comply with provisions of the Conditions of the Contract).

Improper RFIs: Requests not prepared in compliance with requirements of this section, and/or missing key information required to render an actionable response. They will be returned without a response.

Frivolous RFIs: Requests regarding information that is clearly indicated on, or reasonably inferable from, Contract Documents, with no additional input required to clarify the question. These will be returned without a response.

Content: Include identifiers necessary for tracking the status of each RFI, and information necessary to provide an actionable response.

Official Project name and number, and any additional required identifiers established in Contract Documents.

The Contracting Officer's, COR's, and Construction Contractor's names.

Discrete and consecutive RFI number, and descriptive subject/title.

Issue date, and requested reply date.

Reference to particular Contract Document(s) requiring additional information/interpretation. Identify pertinent drawing and detail number and/or specification section number, title, and paragraph(s).

Annotations: Field dimensions and/or description of conditions which have engendered the request.

Construction Contractor's suggested resolution: A written and/or a graphic solution, to scale, is required in cases where clarification of coordination issues is involved, for example; routing, clearances, and/or specific locations of work shown diagrammatically in Contract Documents. If applicable, state the likely impact of the suggested resolution on Contract Time or the Contract Sum.

Attachments: Include sketches, coordination drawings, descriptions, photos, submittals, and other information necessary to substantiate the reason for the request.

RFI Log: Prepare and maintain a tabular log of RFIs for the duration of the project in the PMIS.

Indicate current status of every RFI. Update log promptly and on a regular basis.

Note dates of when each request is made, and when a response is received.

Highlight items requiring priority or expedited response.

Highlight items for which a timely response has not been received to date.

Identify and include improper or frivolous RFIs.

Review Time: COR will respond and return RFIs to Construction Contractor within 15 calendar days of receipt. For the purpose of establishing the start of the mandated response period, RFIs received after 12:00 noon on a regular working day will be considered as having been received on the following regular working day.

Response period may be shortened or lengthened for specific items, subject to mutual agreement, and recorded in a timely manner in progress meeting minutes.

Responses: Content of answered RFIs will not constitute in any manner a directive or authorization to perform extra work or delay the project. If in Construction Contractor's belief it is likely to lead to a change to Contract Scope, Contract Price, or Contract Time, promptly issue a notice to this effect, and follow up with an appropriate Change Order request to the Government.

Response may include a request for additional information, in which case the original RFI will be deemed as having been answered, and an amended one is to be issued forthwith. Identify the amended RFI with an R suffix to the original number.

Do not extend applicability of a response to specific item to encompass other similar conditions, unless specifically so noted in the response.

Upon receipt of a response, promptly review and distribute it to all affected parties, and update the RFI Log.

Notify COR within seven calendar days if an additional or corrected response is required by submitting an amended version of the original RFI, identified as specified above.

SUBMITTAL SCHEDULE

Submit to COR for review a schedule for submittals in the PMIS.

Submit at the same time as the preliminary schedule.

Coordinate with Construction Contractor's construction schedule and schedule of values.

Format schedule to allow tracking of status of submittals throughout duration of construction.

Arrange information to include scheduled date for initial submittal, specification number and title, submittal category (for review or for information), description of item of work covered, and role and name of subcontractor.

Account for time required for preparation, review, manufacturing, fabrication and delivery when establishing submittal delivery and review deadline dates.

For assemblies, equipment, systems comprised of multiple components and/or requiring detailed coordination with other work, allow for additional time to make corrections or revisions to initial submittals, and time for their review.

SUBMITTALS FOR REVIEW

When the following are specified in individual sections, submit them for review:

Product data.

Shop drawings.

Samples for selection.

Samples for verification.

Submit to COR for review for the limited purpose of checking for compliance with information given and the design concept expressed in Contract Documents.

Samples will be reviewed for aesthetic, color, or finish selection.

After review, provide copies and distribute in accordance with SUBMITTAL PROCEDURES article below and for record documents purposes described in Section 017800 - Closeout Submittals.

SUBMITTALS FOR INFORMATION

When the following are specified in individual sections, submit them for information:

Design data.

Sustainability performance submittals and reports.

Certificates.

Test reports.

Inspection reports.

Manufacturer's instructions.

Manufacturer's field reports.

Field engineering daily reports.

Other types indicated.

Submit for COR's knowledge as contract administrator or for the Government.

SUBMITTALS FOR PROJECT CLOSEOUT

Submit Correction Punch List for Substantial Completion.

Submit Final Correction Punch List for Substantial Completion.

When the following are specified in individual sections, submit them at project closeout in compliance with requirements of Section 017800 - Closeout Submittals:

Project record documents.

Operation and maintenance data.

Warranties.

Bonds.

Other types as indicated.

Submit for the Government's benefit during and after project completion.

NUMBER OF COPIES OF SUBMITTALS

Electronic Documents: Submit one electronic copy in PDF format; an electronically-marked up file will be returned. Create PDFs at native size and right-side up; illegible files will be rejected.

Samples: Submit the number specified in individual specification sections; one of which will be retained by COR.

After review, produce duplicates.

Retained samples will not be returned to Construction Contractor unless specifically so stated.

SUBMITTAL PROCEDURES

General Requirements:

Use a single transmittal for related items.

Submit separate packages of submittals for review and submittals for information, when included in the same specification section.

Transmit using PMIS form.

Sequentially identify each item. For revised submittals use original number and a sequential numerical suffix.

Identify: Project; Construction Contractor; subcontractor or supplier; pertinent drawing and detail number; and specification section number and article/paragraph, as appropriate on each copy.

Apply Construction Contractor's stamp, signed or initialed certifying that review, approval, verification of products required, field dimensions, adjacent construction work, and coordination of information is in accordance with the requirements of the work and Contract Documents.

Submittals from sources other than the Construction Contractor, or without Construction Contractor's stamp will not be acknowledged, reviewed, or returned.

Deliver each submittal on date noted in submittal schedule, unless an earlier date has been agreed to by all affected parties, and is of the benefit to the project.

Upload submittals in electronic form to PMIS website.

Schedule submittals to expedite the Project, and coordinate submission of related items.

For each submittal for review, allow 15 days excluding delivery time to and from the Construction Contractor.

For sequential reviews involving COR's consultants, the Government, or another affected party, allow an additional 7 days.

For sequential reviews involving approval from authorities having jurisdiction (AHJ), in addition to COR's approval, allow an additional 30 days.

Identify variations from Contract Documents and product or system limitations that may be detrimental to successful performance of the completed work.

Provide space for Construction Contractor and COR review stamps.

When revised for resubmission, identify all changes made since previous submission.

Distribute reviewed submittals. Instruct parties to promptly report inability to comply with requirements.

Incomplete submittals will not be reviewed, unless they are partial submittals for distinct portion(s) of the work, and have received prior approval for their use.

Submittals not requested will be recognized, and will be returned "Not Reviewed", Product Data Procedures:

Submit only information required by individual specification sections.

Collect required information into a single submittal.

Submit concurrently with related shop drawing submittal.

Do not submit (Material) Safety Data Sheets for materials or products.

Submit sustainable design reporting submittals under separate cover.

Shop Drawing Procedures:

Prepare accurate, drawn-to-scale, original shop drawing documentation by interpreting Contract Documents and coordinating related work.

Do not reproduce Contract Documents to create shop drawings.

Generic, non-project-specific information submitted as shop drawings do not meet the requirements for shop drawings.

Samples Procedures:

Transmit related items together as single package.

Identify each item to allow review for applicability in relation to shop drawings showing installation locations.

Include with transmittal high-resolution image files of samples to facilitate electronic review and approval. Provide separate submittal page for each item image.

SUBMITTAL REVIEW

Submittals for Review: COR will review each submittal, and approve, or take other appropriate action.

Submittals for Information: COR will acknowledge receipt and review. See below for actions to be taken.

COR's actions will be reflected by marking each returned submittal using virtual stamp on electronic submittals.

Notations may be made directly on submitted items and/or listed on appended Submittal Review cover sheet.

COR's and consultants' actions on items submitted for review:

Authorizing purchasing, fabrication, delivery, and installation:

Approved: Acceptable with no corrections Approved as Noted, Resubmission not required”: Minor notes or clarifications required. All comments are clear and no further review is required. Address all review comments when proceeding with the Work. At Construction Contractor's option, submit corrected item, with review notations acknowledged and incorporated.

Approved as Noted, Resubmit for Record: Minor notes or clarifications required. No further review is needed. Resubmit corrected item, with review notations acknowledged and incorporated. Resubmit separately, or as part of project record documents.

Not Authorizing fabrication, delivery, and installation:

Revise and Resubmit: Rejected as not in accordance with the Contract or requiring major correction or clarifications. The Government will identify the reasons for not approving. Clearly identify changes when submitting revision.

Resubmit revised item, with review notations acknowledged and incorporated.

COR's and consultants' actions on items submitted for information:

SUBMITTALS

See Section 013000 - Administrative Requirements, for submittal procedures.

Construction Quality Control Plan: prior to the Preconstruction meeting, submit for approval a written Construction Contractor Quality Control Plan.

If the plan requires any revisions or corrections, the Construction Contractor shall resubmit the plan within ten (10) days.

The Government reserves the right to require changes in the plan during the contract period as necessary to obtain the quality specified.

No change in the approved plan may be made without written concurrence by the CO.

The plan shall include:

A list of personnel responsible for quality control and assigned duties. Include each person's qualifications.

A copy of a letter of direction to the Construction Contractor's Quality Control Supervisor outlining assigned duties.

Names, qualifications, and descriptions of laboratories to perform sampling and testing, and samples of proposed report forms.

Methods of performing, documenting, and enforcing quality control of all work.

Methods of monitoring and controlling environmental pollution and contamination as required by regulations and laws.

Test Reports: After each test/inspection, promptly submit two copies of report to the Government and to Construction Contractor.

Include:

Date issued.

Project title and number.

Name of inspector.

Date and time of sampling or inspection.

Identification of product and specifications section.

Location in the Project.

Type of test/inspection.

Date of test/inspection.

Results of test/inspection.

Compliance with Contract Documents.

When requested by COR, provide interpretation of results.

Test report submittals are for COR's knowledge as contract administrator for the limited purpose of assessing compliance with information given and the design concept expressed in the Contract Documents, or for the Government's information.

Certificates: When specified in individual specification sections, submit certification by the manufacturer and Construction Contractor or installation/application subcontractor to COR, in quantities specified for Product Data.

Indicate material or product complies with or exceeds specified requirements. Submit supporting reference data, affidavits, and certifications as appropriate.

Manufacturer's Instructions: When specified in individual specification sections, submit printed instructions for delivery, storage, assembly, installation, start-up, adjusting, and finishing, for the Government's information. Indicate special procedures, perimeter conditions requiring special attention, and special environmental criteria required for application or installation.

Manufacturer's Field Reports: Submit reports for COR's benefit as contract administrator or for the Government.

Submit for information for the limited purpose of assessing compliance with information given and the design concept expressed in the Contract Documents.

Erection Drawings: Submit drawings for COR's benefit as contract administrator or for the Government.

Submit for information for the limited purpose of assessing compliance with information given and the design concept expressed in the Contract Documents.

Data indicating inappropriate or unacceptable Work may be subject to action by COR or the Government.

Quality Assurance Testing Agency Qualifications:

Prior to start of work, submit agency name, address, and telephone number, and names of full time registered Engineer and responsible officer.

Submit copy of report of laboratory facilities inspection made by NIST Construction Materials Reference Laboratory during most recent inspection, with memorandum of remedies of any deficiencies reported by the inspection.

Qualification Statement: Provide documentation showing testing laboratory is accredited under IAS AC89.

Designer Qualifications: Where professional engineering design services and design data submittals are specifically required of Construction Contractor by Contract Documents, provide services of a Professional Engineer experienced in design of this type of work and licensed in the State in which the Project is located.

Construction Contractor's Quality Control (CQC) Plan:

Prior to start of Construction, submit a comprehensive plan describing how contract deliverables will be produced. Tailor CQC plan to specific requirements of the project. Include the following information:

Management Structure: Identify personnel responsible for quality. Include a chart showing lines of authority.

Include qualifications (in resume form), duties, responsibilities of each person assigned to CQC function.

Management Approach: Define, describe, and include in the plan specific methodologies used in executing the work.

Management and control of documents and records relating to quality.

Communications.

Coordination procedures.

Resource management.

Process control.

Inspection and testing procedures and scheduling.

Control of noncomplying work.

Tracking deficiencies from identification, through acceptable corrective action, and verification.

Control of testing and measuring equipment.

Project materials certification.

Managerial continuity and flexibility.

The Government will not make a separate payment for providing and maintaining a Quality Control Plan. Include associated costs in the Contract Price.

Acceptance of the plan is required prior to start of construction activities not including mobilization work. the Government's acceptance of the plan will be conditional and predicated on continuing satisfactory adherence to the plan. the Government reserves the right to require Construction Contractor to make changes to the plan and operations, including removal of personnel, as necessary, to obtain specified quality of work results.

Quality-Control Personnel Qualifications. Engage a person with requisite training and experience to implement and manage quality assurance (QA) and quality control (QC) for the project.

REFERENCES AND STANDARDS

For products and workmanship specified by reference to a document or documents not included in the Project Manual, also referred to as reference standards, comply with requirements of the standard, except when more rigid requirements are specified or are required by applicable codes.

Comply with reference standard of date of issue current on date of Contract Documents, except where a specific date is established by applicable code.

Obtain copies of standards where required by product specification sections.

Maintain copy at project site during submittals, planning, and progress of the specific work, until Substantial Completion.

Should specified reference standards conflict with Contract Documents, request clarification from COR before proceeding.

Neither the contractual relationships, duties, or responsibilities of the parties in Contract nor those of COR shall be altered from Contract Documents by mention or inference otherwise in any reference document.

Testing and Inspection Agencies and Services The Construction Contractor shall employ and pay for services of an independent testing agency to perform testing and inspection.

Employment of agency in no way relieves Construction Contractor of obligation to perform Work in accordance with requirements of Contract Documents.

Construction Contractor Employed Agency:

Testing agency: Comply with requirements of ASTM E329, ASTM E543, ASTM E699, ASTM C1021, ASTM C1077, ASTM C1093, and ASTM D3740.

Inspection agency: Comply with requirements of ASTM D3740 and ASTM E329.

Laboratory Qualifications: Accredited by IAS according to IAS AC89.

Laboratory: Authorized to operate in the State in which the Project is located.

Laboratory Staff: Maintain a full time registered Engineer on staff to review services.

Testing Equipment: Calibrated at reasonable intervals either by NIST or using an NIST established Measurement Assurance Program, under a laboratory measurement quality assurance program.

PART 2 PRODUCTS - Not Used

PART 3 EXECUTION

CONTROL OF INSTALLATION

Monitor quality control over suppliers, manufacturers, products, services, site conditions, and workmanship, to produce work of specified quality.

Comply with manufacturers' instructions, including each step in sequence.

Should manufacturers' instructions conflict with Contract Documents, request clarification from COR before proceeding.

Comply with specified standards as minimum quality for the work except where more stringent tolerances, codes, or specified requirements indicate higher standards or more precise workmanship.

Have work performed by persons qualified to produce required and specified quality.

Verify that field measurements are as indicated on shop drawings or as instructed by the manufacturer.

Secure products in place with positive anchorage devices designed and sized to withstand stresses, vibration, physical distortion, and disfigurement.

MOCK-UPS

Tests shall be performed under provisions identified in this section and identified in the respective product specification sections.

Assemble and erect specified items with specified attachment and anchorage devices, flashings, seals, and finishes.

COR will use accepted mock-ups as a comparison standard for the remaining Work.

Where mock-up has been accepted by COR and is specified in product specification sections to be removed, protect mock-up throughout construction, remove mock-up and clear area when directed to do so by COR.

TOLERANCES

Monitor fabrication and installation tolerance control of products to produce acceptable Work. Do not permit tolerances to accumulate.

Comply with manufacturers' tolerances. Should manufacturers' tolerances conflict with Contract Documents, request clarification from COR before proceeding.

Adjust products to appropriate dimensions; position before securing products in place.

TESTING AND INSPECTION

Testing Agency Duties:

Provide qualified personnel at site. Cooperate with COR and Construction Contractor in performance of services.

Perform specified sampling and testing of products in accordance with specified standards.

Ascertain compliance of materials and mixes with requirements of Contract Documents.

Promptly notify COR and Construction Contractor of observed irregularities or non-compliance of Work or products.

Perform additional tests and inspections required by COR.

Submit reports of all tests/inspections specified.

Limits on Testing/Inspection Agency Authority:

Agency may not release, revoke, alter, or enlarge on requirements of Contract Documents.

Agency may not approve or accept any portion of the Work.

Agency may not assume any duties of Construction Contractor.

Agency has no authority to stop the Work.

Construction Contractor Responsibilities:

Deliver to agency at designated location, adequate samples of materials proposed to be used that require testing, along with proposed mix designs.

Cooperate with laboratory personnel, and provide access to the Work and to manufacturers' facilities.

Provide incidental labor and facilities:

To provide access to Work to be tested/inspected.

To obtain and handle samples at the site or at source of Products to be tested/inspected.

To facilitate tests/inspections.

To provide storage and curing of test samples.

Notify COR and laboratory 24 hours prior to expected time for operations requiring testing/inspection services.

Employ services of an independent qualified testing laboratory and pay for additional samples, tests, and inspections required by Construction Contractor beyond specified requirements.

Arrange with the Government's agency and pay for additional samples, tests, and inspections required by Construction Contractor beyond specified requirements.

Re-testing required because of non-compliance with specified requirements shall be performed by the same agency on instructions by COR.
