INDIAN HEALTH SERVICE

PHOENIX INDIAN MEDICAL CENTER

Statement of Work

Background The Phoenix Indian Medical Center (PIMC) is a 1.2M SF campus comprised of a hospital tower and fifteen (15) outpatient care and support buildings. The campus is located at 4212 North 16th Street, Phoenix, Arizona 85016. The PIMC hospital tower (Building 233) is a five-story, Joint Commission (TJC) accredited, 127-bed hospital employing 1,200 staff that primarily serves a patient population of 140,000 Native Americans in the Phoenix metropolitan area. The hospital tower was built in 1970 and is approximately 163,000 SF. PIMC is a federal facility that must follow federal law and regulations (IE OSHA) and also must follow county laws and regulations that the federal government has given jurisdiction to (IE Maricopa Air Quality). PIMC must comply with City of Phoenix regulations regarding city services the hospital impacts such as sewer and stormwater discharge but generally is not required to follow other city laws/regulations.

The facility currently utilizes a modified version of the original ASHE ICRA matrix and permit but is looking to implement a new pre-construction risk assessment policy utilizing ASHE ICRA 2.0. PIMC currently utilizes in house construction staff to set up ICRA walls and establish/maintain negative pressure when required by the ICRA permit. The current in house staff does not have the equipment or expertise necessary to fully comply with ICRA 2.0. PIMC is also looking to implement a new water intrusion policy requiring equipment and training that the in-house staff does not currently possess. The in house staff have capacity to perform minor (Class II) asbestos work however the program struggles with ensuring asbestos training certificates are kept up to date. Asbestos on the PIMC campus is limited to sections of VCT flooring/mastic that have not yet been abated and tape around HVAC ductwork. A full site survey for asbestos was conducted in 2003 and there is a current contract in place to completely update the map. The asbestos location map is expected to be fully updated in January of 2026. Lead exists in the facility in the form of 4”x4” and smaller ceramic tile glazing in restrooms, locker rooms, showers, soiled utility rooms, etc. The tile glazing contains low levels of lead.

Objective The objective of this contract is to provide PIMC with the support necessary to comply with PIMC policies concerning pre-construction risk assessment, asbestos safety, and water intrusion:

1) To assist PIMC in complying with the requirements of ICRA 2.0 as outlined by the American Society of Healthcare Engineers (ASHE) and to assist in house construction staff with additional construction labor once ICRA 2.0 requirements have been established.

2) To deliver 24 hour a day, seven day a week, rapid response to dry out clean and contaminated water leaks at PIMC in compliance with ANSI/IICRC S500-2021, “Standard and Reference Guide for Professional Water Damage Restoration”.

3) To deliver industrial hygiene services and recommendations for remediation of materials affected by mold and contaminated water. Industrial hygienist to be certified as a Certified Industrial Hygienist (CIH) from ANSI/NCAA and hold a Mold Remediation Specialist (MRS) certification through IICRC

4) To perform demolition and remediation of building materials containing asbestos/lead and/or affected by mold and/or contaminated water and to assist the facility with construction labor to replace demolished areas.

All technicians performing drying and remediation tasks must hold an Applied Structural Drying Technician (ASD), Building Moisture Inspector (BMI), Commercial Drying Specialist (CDS), and/or Water Damage Restoration Technician (WRT) certification through IIRC.org. All technicians providing asbestos abatement must hold Class II/III worker certificates as appropriate. All work must be performed by sub-contractors and technicians with experience working in hospital occupancies.

Period of Performance Contract performance shall be for a base year and four (4) one (1) year option for renewal. If the Government exercises its option pursuant to FAR 52.217-9, “Option to extend the term of the contract”, the performance period will be extended in accordance with the following schedule:
