About this file
This is a Resident Bidder Affidavit required for Solicitation/Contract #RFB 758 1900000749, issued by an unspecified Kentucky state or local government agency. The affidavit establishes the mandatory requirements for bidders, offerors, and contractors claiming resident bidder status under Kentucky Revised Statutes (KRS) 45A.494(2). To qualify as a resident bidder, the entity must be authorized to transact business in the Commonwealth and must demonstrate compliance with three conditions for the one-year period prior to and through the date of contract advertisement: filing Kentucky income taxes, making payments to the Kentucky unemployment insurance fund, and maintaining an active Kentucky workers' compensation policy.
The bidding agency reserves the right to request supporting documentation to verify a bidder's resident status claim, and failure to provide such documentation upon request results in automatic disqualification or contract termination. The affidavit requires the bidder's signature, printed name, title, date, and company information, and must be notarized with the notary's seal and commission expiration date. This affidavit is a standard requirement form designed to ensure bidders meet Kentucky's resident bidder preferences and eligibility criteria.
