About this file
This is a Resident Bidder Affidavit document for Solicitation/Contract #RFB 758 1900000240, applicable to state and local contracting opportunities in Kentucky. The affidavit establishes the requirements and declaration process for bidders, offerors, and contractors seeking to claim resident bidder status under Kentucky Revised Statutes (KRS) 45A.494(2). Bidders must affirm under penalty of perjury that on the date the contract is first advertised, the entity is authorized to transact business in the Commonwealth and has maintained compliance with specific state requirements for the one-year period prior to and through the advertisement date.
To qualify for resident bidder status, entities must demonstrate that they have filed Kentucky income taxes, made payments to the Kentucky unemployment insurance fund established in KRS 341.49, and maintained an active Kentucky workers' compensation policy throughout the required period. The bidding agency reserves the right to request supporting documentation to verify a bidder's claim of resident status, and failure to provide such documentation upon request will result in bidder disqualification or contract termination. The affidavit must be notarized and executed by the authorized representative of the bidding entity.
