About this file

This is a Required Affidavit for Resident Bidder Status document associated with Solicitation/Contract #RFB 758 1900000749 for the Ohio River Asian Carp Removal Program in Kentucky. The affidavit establishes the requirements and verification process for bidders claiming resident bidder status under Kentucky Revised Statutes 45A.494(2). Bidders must swear under penalty of perjury that their entity is authorized to transact business in Kentucky and has continuously met three specific requirements for the one-year period prior to and through the contract advertisement date: filed Kentucky income taxes, made payments to the Kentucky unemployment insurance fund, and maintained an active Kentucky workers' compensation policy.

The Kentucky bidding agency reserves the authority to request documentation supporting any claim of resident bidder status, and failure to provide such documentation upon request will result in bidder disqualification or contract termination. The affidavit requires the bidder's signature, printed name, title, date, company name, and address, as well as notarization by a public notary with their seal and commission expiration date. This requirement applies to all individuals, partnerships, associations, corporations, and other business entities seeking to qualify for resident bidder preferences in this competitive solicitation process.
