About this file
This is a Required Affidavit for Resident Bidder Status document associated with RFB 758 2100000184. The affidavit establishes the criteria and certification requirements that bidders, offerors, and contractors must meet to claim resident bidder status in accordance with Kentucky Revised Statutes 45A.494(2). Bidders must affirm under penalty of perjury that their entity—whether an individual, partnership, association, corporation, or other business entity—meets three specific conditions as of the contract advertisement date: authorization to conduct business in the Commonwealth of Kentucky, filing of Kentucky income taxes for one year prior to and through the advertisement date, and maintenance of both Kentucky unemployment insurance fund payments and an active Kentucky workers' compensation policy throughout that same period.
The contracting agency reserves the right to request documentation to verify any bidder's resident status claim, with failure to provide such documentation upon request resulting in bidder disqualification or contract termination. The affidavit requires execution before a notary public, with the bidder providing signature, printed name, title, date, company name, and address, along with notarization including the notary's seal and commission expiration date.
