About this file

This is a Resident Bidder Affidavit required for Solicitation/Contract RFB 758 2100000196, applicable to state and local contracting opportunities in Kentucky. The affidavit establishes eligibility requirements for entities claiming resident bidder status, which provides a competitive advantage in the bidding process. Bidders must certify under penalty of perjury that their entity is authorized to transact business in the Commonwealth and has maintained specified compliance documentation for at least one year prior to and through the contract advertisement date.

To qualify for resident bidder status, entities must demonstrate that they have filed Kentucky income taxes, made payments to the Kentucky unemployment insurance fund, and maintained an active Kentucky workers' compensation policy throughout the required period. The bidding agency reserves the right to request supporting documentation to verify resident bidder claims, and failure to provide such documentation upon request will result in bidder disqualification or contract termination. The affidavit must be notarized and signed by an authorized representative of the bidding entity, with the notary public's seal and commission expiration date affixed to the document.
