About this file

This is a Resident Bidder Affidavit required for RFB 758 2100000166, applicable to state and local contract opportunities in the Commonwealth of Kentucky. The affidavit serves as a declaration enabling bidders to claim resident bidder status when submitting bids or proposals. Bidders must certify under penalty of perjury that their entity—whether an individual, partnership, association, corporation, or other business entity—meets specific residency requirements on the date the contract is first advertised or announced for bidding.

To qualify for resident bidder status, entities must demonstrate: authorization to transact business in Kentucky; filing of Kentucky income taxes for one year prior to and through the advertisement date; payments to the Kentucky unemployment insurance fund during the same period; and maintenance of an active Kentucky workers' compensation policy throughout that timeframe. The bidding agency retains the right to request supporting documentation to verify claimed resident bidder status, and failure to provide such documentation upon request results in bidder disqualification or contract termination. The affidavit requires the signature of an authorized representative, notarization, and completion of required identifying information including company name, address, and date of execution.
