About this file

This document is a Required Affidavit for Resident Bidder Status for RFB 758 2000000593 issued by an unnamed bidding agency in Kentucky. The affidavit establishes the eligibility criteria that bidders, offerors, and contractors must meet to claim resident bidder status when competing for state contracts. Bidders must certify under penalty of perjury that their entity is authorized to transact business in the Commonwealth of Kentucky and has maintained continuous compliance with state requirements for the one-year period preceding contract advertisement. These requirements include filing Kentucky income taxes, making payments to the Kentucky unemployment insurance fund as established under KRS 341.49, and maintaining an active Kentucky workers' compensation policy throughout the specified timeframe.

The bidding agency reserves the right to request documentation from any bidder claiming resident status to verify compliance with these requirements. Failure to provide requested supporting documentation will result in bidder disqualification or contract termination. The affidavit must be signed by an authorized representative of the bidding entity, notarized, and include the affiant's printed name, title, date, company name, and address. This certification mechanism ensures that resident bidder preferences under KRS 45A.494(2) are claimed only by entities meeting all statutory requirements.
