1. Will the sign-in sheet from site visit be available in addendum?

See addendum 1.

2. Please clarify if a superintendent can be a dual hat to cover the requirements of

SSHO? See response to question #6.

3. If no to the question above, will the SSHO need to be full-time for this project?

See response to question #6.

4. Please clarify if a superintendent can be a dual hat to cover the requirements of

QA/QC? See response to question #6.

5. If no to the question above, will the QA/QC need to be full time for this project?

See response to question #6.

6. Can a superintendent be a triple hat to cover the requirements of SSHO and

QA/QC? It is acceptable for the prime contractor’s superintendent to also serve as the QA/QC manager and safety official. Contractor proposed personnel shall meet training and experience requirements as outlined in the provided specifications.

7. Please confirm if tax should be included in our proposal? Yes

8. Is the use of temporary plastic wall panels (such as STARC panels) acceptable to meet ICRA requirements?

Use of STARC panels is acceptable to satisfy certain ICRA level barrier requirements. This project is anticipated to require level 2 ICRA requirements at a maximum. See attached ICRA template for reference to contractor requirements.

PCRA & ISLM documents are also included for reference to control measure requirements to be met by contractor. These forms are to be completed & distributed by project COR and available for inspection at all work areas during construction.

9. Will shutdowns be required to be done during off hours or weekends?

Contractor shall provide premium off-hours pricing for any necessary shutdown work associated with AH-38 located in the penthouse of the Main Hospital (Building 70).

MAU #3 related work can be performed during normal working hours 7am – 5pm.

10. Please confirm if there is assumed asbestos on any piping that is to be removed?

Confirmed, there is no asbestos in the AHU-38 or MAU-3 work areas.

11. Please confirm the extent of any duct cleaning scope if required.

Duct cleaning is not required or included in the project scope.

12. Please confirm MAU #3 coil connection pipe size. MAU #38 is provided.

Coil connection size will need to be coordinated with manufacturer representative.

13. Do you have the Model and Serial Numbers of the MUA’s that need new coils?

AHU #38 – Model: ERU-SS-WH-45000-IB-HM-CW-FF-460, Serial: 8353177.0040 MAU #3 – Model: NMN, Serial:25215

14. Do you have any pictures of the existing coils and MUA units?

See photos attached.

15. What access do we have to get the coils in and out of the mechanical rooms?

See responses in amendment 1 regarding access to freight elevator and potential crane pick location.

16. Do you have a piping detail we are to follow for the steam and condensate piping?

See detail attached.
