Past Performance Questionnaire Each offeror must submit (as part of the technical quote) Past Performance Information on a minimum of three (3) contracts performed for services comparable to the government's requirement in the last 5 years and must be contracts performed with similar scope and complexity, as this requirement. For each reference, include the following information. Please return this package with your quotation.

1. A brief narrative of why the referenced contract is relevant to this effort

2. The organizations history of successful completion of projects; history of producing high-quality; history of staying within budget.

3. The quality of cooperation and performance between your organization and its clients.

4. A discussion of any situations encountered on the identified contract and the corrective action taken by the Offeror.

Past Performance Questionnaire

The Offeror shall complete the company specific information on this form, contact their clients, forward this form to their clients and request that their clients complete it (verify/concur and score) and send it via email to Patrick Ferguson at patrick.ferguson@sba.gov and Tracie Gutierrez at tracie.gutierrez@sba.gov. This will expedite the response from each referenced source.

Two forms are provided to complete the evaluation: 1) Performance Rating Form; and 2) Supplemental Comment Form. Upon completion of this form, please submit it as an email attachment to the above listed individuals and in the subject line of the email it should read “73351022Q0054 – Past Performance Questionnaire – Insert Evaluated Company’s Name.”

The following standards shall be used in arriving at the rating.

Outstanding
Performance meets contractual requirements and exceeds many requirements that benefit the end user. Work was accomplished with few, if any, minor problems for which corrective actions taken by the contractor were highly effective.
Good
Performance meets contractual requirements and exceeds some requirements that benefit the end user. Work was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Acceptable
Performance meets contractual requirements. Work was accomplished with some minor problems for which corrective actions taken by the contractor were satisfactory.
Marginal
Performance does not meet some contractual requirements. Serious problems with contractor performance were experienced for which the contractor has either not yet identified corrective actions, or the corrective actions taken appear only marginally effective.
Unsatisfactory
Contractor’s performance was either marginal or did not meet customer expectations or contract requirements.
Other
If the element is not applicable, indicate with “N/A.” If no data has been obtained or additional comments are provided, please note in this column.

(For example: Was the contractor able to obtain in a timely manner the amount and type of personnel resources required to support the project, effectively train personnel to perform the work required for the project, and maintain the required workforce throughout the term of the contract?)

(For example: Was the contractor successful in planning and proposing realistic schedules, monitoring performance, completing work on time, and implementing corrections/changes in a timely manner?)
