About this file

This is a Contractor Responsibility Verification form based on Federal Acquisition Regulation (FAR) 9.103 and 9.104 requirements. The document serves as a template for prospective contractors to demonstrate their responsibility and qualification to receive federal contract awards.

The form requires contractors to provide evidence or confirm their capability across six key responsibility factors. For financial resources, contractors must submit profit/loss statements, balance sheets, lines of credit documentation, or a statement from a financial institution confirming sufficient financial resources. Regarding delivery requirements, contractors must confirm their ability to meet proposed schedules considering existing commitments. For performance record, contractors must provide a minimum of two past performance references from the last three years or a capability statement for new entrants, with an exemption for contractors with previous government CPARS and SPRS ratings. Business integrity and ethics must be verified through SAM.gov information certification, including confirmation of accurate exclusions and proceedings data. Contractors must also confirm adequate organizational structure with appropriate experience, accounting controls, and technical skills, as well as sufficient production capabilities including technical equipment and facilities. A company representative must certify the accuracy and completeness of all submitted information across all six factors.
