I. TABLE 1 – List of Analytical Methods Compatible for Direct Upload to Enterprise Environmental Safety and Occupational Health – Management Information System (EESOH-MIS)

SECTION 1.0 DESCRIPTION OF SERVICES. The Contractor shall provide all materials, transportation and any incidentals necessary to perform required laboratory analytical support services as defined in this performance work statement (PWS). The contractor must perform to the standards in this contract. Various analytical requirements include, but are not limited to:

Hazardous Waste Analysis

1.1. BASIC SERVICES. Work to be performed consists of laboratory analysis in support of MacDill Air Force Base's Hazardous Waste Program and Solid Waste Program. The Contractor shall furnish transportation needed to pick-up the samples described in Section 1.2 and provide the sample containers. Unless otherwise prearranged, samples shall be picked up during normal duty hours as described in Section 4.3.

1.2. SPECIFIC SERVICES. The analytical methods and parameters for the proposed pick–up test samples, which require analysis, are identified in Appendix B of this PWS.

1.2.1 Sampling, Analytical Support, and Collecting and Transportation of Samples. The Contractor shall provide support as follows:

1.2.1.1 Sampling. The Contractor shall provide their own transportation to all of the required sampling locations. Any required rescheduling shall be coordinated and approved through the COR. Extracted test samples will be analyzed per the analytical methods and parameters described in Appendix B at various base locations according to the proposed Projected Lab Support Waste Sample Locations identified in Appendix A of this PWS. Appendix A, Projected Lab Support Waste Sample Locations, provides the contractor with a guide of the possible lab sampling work MacDill AFB locations the contractor is expected to provide service during the contract period. This contract is based in an Indefinite Delivery Indefinite Quantity (IDIQ) lab sampling work completion. Therefore, Appendix A, of this PWS, is only intended to provide the contractor with an informational guidance tool of the future projected lab sampling work at MacDill AFB.

1.2.1.2. Analytical Support. The Contractor shall provide analytical laboratory support for MacDill AFB building facilities indicated in Appendix A of this PWS. The waste description and test description for each sample pick- up test sample is also identified in Appendix A of this PWS. Analytical support shall be provided according to the schedule provided, unless authorized by the COR and approved by the contracting office. The services shall include the identification of the parameters to be analyzed and shall be performed in accordance with methods identified in Appendix B of this PWS. Blank, standard, duplicate, and a spike analysis shall be conducted 100% of the time on each parameter analyzed for MacDill AFB. Analytical methods used must be compatible with the Air Force Enterprise Environmental, Safety and Occupational Health – Management Information System (EESOH-MIS). EESOH-MIS approved analytical methods are provided in Appendix I (Table 1).

1.2.1.3. Collection and Transportation of Samples. The Contractor shall provide all the necessary equipment to properly collect and transport samples for analysis. The collection and transportation of samples shall be accomplished in accordance with schedule provided in Appendix A. Any required rescheduling shall be coordinated and approved through the COR. After analysis of the samples, the Contractor shall properly dispose of the samples in accordance with Federal, State, and Local laws and regulations.

1.3 REPORTING REQUIREMENTS.

1.3.1 Finalized Analytical Reports. The Contractor shall provide the COR with the finalized written reports for the analyses described in paragraphs 1.3.1.1 thru 1.3.1.9 with the following information:

1.3.1.1 Parameter tested.

1.3.1.2 Results.

1.3.1.3 Date of sample collection.

1.3.1.4 Initials of collector.

1.3.1.5 Date analysis performed.

1.3.1.6 Analytical method.

1.3.1.7 Initials of chemist performing analysis.

1.3.1.8 Remarks indicating any departure from standard procedure or anything unusual about the sample, which might indicate a questionable result.

1.3.1.9 Quality assurance for the sample to include- laboratory sample #; Description (i.e., blank, standard, duplicate, or spike); result; percent recovery; time; date; and initials of chemist.

1.3.1.10 The Contractor shall email final written reports to the COR within 10 calendar days after the due date indicated in Appendix A of this PWS to the following email addresses: andrew.rider.2@us.af.mil

1.3.2. Record Keeping. In addition to the reports submitted to the COR, Contractor shall maintain a spreadsheet containing a written record history of each pick up test sample indicating the following documented information the first day of each month:

1.3.2.1 Shop, Bldg. Number, Waste Description, Test Description, Initial Attempted Pick Up Sample Date, Follow Up Attempted Pick Up Sample Dates, Pick Up Samples Not Completed due to Insufficient Waste Product Material, Final Email Written Report Dates Sent to COR, and Wide Area Work Flow (WAWF) Finalized Sample Reports Dates submitted for payment.

1.3.2.2 The contractor shall keep this documented information current to keep accurate track of all pick up sample tests taken during this contract performance period. The benefits of maintaining this information current and accurate is the avoidance of a possible Federal or State Environmental Notice of Violation (NOV) that could occur as a result of missed pick up sample tests or missed final written report submittal due dates.

1.3.2.3 Record Keeping for United States Environmental Protection Agency (EPA). In addition to the reports submitted to the COR, contractor shall maintain a written record of the analysis. MacDill AFB requires the contractor to provide analytical data in Staged Electronic Data Deliverables (SEDD) version 5.2 format (see link and figure below for information on SEDD). The data deliverable shall be SEDD Stage 2a which provides analytical test results and all sample-related quality control results (e.g., laboratory control sample performance associated with field samples). This is required so Air Force EESOH-MIS Hazardous Waste Module can be populated directly through upload of the sampling data from EPA SEDD program version 5.2. Electronic data must be provided in a format that allows direct upload of laboratory data to the EESOH-MIS database. This data shall be provided to the COR. The COR in turn will submit this contractor provided SEDD data to the Air Force EESOH-MIS.

1.4 LABORATORY QUALITY CONTROL.

1.4.1 Quality Control Plan: Contractor shall have on file and shall be required to submit its Quality Control Plan to 6 CES/CEIE CTR as part of its proposal and upon request by the COR or the Contracting Officer.

1.4.2 Quality Control Validation. The Contractor shall conduct all quality control, sample custody, calibration of instruments, and data validation according to EPA protocols.

1.4.3 Quality Control Methods. Utilize effective methods for maintaining chain of custody samples, analysis of samples in accordance with establishing test procedures, and reporting of analytical results.

1.4.4 Quality Control Documentation. The Contractor shall submit to the government, laboratory quality control documentation indicated in paragraphs 1.4.4.1 through 1.4.4.2.5 of this PWS.

1.4.4.1 Sample Custody and Holding Procedures. Follow EPA and OSHA sample chain of custody and holding time regulations from the time of sample collection at MAFB, until completion of analysis at the Contractor's laboratory. The Contractor shall document the procedures used to accomplish sample transportation from MacDill AFB to the Contractor's laboratory, preparation methodology, sample container identification such as marking and labeling, field tracking forms to be used, name and title of designated sample custodian, and how sample custody is maintained until completion of analysis.

1.4.4.2 Quality Control Objectives for Measurement Data. The Contractor shall provide a list of all parameters that will be tested, the reference of the method to be used (EPA or other), and method detection limit, along with precision, accuracy, percent recovery, and competition objectives as percent of samples analyzed. The Contractor shall provide control charts and the percent of analysis in each category; above and below the control limit, between the warning limit and control limit, and below the warning limit, and indicate what these limits are.

1.4.4.2.1 Calibration Procedures and Frequency. The Contractor shall provide the frequency and method of calibration of all instruments and equipment, including temperature dependent equipment, i.e., ovens, refrigerators.

1.4.4.2.2 Preparation of Reagent Standards. The Contractor shall include the preparation of reagent standards to be used for laboratory analyses.

1.4.4.2.3 Data Reduction, Validation and Reporting. The Contractor shall include the procedures to be used to reduce, validate, and report data generated in the laboratory.

1.4.4.2.4 Performance and System Audits and Frequency. The contractor shall provide the results of any external performance audits or inspections made on the laboratory within the past five years by EPA or state Inspectors.

1.4.4.2.5 Internal Quality Control Checks and Frequency. The contractor shall include a description of how spike, duplicate, blank and standard analysis shall be used to maintain quality control.

1.4.5 Special Qualifications. The Contractor shall ensure that the laboratory has all the following certifications, and that they are current prior to award. The Contractor shall maintain all applicable certifications throughout the entire contract period.

1.4.5.1 The Contractor shall provide proof of certification of the laboratory by the Florida Department of Environmental Quality (FDEQ) Laboratory Accreditation Program and the National Environmental Laboratory Accreditation Program (NELAP) under the Florida Department of Health will be required in all of the applicable categories of sample analysis.

Note: Contractor may obtain State of Florida accreditation upon contract award; however, NELAP accreditations in states other than Florida must be acceptable/approved by the Florida Department of Health.

1.4.5.2 The contractor shall use a DOD ELAP (Environmental Laboratory Accreditation Program) accredited lab. The laboratory shall follow the DOD QSM (Quality System Manual) v.5.0.

SECTION 2.0 SERVICES SUMMARY. The Services Delivery Summary identifies the performance objectives and performance thresholds for all critical tasks associated with providing the services outlined in this PWS. This section contains only those items considered most important for mission accomplishment. The performance objectives are those services/tasks required in the PWS. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. The thresholds are critical to mission success. The Government expects the Contractor to perform at the acceptable levels throughout the life of the contract.

Contractor Personnel. The contractor shall provide an onsite point of contact (POC) to the contracting officer for all contractual matters. The contractor’s POC shall be fully knowledgeable in all aspects of the contract to include paragraphs 1.1, 1.2, 1.3, 1.4, Section 2, Section 3 and Section 4 of this PWS.

SECTION 3.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES.

SECTION 4.0 GENERAL INFORMATION

4.1. Qualifications and Certifications: The Contractor shall provide a copy of all documentation indicated in paragraphs

1.4.4 and 1.4.5 of this PWS for review and approval.

4.2. Performance Period: The period of performance for this requirement will consist of one - five year ordering period.

4.3. Hours of Operation. The contractor is responsible for conducting business, between the hours of 0730 – 1630 Monday through Friday except Federal holidays or when the government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. The Contractor must maintain at all times an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons. When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential.

4.4. Federal Holidays: The Contractor shall observe the same holidays observed by the Government, as follows:

4.5. Notification: Contractor shall provide a tentative work schedule after each delivery order request and shall provide exact time and dates of work one week prior to commencement of work. The work schedule shall be coordinated with the 6th Civil Engineer Squadron Field Representative (6 CES/CEIE CTR). If 6 CES/CEIE CTR is not notified of scheduled work, the Contractor shall be required to revisit the site with the 6 CES/CEIE and uncover the work for inspection and acceptance at no additional cost to the government.

4.6. Interruption/Utility Shutdowns: No utility interruptions are expected with the scope of work. If unexpected utility interruptions are required, they shall be accomplished by 6th Civil Engineer Squadron personnel, only as directed by the government and never on a Saturday. Request for interruption shall be submitted to 6 CES/CEIE CTR in writing five (5) working days prior to the scheduled interruption.

4.7. Inspection and Acceptance: 6 CES/CEIE CTR will act as the Contracting Officer’s Representative (COR) field representative and will evaluate the Contractor’s performance to ensure services are received. COR will evaluate the contractor’s performance through intermittent on-site inspections and receipt of complaints from 6CES/CEIE CTR. COR may inspect each task as completed or increase the number of quality assurance inspections if deemed appropriate because of repeated performance failures or because of repeated customer complaints. COR shall make determination of the validity of customer complaint(s) from 6CES/CEIE CTR.

4.8 Contractor Identification: Contractor personnel shall at all times, display a distinctive badge or wear company uniforms provided by the Contractor, identifying such personnel as employees of the Contractor and shall observe and otherwise be subject to such security regulations as are in effect for the particular premises involved. The Contractor shall provide vehicle description and license plate numbers of all Contractor-owned vehicles as well as all required employee information requiring access to MacDill AFB when requesting vehicle/employee passes. All Contractor personnel shall possess at all times, the AF Form 75, Visitor/Vehicle Pass, for each employee working on MacDill AFB.

4.9 Entry Control: Contractor shall restrict unauthorized entrance of persons and vehicles into Project site and existing facilities and allow entrance only to authorized persons with proper identification. In addition, the contractor shall maintain a log of workers and visitors, to be made available to 6 CES/CEIE CTR and COR on request.

4.10 Restrictions: The Contractor shall not allow cameras to be on the site nor photographs to be taken without the written permission from 6th Security Forces Squadron.

4.11 Hazardous Material: All HAZMAT contemplated for use or brought on base for the purpose of this contract, must be coordinated through the HAZMAT Pharmacy, located in bldg. 49 at Paperclips, Etc. Contractor must provide the Contracting Officer’s Representative with a copy of the MSDS (Material Safety Data Sheet) prior to using the HAZMAT and maintain a copy at the work site at all times that work is being performed.

4.12 Contractor Personnel: The contractor shall provide an on-site point of contact to the Contracting Officer for all contractual matters. This point of contact shall be available within 24 hours during normal hours of operation to meet with government personnel to discuss problem areas. The contractor’s point of contact shall be fully knowledgeable in all aspects of the contract to include paragraphs 1.1, 1.2, 1.3, 1.4, section 2, section 3, and section 4 of this PWS. Contractor personnel shall present a neat appearance and be easily recognized. This shall be accomplished by wearing uniforms bearing the name of the company. Contractor personnel shall not smoke in facilities, block fire lanes at any time, or prop open exit doors while performing services. Contractor personnel shall park their vehicles in the area designated by the facility manager. Contractor personnel will interrupt their work at any time to allow for the passage of patrons, personnel, and/or visiting dignitaries. Contractor personnel will comply with all safety and security regulations in accordance with base policy and/or specific site or building requirements.

The contractor shall provide a sufficient number of personnel possessing the skills, knowledge, and training to satisfactorily perform the services required by this PWS. Contractor personnel shall meet industry minimum education or experience requirements for each specific service area. The contractor shall not employ persons who are identified to the contractor by the Contracting Officer (CO) as potential threats to the health, safety, security, general well-being, or operational mission of the installation and its population. All contractor personnel shall be able to fluently understand, read, write, and speak English. The CO may require the contractor to remove employees from the workplace for reasons of misconduct or security. The contractor shall ensure employees have current and valid certifications and licenses, as identified by the Government or Federal, State, and local requirements or laws, to accomplish the services of the PWS before starting work. The contractor shall not employ any person who is an employee of the United States Government if employing that person would create a conflict of interest or the appearance of a conflict of interest. The contractor is cautioned that off-duty active military personnel hired to perform work may be subject to permanent change of station, change in duty hours, or deployment. Military Reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the contractor’s ability to perform; however, their absence at any time shall not constitute an excuse for non-performance.

4.13 Physical Security: The contractor shall make prior arrangements to enter designated secure areas at MacDill AFB.

The contractor personnel will be escorted at all times in secured areas.

4.14 Performance Of Services During Crisis Declared By The President or Secretary of Defense: Unless otherwise directed by an authorized Government representative, it is determined that services may be performed on an extended basis up to 24 hours per day, as required. Emergency services will be as directed by contracting officer. All basic services will continue as directed by the Contracting Officer. During such periods, some or all of the personnel employed under this contract may be identified as emergency essential and may be granted access to the base during increased security measures. During performance of specific services, the contractor may be required to work any of the 24 hours of a day due to operational requirements. Any extended special or emergency (e.g. disaster relief operations) after-hours service, which increases the cost of performance above and beyond the negotiated contract price, shall be addressed by the contracting officer before requested services are performed (or at the earliest practicable time, when an immediate response to the situation is required to prevent injury or loss).

4.15 Quality Control: The contractor shall develop and maintain a quality control program to ensure laboratory services are performed in accordance with commonly accepted commercial practices and standards. Quality Control Plan shall be submitted to the Contracting Officer NLT 10 days after contract award. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor shall provide a quality control manager who will be independently responsible for planning, organizing, directing, and controlling the Quality Control Plan. The authority delegated to him will be comparable to that of the project manager on all matters pertaining to quality. The Quality Control Manager will also designate the inspection method and frequency for each function and performance standard.

4.16 Quality Assurance: The Government will periodically evaluate the contractor’s performance by appointing a representative(s) (i.e. COR) to monitor performance to ensure services are received. The Government representative will evaluate the contractor’s performance through intermittent inspections of contractor’s records and receipt of complaints from government personnel. The Government may request a quality control inspection at any time based on criteria such as, but not limited to repeated failures during quality control inspections and/or customer complaints. The Government may investigate, at any time, complaints from government personnel receiving these laboratory reports. The contractor shall be responsible for initially validating customer complaints and providing a written response to the Government representative within one business day. However, the Government representative shall make final determination of the validity of customer complaint(s) in cases involving of disputes. The contractor will be given four hours after verbal notification to correct the unacceptable performance.

4.17 Security Requirements: The MacDill AFB security requirements are listed in Appendix C of this PWS.

4.18 Change Requirement: Laboratory analytical support services are subject to change during this contract. The change may be initiated by the Civil Engineer Squadron (CES) or a designated representative who shall determine increased or decreased laboratory analytical support service requirements; however, no change shall be made to services without prior approval from the Contracting Officer. Any variation in the PWS shall be furnished by the COR personnel to the Contracting Officer for contract modification.

4.19 Unscheduled Interruptions: Laboratory analytical support service operations are subject to unscheduled interruptions and laboratory analytical support services personnel may be required to immediately leave the designated work area or be denied access due to security operations. Mission requirements may necessitate suspension or cancellation of laboratory analytical support services operations for that period. All incomplete work shall be rescheduled for completion within one working day.

4.20 Conservation of Utilities: The contractor must make sure employees practice utilities conservation. The contractor must be responsible for operating under conditions that prevent the waste of utilities. (a) Lights must be used only in areas where work is actually being performed. (b) Employees must not adjust mechanical equipment controls for heating, ventilation, and air conditioning systems. (c) Water faucets or valves must be turned off when not in use.

4.21 Traffic Laws. The Contractor and its employees shall comply with base traffic regulations.

4.22 Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their Contractor-owned or privately owned vehicle while on MacDill AFB.

4.23 Controlled Unclassified Information (CUI). The Contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding CUI material.

4.24 Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, Contractor personnel, resources, and classified or unclassified defense information.

4.25 Physical Security. The Contractor shall be responsible for safeguarding all government property and controlled forms provided for Contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

4.25.1 Antiterrorism/Force Protection. Please see Appendix D.

MacDill AFB CONTRACTOR SECURITY APPENDIX

INSTALLATION ENTRY CONTROL PROCEDURES FOR ALL CONTRACTORS

The following appendix provides information from Air Force Instruction 31-101, Air Force Instruction 31-113 and local supplements on requirements for entering and conducting business while on MacDill Air Force Base (MAFB), Florida.

1. Contract Award.

1.1. Upon award of a contract, all contractors (including subcontractors), will have a background check conducted on all requiring access to MAFB.

1.2. The base Point of Contact (POC) is the unit or the base contracting office administratively who services the contract.

The authorized format used for submitting employee personal information for background checks is located at Attachment 1, Request for Background Check.

1.3. Each base POC is appointed by their commander or management director on DD Form 577, Appointment/Termination Record – Authorized signature, and forwards a signed copy to the Visitor’s Reception Facility (VRF) annually.

1.4. The Base POCs identify and appoint responsible contractor supervisors to sponsor the sub-contractors and contract workers on base to perform services. The supervising contractor is responsible for providing their employees’ personal information to their base POC. Failure to provide all personal information required or providing fraudulent information will result in the employee’s base access being denied.

1.5. The Base POC will digitally sign and sends a contractor roster request (Attachment 1), to 6sfs.backgroundcheck@us.af..mil no later than two weeks prior to the start of the contract, for the initial background check screening (vetting).

1.5.1. As a minimum, the contractor roster request must contain the following.

1.5.1.1. Full name which appears on their government issued license or ID, to include middle names or any known aliases.

1.5.1.2. Date of Birth.

1.5.1.3. Social Security Number.

1.5.1.4. Driver’s license number and state of issue.

1.5.1.5. Project location.

1.5.1.6. Certifying Official’s Name (printed and signature).

1.5.1.6. Hours/days authorized on base.

1.6. The 6th Security Forces Squadron (6 SFS) will, upon receiving the EAL from the Base POC, conduct a background check of the contractor personnel using approved authoritative government databases to include NCIC/FCIC and DAVID.

1.7. Any contractor employee found to have a criminal conviction listed below or have an outstanding warrant(s) will not be allowed entry to the base.

1.7.1. The individual is known to be or reasonably suspected of being a terrorist or belongs to an organization with known terrorism links/support.

1.7.2. The installation is unable to verify the individual's claimed identity.

1.7.3. There is a reasonable basis to believe the individual has submitted fraudulent information concerning his or her identity.

1.7.4 There is a reasonable basis to believe the individual will attempt to gain unauthorized access to classified documents, information protected by the Privacy Act, information that is proprietary in nature, or other sensitive or protected information.

1.7.5. There is a reasonable basis to believe the individual will unlawfully or inappropriately use an access credential outside the workplace.

1.7.6. There is a reasonable basis to believe based on an individual's criminal or dishonest history, that issuance of an access credential poses an unacceptable risk to the installation/mission.

1.7.7. The individual has/had been barred from entry/access to a Federal installation or facility.

1.7.8. The individual is wanted by Federal or civil law enforcement authorities, regardless of offense or violation.

1.7.8.1. Non-extraditable warrants for DoD approved ID card holders will have 5 days of access to clear warrants with issuing agencies. After 5 days, member will be denied access until warrant is cleared from system.

1.7.9. The individual has been incarcerated within the past ten years for 90 days or more, regardless of offense/violation, unless released on proof of innocence.

1.7.10. The individual has any arrest/conviction for espionage, sabotage, treason, terrorism, or murder.

1.7.11. The individual’s name appears on any Federal or State agency’s “watch list” or “hit list” for criminal behavior or terrorist activity.

1.7.12. The individual has been arrested/convicted of a firearms or explosive violation within the past ten years.

1.7.13. The individual has been arrested/convicted of sexual assault, armed robbery, rape, child molestation, child pornography, trafficking in humans, drug possession with intent to sell or drug distribution.

1.7.14. There is a reasonable basis to believe, based on the individual's material, intentional false statement, deception, or fraud in connection with Federal or contract employment, that issuance of an access credential poses an unacceptable risk to the installation/mission.

1.7.15. There is a reasonable basis to believe, based on the nature or duration of the individual's alcohol abuse or illegal use of narcotics, drugs, or other controlled substances without evidence of substantial rehabilitation, that issuance of an access credential poses an unacceptable risk to the installation/mission.

1.7.16. A statutory or regulatory bar prevents the individual's contract employment; or would prevent Federal employment under circumstances that furnish a reasonable basis to believe that issuance of a access credential poses an unacceptable risk to the installation/mission.

1.7.17. The individual has knowingly and willfully engaged in acts or activities designed to overthrow the U.S.

Government by force.

1.7.18. In addition to the above disqualifying factors, 6 AMW/CC has determined the following criminal violations will be grounds for denial on MacDill Air Force Base IAW 6 AMW Integrated Defense Plan (IDP) 31-101.

1.7.18.1. The individual has been convicted of attempted murder or manslaughter.

1.7.18.2. The individual has been convicted of kidnapping or false imprisonment.

1.7.18.3. The individual has 10 or more felony convictions within the last 10 years.

1.7.18.4. The individual has been convicted of prostitution or solicitation of prostitution within the past five (5) years.

1.7.18.5. The individual has been convicted of assault on emergency services personnel within the past five (5) years.

1.7.18.6. The individual has had five (5) or more convictions of domestic assault or child abuse.

1.8. Qualified Contractors: After receiving favorable results from a background check, the Base POC will email the long-term pass memorandum to 6sfs.backgroundcheck@us.af.mil or will hand deliver the memo for the contractor, subcontractor, or contract worker to the VRF.

1.8.1. Individuals approved for official business working on MAFB for 60 days or more will be issued a Digital Biometrics Identification System (DBIDS) card by VRF personnel. Individuals on approved official business working on MAFB for less than 60 days will receive temporary paper pass. Attachments 1 & 2 will be completed for all applicants. NOTE: MAFB Government Contracting Activity or 6 SFS personnel reserve the right to deny base access to any contractor, sub-contractor and/or day laborer at any time.

1.8.2. Passes will not exceed one year. The Base POC determines the days of the week (examples: Mon through Fri, Mon & Tues), times authorized on base (example: 0600-1800). Background checks will be valid for the duration of their contract not to exceed one year, unless the following occurs:

1.8.2.1. Individual is terminated from the job. The Base POC will immediately notify the VRF.

1.8.2.2. Individual is involved in an offense on the Fitness Determination Factors list.

1.8.3. The contractor must pick up their pass within 15 days from the dated memo.

1.8.4. Requests for 24 hour/7 day (24/7) access to the base are limited to contract supervisors, as designated by the Base POC.

1.8.5. If an internal badging system is needed while performing the contract, the contract company is responsible for the badging system; the internal badge will not grant access to the installation. The contractor ID card issued by SF is used only for base access and for identification of a contractor when they do not possess any type of other DoD identification.

1.8.6. Some contractors, sub-contractors, day laborers and/or employees may be issued a white contractor badge.

Either installation access badges or CACs can be used to gain access to the installation during normal FPCONs.

During increased FPCON measures the decision to limit access to mission essential contractor personnel will be determined by the 6 AMW/CC.

1.8.7. If contractors, sub-contractors, day laborers and/or employees lose an installation access pass, that individual must report the loss to the project manager for the project who will investigate the loss and prepare a written statement, via AF Form 1168. A copy of the statement must be forwarded/hand carried to VRF before a new DBIDS credential will be issued. If three or more passes are lost the individual could lose their privileges to access the installation.

1.8.8. Use of the pass by anyone other than the rightful owner will result in the pass being revoked.

1.8.9. The VRF may conduct random periodic screenings to re-evaluate a contractor’s fitness determination.

1.9. Disqualified Contractors: 6 AMW/CC will deny access and access credentials based on information obtained during identity vetting that indicates the individual may present a threat to the good order, discipline and morale of MAFB. Refer to paragraphs 1.7 thru 1.7.18.6 above.

1.9.1. All non-DoD ID card holders are checked (vetted) through the NCIC and DAVID.

1.9.2. An individual denied access to the installation for one or more of the Fitness Determination Factors will be issued a letter or their Base POC will be notified via email, acknowledging the denial and informing the individual that he or she may appeal that denial by submitting a memorandum detailing the basis of their appeal to the VRF or TWG, who will coordinate the appeal through the 6 AMW/JA to the 6 MSG/CC for resolution. Appeals denied by the 6 MSG/CC can resubmit one last and final time to the 6 AMW/CC. If appeals are denied by the 6 AMW/CC then individuals will be denied access to the installation.

2. Contractor Visitor Passes for Entry to MAFB

2.1. The base POC completes Attachment 2, Request for Issuance of Physical Access Control System (PACS) pass, and emails it to the 6 SFS/Background Check email address, which can be located on the Global Listing (MacDill).

2.2. Contractors must possess the proper photo identification media (driver’s license/state identification card, military identification card, or other authorized U.S. governmental photo media) to be issued a base pass. Non-U.S. citizens must provide one of the following additional forms of documentation: Permanent Resident Card (I-551), Employment Authorization Card (I-766), or their foreign passport with a temporary (I-551) stamp or printed notation.

3. Contractor Vehicle Passes for Entry to MAFB.

3.1. Operators of vehicles who are non-DoD Contractors must provide the following documentation to register vehicles via AF Form 75, Visitor’s Pass.

3.1.1. Valid driver’s license.

3.1.2. Valid vehicle registration or rental agreement.

3.1.3. Valid insurance (except fleet vehicles) or rental agreement.

3.1.4. Drivers of borrowed vehicles must present a valid registration and proof of current insurance.

3.2. Operators are required to sign a consent to search and impoundment disclaimer upon receiving their vehicle pass.

4. Contract, Commercial, and/or Oversized Vehicles

4.1. All contractor, commercial, and oversized vehicles must enter MAFB through the Tanker Way Gate located off Interbay Boulevard.

4.2. The following are considered “commercial or oversized” vehicles.

4.2.1. All vehicles registered and licensed with “commercial” license plates, regardless of state of issue.

4.2.2. All vehicles, regardless of type, license plate, or size having more than two axles.

4.2.3. All vehicles displaying a commercial plaque, logo, or emblem.

5. Contractor Vehicle and Personnel Processing Requirements

5.1. The Tanker Way Gate is open for personnel and vehicle processing from 0500 – 1500 hours Monday through Friday, and closed weekends and holidays.

5.2. During closure of Tanker Way Gate, contractor/commercial vehicles requesting entry will use the Dale Mabry Gate.

6. Subcontractors. All subcontractors are required to follow the same instructions listed above for their personnel.

7. Contractors working in USAF Restricted Areas

7.1. Contract employees will be required to obtain a favorable National Agency Check with Written Inquiries (NACI).

Contract employees will be required to complete a Standard Form 85P, a Questionnaire for Public Trust Position, before being granted access to restricted areas.

7.1.1. Contract employee/s will coordinate with the unit security manager (SM) to gain access to Electronic Questionnaires for Investigation Processing (E-QIP) and complete the electronic security questionnaire (SF 85P).

7.1.2. The SM will complete an AF Form 2583, Request For Personnel Security Action. The medical records check portion of this form is not required.

7.2. The SM will make an appointment with 6 AMW/IPP, Wing Personnel Security office, (813) 828-2868, for the contract employee to submit fingerprints and sign a SF 85P.

7.2.2. After completion of a favorable local files check the contract employee will be eligible for the issuance of a restricted area badge on an interim basis based on submission of the SF 85P to 6 AMW/IPP. Coordinate with the SM to complete Phase I Orientation Training as outlined in AFI 31-101, para. 7.2.2 and Phase II Continuation Training annually.

The SM will provide the contract employee with AF Form 2586, Unescorted Entry Authorization Certificate, with instructions on completing the form.

7.3. The contractor will return to 6 SFS/S5R, Bldg 203, to obtain a Restricted Area Badge (RAB).

7.4. Issues identified during the course of the investigation may result in immediate revocation of restricted area access.

7.5. Any work on or near the flightline will be coordinated through the 6 Security Forces Squadron in Building 203. The Physical Security NCO will verify the need to access the restricted area and determine if a Free Zone can be established.

7.6. Access to Tenant Units.

7.6.1. For contractors requiring unescorted access to USCENTCOM facilities, a favorable NACI is required for entry to the facilities and contractors must be US citizens. For issuance of badges for entry into USCENTCOM facilities, contractors will contact USCENTCOM Personnel Security office at (813) 827-6281 for the issuance of the appropriate USCENTCOM badge. In this instance, paragraphs 7.3. and 7.4. does not apply.

7.6.2. For contractors requiring access to USSOCOM facilities, personnel will report to the front or back guard post in building 501 and be issued a white badge with black border that will authorize them limited unescorted access to common areas of building 501 and 501 C only. Contractor personnel who require access to all other areas within USSOCOM must be escorted. A NACI and US citizenship is not required to enter USSOCOM facilities. The Contractor will provide USSOCOM Security with an access list of contractor personnel performing under this contract. Only personnel on this access list will be allowed entry into USSOCOM facilities. It is the responsibility of the contractor to maintain an updated list.

7.6.3. For contractors requiring unescorted access to MARCENT facilities, a favorable NACI is required for entry to the facilities and contractors must be US citizens. Badges will not be issued to contractors entering this facility. The Contractor will provide MARCENT Security with an access list of contractor personnel performing under this contract.

Only personnel on this access list will be allowed entry into MARCENT facilities. It is the responsibility of the contractor to maintain an updated list.

7.6.4. For contractors requiring unescorted access to SOCCENT facilities, a favorable NACI is required for entry to the facilities and contractors must be US citizens. Badges will not be issued to contractors entering this facility. The Contractor will provide SOCCENT Security with an access list of contractor personnel performing under this contract.

Only personnel on this access list will be allowed entry into SOCCENT facilities. It is the responsibility of the contractor to maintain an updated list. Personnel will not be allowed unescorted entry into any portion of SOCCENT facilities.

SOCCENT personnel will provide escort for contractor personnel.

8. Flightline Driving.

8.1. Only authorized contracted or privately owned vehicles with colored cones are authorized on the flightline. Base operations will issue colored cones. The cone must be visible at all times when operating a vehicle on the flightline. The cone must be secured when not in use.

8.2. Contractors will never enter restricted areas unless properly escorted or authorized by the installation commander or designee through the issuance of a restricted access badge.

8.3. If security forces or a military member detains a contractor or contract employee at any time or for any reason, the contractor will comply with their request and will not become combative or argumentative.

9. Miscellaneous Requirements

9.1. All requests for additional contract employees must meet the same requirements listed in paragraph 1, before vehicle passes will be issued.

9.2. The 6th Contracting Squadron will be notified when personnel leave the contractor company for any reason. The 6th Contracting Squadron will provide that information to the Visitor Reception Facility (VRF) to ensure the pass is terminated in DBIDS.

9.3. The contractor is responsible for the return of all identification media and vehicle passes at the end of the contract or when personnel depart for any reason. All identification media and passes issued by security forces will be shredded by the Contracting Squadron, except in the case of an employee who has been terminated or the ID was lost/stolen, which must be returned to the VRF. CAC cards issued will be returned to the Trusted Agent (TA).
