STATEMENT OF WORK

Replace Flooring 89th Attack Squadron Project Number: 1140857

Period of Performance: 60 Days

SECTION 1: PROJECT INFORMATION

1.1 PROJECT LOCATION: Project shall be completed as a routine requirement at the 89th Attack Squadron Building 1008, along main o ice area, and room 131. Project hours are standard work hours.

1.2 UNIQUE REQUIREMENT: No unique requirements.

1.3 PROJECT DESCRIPTION: The user for B 1008 is requesting the current flooring in designated areas (outlined in attachment 2) be 100% remodeled. Work will include, but is not limited to:

relocating furniture as needed, demolition of carpet flooring, surface prep/removal of adhesive, installation of new carpet flooring, installation of vinyl floor tile, installing rubber wall base along all new flooring, transitions strips, and returning furniture back to original location.

SECTION 2: GENERAL REQUIREMENTS

2.1 WORKMANSHIP: All work shall be accomplished in accordance with all applicable building and fire codes. Contractor shall field verify all measurements, quantities, or value stated by the Government.

The Contractor shall be responsible for procuring, supplying, transporting, and providing all labor, materials, tools, plant, and equipment to complete the project as per scope of work, specifications, and attached drawings.

2.2 EXISTING CONDITIONS: The plans represent the best available information at the time they were prepared. While every e ort has been made to correctly show all major existing items which will a ect the work, it is recommended that the Contractor examine the site prior to submitting a bid. No additional payment will be made for extra work required by obstacles which were in place and clearly visible at the time of bidding.

2.3 WORKSITE: The contractor shall limit the work to the areas indicated by the government.

The contractor shall make every e ort not to disturb or obstruct portions of the facilities still occupied by the government. The contractor shall make e orts to minimize interference to Government operations. The contractor shall coordinate and schedule work with the 28 CES/SABER o ice prior the start of any operation. The contractor shall dispose of all construction debris o base at an approved disposal facility.

2.4 SITE SECURITY AND PROTECTION: The contractor shall make every e ort to secure the work site, allowing only authorized personnel to enter the work site. The contractor shall protect the site from damage from weather, vandalism, and/or theft.

2.5 DAMAGE TO EXISTING SITE CONDITIONS: The contractor shall be responsible to repair any and all damage incurred to existing Real Property/Grounds due to poor workmanship at no cost to the government. Contractor shall not park or drive on sidewalk or grassy/snowy areas. Damage incurred shall be repaired at no cost to the government.

2.6 HEALTH/SAFETY: The contractor shall be responsible for compliance with OSHA and AFOSH standards. The contractor shall maintain a clean and debris free site at all times.

The contractor shall keep driveways and entrances into and around the work site clear at all times. The contractor shall not use driveways or entrances as employee parking or storage of materials unless otherwise stated. The contractor shall minimize storage of materials and equipment on site.

2.7 GREEN PROCURMENT: Contractor must incorporate “green procurement” principles into the design of their projects. Specifically, consult the EPA’s web site at www.epa.gov/cpg/products.htm for a listing of the products that have been mandated as green procurement requirements for purchases.

SECTION 3: DESCRIPTION OF WORK AND SPECIAL INSTRUCTIONS

BUILDING 1008

Relocate furniture as needed.

3.1 DEMO (B 1008)

3.1.1 Demolish and dispose of existing carpeting in designated areas (see attachment 1)

(Approx. 5,968 SQFT)

3.1.2 Scrape and surface prep for new adhesive and flooring (Approx. 5,968 SQFT)

3.2 STRUCTURAL (B 1008)

3.2.1 Install new carpet in designated areas (Approx. 5,382 sqft)

3.2.2 Install new vinyl tile in designated area (Approx. 586 sq ft)

3.2.3 Install new 4” standard rubber wall base around new floor (Approx. 600 LF)

3.2.4 Install transition strips from carpeted areas to tile and o ices.

3.3 FINISHES (B 1008)

3.3.1 None

Return furniture back to original location.

SECTION 5: SPECIAL PROVISIONS/NOTES/SPECIFICATIONS

5.1 If due to unforeseen circumstances, work is uncovered which is necessary for the completion of the project, the Contractor shall immediately submit to the Government, in writing, an email detailing the additional work. The Contractor shall not proceed with the additional work identified until the change order is executed.

5.2 Carpet Specifications

Material: The carpet must be made of industrial grade materials.

Shape: The carpet must be manufactured and installed in square shapes.

Color: The carpet must be dark gray in color.

5.3 Tile/Luxury Vinyl Plank Specifications

Non-Porosity: The tile or luxury vinyl plank must be non-porous.

Color: The tile or luxury vinyl plank must be dark gray in color.
