About this file
This is a Small Business Participation Commitment Document (SBPCD) template for RFP No. 75N96025R00001 from the National Institute of Environmental Health Sciences. The document must be completed by all offerors, both large and small businesses, to demonstrate their small business utilization and participation commitments under Factor 6 of the evaluation.
The form requires offerors to provide their business size classification, identify any applicable socioeconomic categories (SDB, WOSB, EDWOSB, HUBZone, VOSB, SDVOSB), and detail their subcontracting breakdown including total contract value and amounts to be self-performed versus subcontracted. A minimum of 5% small business participation is required, with offerors needing to provide dollar values and percentages for participation across various small business categories. The form includes a table for identifying specific small businesses where firm written commitments (via teaming agreements, joint ventures, mentor-protégé agreements, or letters of commitment) are in place. Offerors must provide explanations if they do not meet the 5% requirement or if they have no identified subcontractors with firm commitments.

A SB commitment is a written agreement between a prime and small business to work together on this contract. This commitment in writing is considered the nature of commitment, which can include written teaming agreement, joint venture agreement, mentor-protégé agreement, written/signed letter of commitment from the small business, etc. This table can be expanded as needed.
