About this file

This is a Staffing Plan document for the Healthy Families Oregon (HFO) program covering two sites: Baker and Malheur, for the 2025-2027 contract period. The document outlines the organizational structure and staffing requirements for both locations, detailing positions across administrative, support, and direct service functions. Both sites are structured to include a Program Manager, Assistant Program Manager, three Eligibility Screeners, Community Outreach, Data Entry, and Administrative Support roles, followed by multiple Direct Service Teams consisting of Supervisors and Home Visitors. The staffing plan adheres to HFO Program Personnel and Practice Manual (PPPM) Standard 12 requirements, which mandate a Program Manager at a minimum of 0.5 FTE (with recommended minimums of 0.75 FTE for small sites and 1.0 FTE for medium/large sites), and establish supervisor-to-home visitor ratios of 1:5 as preferred or 1:6 as allowable for full-time supervisory positions.

The document does not specify actual FTE allocations or annual salaries, indicating that the staffing plan template is presented in blank format for completion. Both the Baker and Malheur sites are configured with seven Supervisor-led teams, each Supervisor overseeing five Home Visitors, resulting in a consistent supervisory structure across locations. The staffing model demonstrates alignment with HFO quality standards for service delivery, with multiple supervisor-led teams enabling appropriate caseload management and direct service oversight during the three-year contract term.
