About this file

This is a Business Proposal Attachment E document from the State of Indiana, Department of Administration (IDOA), establishing submission requirements for respondents to an RFP. The document outlines comprehensive instructions for vendors to provide detailed organizational, financial, and operational information necessary for state evaluation of product and service proposals. Respondents must submit three client references using Attachment H to idoareferences@idoa.in.gov by the proposal submission due date specified in RFP Section 1.24, with references required to email responses directly to the state. The attachment does not specify particular product categories, project quantities, site visits, bidder conferences, award dates, or contract terms in this section, as it functions as a template for proposal submission rather than a specific solicitation notice.

The proposal requirements mandate that respondents provide financial documentation demonstrating stability, including Dunn & Bradstreet reports or audited financial statements for the two most recently completed fiscal years, along with a personal responsibility statement from the CEO and/or CFO regarding financial information accuracy. Respondents must verify registration with the Indiana Secretary of State prior to contract negotiations, provide proof of legal authorization to commit the organization contractually, and describe experience serving state governments and similar-sized clients with comparable scope. Additional requirements include disclosure of business continuity and disaster recovery plans, state information security processes, willingness to extend pricing to other governmental bodies, and compliance with cloud-based system terms and conditions as specified in Attachments B1, B2, and B3 for Infrastructure-as-a-Service, Platform-as-a-Service, and Software-as-a-Service systems respectively. Indiana Veteran Owned Small Business (IVOSB) subcontractor engagement and diversity initiatives must be documented with explanation of any declined partnerships.
