Att D - Cost Proposal for RFP 26-87343
This is a cost proposal attachment for the DDB Medical Chart Review and Evaluation contract opportunity with the State. The State seeks to contract with one or more vendors to provide medical chart review and evaluation services based on competitive rates per case. The annual caseload is estimated at 100,000 to 114,400 cases, with vendors required to submit rates based on their experience level (0-2 years, 3-4 years, or 5+ years). Additionally, vendors must provide training services at 200 hours annually per consultant. The evaluation will be based solely on the rate per case metric, with awards going to the highest-scoring respondents with the most competitive rates until the annual caseload is met, though the State reserves the right to award to a single respondent. All prices are firm and fixed for the life of the contract, including any extensions.
Pricing is structured by experience level and service type. Training compensation is set at double the per-case rate, with baseline hourly rates of $56 (0-2 years experience), $66 (3-4 years experience), and $76 (5+ years experience). Vendors must complete a cost proposal template that calculates annual costs for both case review work and training hours, resulting in a maximum five-year contract cost. The proposal requires detailed calculations for the number of cases to be completed annually and the associated total costs based on submitted per-case rates.
