About this file

This document is a Contractor Certification attachment (D5) for immigration and nationality compliance, specifically for a Department of Veterans Affairs (VA) contract. The certification requires contractors to comply with immigration laws, including the Immigration and Nationality Act, and mandates that they will not knowingly employ illegal aliens or foreign nationals in violation of their visa status. Contractors must also adhere to E-Verify requirements consistent with Executive Order 12989.

The certification outlines potential consequences for non-compliance, including prohibiting non-compliant foreign nationals from working at VA-related sites and potential contract termination. Contractors must obtain similar certifications from subcontractors and understand that making false statements could result in prosecution under federal law. This document is specifically related to the VA solicitation 36C25626Q0022 for teleradiology services at the Fayetteville Arkansas Health Care System, which is seeking a Service-Disabled Veteran-Owned Small Business (SDVOSB) contractor.
