Cost Proposal Attachment for Enrollment Broker Services RFP 27-87793
This is an Attachment D Cost Proposal template for the State of Indiana, Family and Social Services Administration (FSSA), Office of Medicaid Policy and Planning, for Enrollment Broker Services under RFP 27-87793. The contract covers choice counseling and enrollment services for four Medicaid managed care programs: Hoosier Healthwise, Healthy Indiana Plan, Hoosier Care Connect, and Indiana PathWays for Aging. The contract term is four years beginning January 1, 2028, with two optional one-year extensions available for a potential six-year total. The contract is divided into four fiscal years (SFY 2028-2031) with optional extensions through SFY 2034. Responses were due on October 28, 2026. The cost proposal template requires bidders to complete five primary service categories: Required Staff pricing (Project Manager, Operations Supervisor, Information Systems Coordinator, and Quality and Training Coordinator), Other Staff pricing for supplemental positions, Helpline Services pricing per call across four volume bands, Printing costs per unit across eight volume bands, Information Systems staffing and operations costs, and Other Tasks staffing costs related to contract execution.
Bidders must provide hourly wage rates (excluding benefits) for all staff positions with associated administrative overhead percentages; pricing must be provided to the cent level for helpline services and to the tenth of a cent for printing services. Fixed annual price increase percentages must be submitted for Helpline Services, Printing, Information Systems Operations, Information Systems Staffing, and Other Tasks to drive pricing for subsequent contract years. The template automatically calculates Total Hourly Cost Per Position by combining wage rates and overhead percentages, which then feeds into all relevant tasks. Helpline pricing must account for four call volume bands (0-5,000; 5,001-10,000; 10,001-15,000; and 15,001+ monthly calls) and include staffing full-time equivalents at call volumes of 3,000, 8,000, 12,000, and 17,000. For printing, expected monthly volumes include postcards, single and double-sided sheets in black and white and color, and standard envelopes, with postage billed separately at cost. Information Systems costs must detail all hardware, software, and ancillary expenses with quantity and per-unit pricing. Pricing must remain firm for a minimum of 180 days from the award date.
