SECTION I

1.0 DESCRIPTION OF SERVICES

1.1. General. This is a non-personnel services contract to provide Automatic Door Service. The Government shall not exercise any supervision or control over contract personnel performing the services herein. Such contract service providers shall be accountable solely to the contractor who, in turn is responsible to the Government.

1.1.1. Overview Description of Services/Introduction. The contractor shall provide all management, personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform Automatic Door Service as defined in this Performance Work Statement (PWS) except for those items specified as Government furnished property and services.

The contractor shall perform to the standards in this contract. This includes the planning, coordination, and surveillance of the activities necessary to ensure disciplined work performance and timely resources application to accomplish all tasking under the contract. The contractor shall be responsible for maintaining communication with the Contracting Officer (CO) and the Contracting Officer Representative (COR), and to immediately notify both the CO and the COR of any problems that would prevent timely performance of this contract. The contractor shall be responsible for and required to implement, and maintain management control systems necessary to plan, organize, direct, and control all activities under this contract.

1.2. Objectives. Perform preventative maintenance and interim services for automatic doors.

1.3. Scope. Preventative maintenance and repair shall cover all components of the automatic doors including, but not limited to, controllers, switches, pads, actuators, door tracks and doors.

1.4. Inherently Governmental Function. The contractor shall not perform Inherently Governmental functions as defined in FAR Subpart 7.5 in relationship to this PWS.

1.5. Period of Performance. One12-month base period with four, 12 month option periods.

1.6. Place of Performance. The contractor shall perform the services in the Performance Work Statement at Eglin Air Force Base, Florida to include all outlying ranges and sites as defined in workload summary.

1.7. Specific Tasks.

1.7.1. Basic Services. The contractor shall provide preventative maintenance and interim services for automatic doors in base facilities identified in the attached Appendix 2, Estimated Workload. The data in Appendix 2 is provided for informational purposes only. The contractor shall expect variations due to the nature of these services. Any fluctuation (increase or decrease) in workload shall be negotiated and executed through a modification to this contract by the Contracting Officer.

1.7.2. Preventative Maintenance Inspection (PMI). One (1) PMI shall be performed annually during the first month of each period of performance in the contract. This inspection shall include, but not be limited

Page - 5 - of 23 to: adjustments, alignments, replacement of malfunctioning parts, cleaning, lubricating, and all necessary repairs to maintain in good working order the above automatic doors. Replacement of parts shall be IAW paragraph 1.7.4. The contractor shall report to the COR prior to the start of inspection and turn in a service ticket to the COR as stated below.

1.7.2.1. Scheduling. The contractor shall submit an emailed list of its Points of Contact (POCs) and a proposed work schedule for the PMI performance period to the CO and COR within 10 workdays of notice of contract award. Schedule shall include, the location for each PMI, date and approximate time.

The contractor shall submit changes or revisions to work schedules to the COR no later than five (5) workdays prior to change date. All schedules and work plans shall be made available to the CO and/or COR upon request. The list of POCs shall include the names, telephone numbers, and email address(es) for individuals appointed as the contractor’s Points of Contact (POCs) for all work requirements stated herein.

1.7.3. Interim Service. Interim service shall consist of routine for repair, modification, installation, transfer and removal of equipment within the required response time specified herein. Response time begins at the ending time of call placement by the CO or COR. Response time is defined as the time required for a maintenance technician to arrive on site with the materials, tools and test equipment necessary to restore automatic doors to a 100 percent fully operational condition.

1.7.3.1. Routine Interim Service. Response time is eight (8) working hours after placement of call, and will be provided during the hours of 0700-1530 hours, Monday through Friday, except Eglin-observed holidays. No afterhours work shall be performed. Contractor shall provide a copy of the Service Ticket upon completion of service being provided.

1.7.4. Replacement of Parts. The contractor shall provide replacement parts in the performance of this contract as stated herein, or as authorized by the CO or the COR. All parts shall be of original manufacturer or equal specification. Payment for replacement parts shall be limited to the factory list price, including approved transportation expenses, less any trade discounts, rebates, allowances and commissions. The CO and COR shall be notified immediately of any unusual delays expected in the attainment of parts. The contractor shall receive prior authorization as detailed below before ordering replacement parts. The contractor shall attach a copy of the related transportation receipts (bill of lading), and submit these with each submitted invoice. Invoices shall be submitted to the COR no later than five

(5) workdays following the month of service and with no more than two (2) discrepancies per quarter.

1.7.4.1. Replacement Parts. The price of part(s) up to a maximum of three thousand dollars ($3,000.00) including estimated transportation expenses less any trade discounts, rebates, allowances and commissions, shall require the prior approval of the COR.

1.7.4.2. Replacement Parts. The price of part(s) exceeding three thousand dollars ($3,000.00) including estimated transportation expenses less any trade discounts, rebates, allowances and commissions, shall require the prior written approval of the CO.

1.8. Minimum Qualifications. All contractor service personnel shall be American Association of Automatic Door Manufacturers (AAADM) certified technicians with a minimum of three (3) years of experience in maintaining and repairing multiple brands of automatic doors.

1.9. Deliverables.

1.9.2. Data. The Government has unlimited rights to all deliverables of this contract.

1.9.3. Inspection. Deliverables will be inspected by the Government within three (3) workdays of receipt.

1.10. Reports and Records. The contractor shall provide a monthly invoice for services rendered. The contractor shall maintain a service log that contains the date and time of service, and shall provide this log with the invoice to the COR. Invoices along with all backup documentation shall be submitted to the COR no later than five (5) workdays following the month of service and shall not contain more than two (2) discrepancies per quarter. Once invoice is reviewed and accepted, the contractor will be notified. The contractor shall submit the COR approved invoicing documents within five (5) workdays of receipt of COR’s approval into Wide Area Work Flow. Proper and timely submission of monthly invoicing is solely the responsibility of the contractor.

SECTION II

2.0 SERVICE SUMMARY (SS)

2.1. General. The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. Unless indicated elsewhere these performance objectives will be measured monthly. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

2.2. Performance Evaluation. Performance of a service shall be inspected in a manner consistent with commercial practices to determine whether or not it meets the performance requirements of this contract. Re-performance of unacceptable services at no additional cost to the Government is the preferred course of action when appropriate.

2.3. Performance Assessment.

2.3.1. Validated Customer Complaint. The COR will receive and investigate customer complaints. The COR shall be responsible for initially validating customer complaints. The CO shall make final determination of the validity of customer complaint(s) in cases of disagreement with the COR and the contractor. Validated customer complaints shall be re-performed after notification by the COR.

Exceeding the performance threshold for performance objective in any one month period may result in a warning or letter of concern from the Contracting Officer. Exceeding the performance threshold for performance objective for any two, or more consecutive or non-consecutive months during a contract period may constitute an immediate Progress Meeting with the Multi-functional Team.

2.3.2. Periodic Inspection. The COR shall inspect work performance monthly through physical Evaluation of samples selected on other than 100% or statistically random basis.

SECTION III

3.0 GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES

3.1. Government Furnished Services. The Government will provide the following services for contractor use under this contract.

3.1.1. Emergency Medical Services. Eglin AFB will provide emergency medical services for contractor personnel in performance of services under this contract base IAW Air Force Instruction (AFI) 41-115 Chapter 1. The contractor shall reimburse the Government for treatment provided at the current reimbursement rate for non-authorized user IAW AFH 41-114, paragraph 42. The contractor shall provide to the 96 Medical Group, the name of a physician and/or hospital to which injured personnel can be transferred for further treatment and care after receiving initial emergency treatment on base. All mishaps will be promptly reported to the base safety office through the COR.

3.1.2. Security Police and Fire Protection. Phone number for Security Forces Squadron, (850)882-8291; Fire Protection, 911.

3.1.3. Utilities. The Government will provide all electricity, water and sewage at no additional cost to the contractor. The contractor shall instruct its personnel in utilities conservation practices. The contractor shall be responsible for operating under conditions that preclude the waste of utilities, which include turning off the water faucets or valves after using the required amount to accomplish requirements under this PWS.

3.2. Government Furnished Facilities. N/A, the Government will NOT provide any facilities or storage space for the contractor’s use under this contract.

3.3. Government Furnished Equipment. N/A, the Government will NOT provide any equipment for the contractor to use under this contract

3.4. Government Furnished Materials/Information. The Government will provide the following information: access or information and documentation to relevant Government organizations, and (upon request) applicable internet URLs or manuals, texts, briefs, and associated materials as required for the contractor to perform the requirements under this contract.

SECTION IV

4.0 GENERAL INFORMATION

4.1. Hours of Operation. The core work hours for performance under this contract shall be between the hours of 07:00 – 15:30 Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. At all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS. When hiring personnel, the contractor shall keep in mind that the stability and continuity of the workforce are essential.

4.1.1 Recognized Holidays.

*If the holiday falls on a Saturday, it will be observed on the preceding Friday. If the holiday falls on Sunday, it will be observed on the following Monday.

4.2. Contract Personnel.

4.2.1. Contract Project Manager. The contractor shall provide the name and contact information of its project manager (PM) who has the authority to execute the requirements for Automatic Door Services under this contract on behalf of the contractor to the CO within two (2) work days of contract start date.

The list shall be kept current throughout the life of the contract. The project manager shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contract manager shall be available within one (1) business day to meet on the installation with Government personnel designated by the CO for discussions, as necessary. The project manager shall be able to read, write, and speak English. The contract manager shall be able to effectively communicate with the Government and Contractor personnel.

4.2.1.1. Personnel Roster. The Contractor may be required to provide a personnel roster consisting of names and other pertinent personnel data to the CO or COR for the purposes of 100% accountability real-world and exercise situations throughout the performance of this contract. When the roster is requested, the personnel roster shall be maintained, updated, and provided within three (3) workdays of request and as individuals are removed and added.

4.2.1.2 Service Contract Reporting -- Manpower data

The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract in the Service Contract Reporting (SCR) section of the System for Award Management (SAM) (http://sam.gov) to support the requirements of title 10, U.S.C, Page - 11 - of 23 section 235 and 2330a. Contractor shall register in SCR/SAM within 30 days of contract award. The contractor is required to completely fill in all required data fields using the following web address http://www.sam.gov. Reporting inputs will be for the labor executed during the period of performance during each government fiscal year (FY), which runs 1 January through 31 December. While inputs may be reported any time during the FY, all data shall be reported no later than 30 January of each calendar year. Contractors may direct questions to the above SAM help desk.

Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

Data for DoD Service Contact Reporting must be input at the System for Award Management www.sam.gov.

Also, reference the SAM Federal User Guide available in the “HELP” section for comprehensive instructions on navigating SAM. Should contractors have issues using SAM, they should contact the Federal Service Desk at https://www.fsd.gov.

4.2.2. Contractor Employees:

4.2.2.1. English Language. Contractor personnel who communicate directly with Government personnel shall be required to effectively communicate with the customers in English. This requirement is strictly for the purpose of efficiently performing the contract tasks.

4.2.3. Use of Government Employees. The contractor shall not employ any person who is an employee of the U.S. Government if employing that person would create a conflict of interest. In addition, the contractor shall not employ any person who is an employee of the Department of the Defense if such employment would be contrary to the policies contained in AFI 64-106, Chapter 1, Industrial Labor Relations Activities.

4.2.3.1. Employment of Spouses/Dependents. The contractor shall not employ any person who is a spouse or dependent child of a Department of Defense (DoD) employee (either military or civilian) when such employee would be placed in a conflict of interest or appearance of a conflict of interest by virtue of such employment.

4.2.3.2. Off-duty Military Personnel. The contractor is cautioned that off-duty active military personnel hired under this contract may be subject to permanent change of station, change in duty hours, or deployment. Military Reservists and National Guard members may be subject to reorder to active duty.

The abrupt absence of these personnel could adversely affect the contractor's ability to perform.

However, their absence at any time shall not constitute an excuse for nonperformance under this contract.

4.2.3.3. Identification of Contractor Employees. All contract personnel working on Eglin AFB, FL shall identify themselves as contractor personnel such to avoid creating an impression in the minds of members of the public that they are Government officials. They must also ensure that all documents or reports produced by Contractors are suitably marked as Contractor products or that Contractor participation is appropriately disclosed.

4.3. Emergency Operations/Mission Essential Personnel.

4.3.1. Continuation of Essential DoD contractor Services During Crisis.

In Accordance With DoDI 1100.22, (12 Apr 2010) Continuation of Essential DoD contractor Services During Crises, DFARS 237.76, and the Air Force implementation thereof, unless otherwise directed by an authorized government representative, it is determined that Automatic Door services as identified in this PWS are not essential for performance during a crisis.

4.4. Quality Control (QC)/Quality Assurance (QA)

The contractor shall develop and maintain a “Quality Control Plan” (QCP) to ensure all services are performed IAW Federal, State, and local requirements and standards and commonly accepted commercial practices and standards. The contractor shall develop and implement procedures to identify and prevent defective services from reoccurring. As a minimum, the contractor shall develop quality control procedures that address the areas identified in Section 2, Service Summary. Contractor records shall be kept and made available to the Government (if requested) throughout the contract performance period.

4.4.1. Government Quality Assurance

According to FAR Part 46.4, the Government will evaluate the contractor's performance under this contract. The Government will evaluate the contractor’s performance IAW the Government’s Quality Assurance Surveillance Plan (QASP). The COR will evaluate the contractor’s performance through surveillance methods available to the Government and defined elsewhere in this document. When an observation indicates defective performance, the COR will require the contract’s project manager or contractor representative at the site to initial the observation. The initialing of the observation does not necessarily constitute concurrence with the observation, only acknowledgment that he or she has been made aware of the defective performance. Government surveillance of tasks not listed in the PWS or by methods other than those listed in the PWS may occur during the performance period of this contract. Such surveillance will be done according to standard inspection procedures or other contract provisions. Any action taken by the CO as a result of surveillance will be according to the terms of this contract. The Government may inspect each task as completed and increase the number of quality assurance inspections if deemed appropriate because of repeated failures discovered during surveillance inspections or because of repeated customer complaints

4.4.2. Contracting Officer Representative (COR)

COR are representatives of the Contracting Officer and will participate in the administration of this contract. Subsequent to contract award, the Contractor will be provided with a letter identifying COR and setting forth their duties and authorities. The COR is not authorized to change any of the terms and conditions of the contract/task order.

4.4.3. Multifunction Team (MFT). The Contract Manager with the Contracting Officer, Contract Administrator, COR and other Government personnel as applicable may schedule meetings as needed.

In addition, the Contractor may request a meeting with the Contracting Officer when the Contractor believes such a meeting is necessary. If the Contracting Officer or Contract Administrator deem necessary, written minutes of any such meetings shall be recorded in the contract file and signed by the COR and CO

4.4.4 Contract Discrepancies. The COR or CO will periodically evaluate the contractor’s performance using the methods of surveillance listed in Section 2 – Service Summary of the PWS. Government personnel will record all surveillance observations. When an observation indicates defective performance, the COR will require the contractor to initial the observation. The initialing of the observation does not necessarily constitute concurrence with the observation, only acknowledgment that he or she has been made aware of the defective performance. If the contractor challenges the validity of any observation, and the COR and Contract Manager cannot come to an agreement, the complaint in question shall be forwarded to the CO for resolution. The preferred method of dealing with any valid observation is re-performance, if possible, and at no additional cost to the Government. Repeat observations or an incident that violates safety or environmental regulations shall require the COR to instigate a Corrective Action Report (CAR) through the CO. The contractor shall be given three (3) business days to respond to the CAR with either an explanation on why it disagrees with the CAR or with its concurrence with the CAR, any disciplinary action taken, and a mitigation plan to avoid repeat occurrences.

4.5 Environmental Controls.

4.5.1. Environmental Protection. The contractor shall be knowledgeable of and comply with all applicable Federal, State, Interstate, and local laws, regulations and standards that are or may become applicable to contractor’s activities in performance of this PWS. This includes Air Force regulations, operating procedures and permit conditions. Contractor operations are subject to environmental compliance inspections without notice. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect.

4.5.2. Non-compliance. If the contractor is found to not be in compliance with any of these regulations, there may be cause for the facility to shut down until contractor, when within the contractor’s control or the Government’s, has remedied the problem. Any sanitary deficiencies shall require immediate correction.

4.5.3. Spill Response. The contractor is responsible to report and promptly clean up all spills in a manner consistent with current environmental regulations. In the event that it is necessary to utilize Government material, equipment, or personnel to clean up a contractor caused spill, the contractor shall be required to reimburse the Government for all associated costs. If the contractor spills or releases any material in performance of this contract (such as oils, sewage or grease) or any substance contained in 40 CFR 302 into the environment, the contractor or its agent shall immediately report the incident to the Base Security Forces at 911 and the CO or designated COR. The COR will notify 96 CEG/CEVCE.

4.5.4. Liability. The contractor shall assume liability for acts of omission in regard to compliance with Federal, State, Interstate, and local environmental statutes, regulations and applicable guidelines, Page - 14 - of 23 including, but not limited to fines or penalties associated with enforcement actions and costs for environmental restoration (removal actions, remedial actions, and corrective actions). The contractor and any subcontractor shall assume liability in the event that their actions result in the release of any hazardous substance in the air, soil, surface water, or ground water. The liability for the spill or release of such substances rests solely with the contractor and its agent. The contractor and any subcontractors shall be responsible for documentation, reporting, assessment, and remedial action requirements associated with such release and any resulting contamination. The contractor shall provide copies of all manifests and/or shipping papers to the Government prior to removing wastes from Eglin AFB. The Government reserves the right to initiate and complete all cleanup actions. This does not alter or amend the contractor’s liability for the site cleanup. The contractor or subcontractor shall reimburse the Government for all costs associated with cleanup.

4.5.5. Prohibitions. Unless otherwise approved, in writing, by Air Armament Center’s Environmental Management Compliance division (AAC/EMC), the contractor shall not use: Lead–based paints containing .5 percent or greater, by weight, of lead, products containing Class I Ozone Depleting Chemicals (ODCs), or paints, paint thinners, solvents or strippers with 3.5 pounds per gallon or more of volatile organic compounds.

4.6. Miscellaneous.

4.6.1 Freedom of Information Act (FOIA). All official Government records affected by this contract are subject to the provisions of the FOIA (5 U.S.C. 552/DoD 5400.7-R/AF Supplement). Any request received by the Contractor for access/release of information from these records to the public (including Government/Contractor employees acting as private citizens), whether oral or in writing, shall be immediately brought to the attention of the CO for forwarding to the Base FOIA Manager to ensure proper processing and compliance with the Act.

4.6.2 For Official Use Only (FOUO). The Contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

4.6.3 Notification of Debarment/Suspension Status. The Contractor shall provide immediate notice to the Contracting Officer in the event of being suspended, debarred or declared ineligible by any other Federal Department or agency, or upon receipt of a notice of proposed debarment from another DOD Agency, during the performance of this contract.

4.6.4 Official Correspondence. The Government singularly identifies each project with a contract number at time of award. The contract number is a unique identifier to purposely and permanently represent an awarded project. The Government issued contract number is to be referenced on all official communication starting upon notice of award.

4.6.5 Past Performance Information (PPI) / Contractor Performance Assessment Report (CPARS). As needed, the Government will formally assess and report on the Contractor’s performance annually.

CPARS allows for assessment a contractor’s performance, both positive and negative, and provides a record on a given contract during a specified period of time. If assessment is needed, the Contractor will

Page - 15 - of 23 be provided a copy of their assessment at the end of each performance period. The Contractor shall have 60 days to review and submit comments for the evaluation activity’s consideration before the assessment is finalized by the Assessing Official. Past Performance assessment reports will be protected by all parties and treated by Government personnel as “For Official Use Only/Source Selection Information in accordance with FAR 2.101 and 3.104”.

4.6.6 Privacy Act. Work on this contract may require that personnel have access to information protected by the Privacy Act. Contractor personnel shall adhere to the Privacy Act, Title 5 of the U.S.

Code, Section 552a and applicable agency rules and regulations when handling such information.

4.6.7 Records. All records created and received by the Contractor in the performance of this contract shall be maintained and readily accessible to the Government as requested records shall remain the property of the Government.

4.6.8 Safety Concerns. The Contractor is solely responsible for compliance with OSHA standards for the protection of their employees. The contract manager shall ensure specific safety requirements in AFOSH standards and Air Force technical orders are complied with by Contractor personnel when non-compliance would clearly present the potential to harm or damage Government resources. (See AFI 91- 301, paragraph 9 and AFI 21-101 paragraph 2.16.) The Air Force is not responsible for ensuring that Contractors comply with “personal” safety requirements that do not present the potential to damage Government resources.

SECTION V

5.0 SAFETY AND SECURITY

5.1. General:

5.1.1 The contractor shall comply with all base security regulations. Additionally, the contractor shall comply with actions dictated by force protection condition changes/measures.

5.1.2. The contractor and its employees shall comply with all base traffic regulations.

5.1.3. Contractor employees are prohibited from possessing weapons, firearms or ammunition, on themselves or within contractor-owned or privately owned vehicle while on Air Force Installations.

5.1.4. The contractor shall report suspicious activity to base Security Forces at 850-882-8291 immediately.

5.2. Safety. It is the contractor's responsibility to ensure its employees and managers have a comprehensive understanding of and full compliance with OSHA requirements. Detailed information is available on the OSHA website at http://www.osha.gov. It is the contractor's sole responsibility for compliance with OSHA, Public Law 91-596. The contractor shall comply with all Federal, State, Interstate, and local laws, and Air Force Instructions for the protection of their employees while working on an Air Force Installation or property. The protection of contractor personnel is exclusively the obligation of the contractor. Contractor shall manage all work areas to ensure the safety of building occupants, base personnel, or visitors in or near the areas. The contractor shall submit a Safety and Health Plan that describes the capabilities and processes to ensure compliance with safety and health requirements during the performance of the work described in the solicitation. The contractor's plan shall include appropriate measures to ensure the contractor reacts promptly to investigate, correct and track alleged safety and health violations and/or uncontrolled hazards in contractor work areas.

Contractor shall ensure that personnel are properly trained on the equipment they are tasked to operate.

The contractor shall develop site specific mishap reporting procedures which result in damage to government property or injury to Air Force personnel. These procedures are to be included in the safety and health plan for review. In accordance with AFI 91-202, AFMC Supplement, Safety Program, the contractor shall submit a Safety and Health Plan to Contracting for review and acceptance by the safety office prior to contract award.

5.2.1. Safety Equipment. The contractor shall furnish personal safety equipment for its employees in performance of this contract. Safety equipment includes shoes or boots, eyewear, and gloves.

5.2.2. Safety Threats. The Government reserves the right to restrict the employment, under this contract, of any contractor employee, or prospective contractor employee who is identified as a potential threat to health, safety, security, general wellbeing, or the operational mission of the installation and its population. Contractor shall perform background checks, at contractor's expense, to ensure that said employee has not been convicted of any sexual crime or any crime against children.

5.2.3. Substance Abuse Testing. The consumption of alcoholic beverages or illegal drugs by contractor personnel, while on duty, is strictly forbidden. Contractor employees shall be subject to substance abuse

Page - 17 - of 23 testing as a result of a mishap investigation when an individual’s actions or inactions are suspected as factors in a mishap sequence. The CO may identify the employees to be tested and the drugs and substances for which those employees must be tested. The service provider shall accomplish the specified testing using a certified (i.e., a certified lab that meets Department of Health and Human Services (DHHS)/ Substance Abuse and Mental Health Services Administration (SAMHSA) certification) medical substance abuse testing facility. Results of the test(s) shall be provided to the Government solely for the purpose of completing an investigation. The contractor shall immediately remove any employee who is found to be under the influence of alcohol or drugs.

5.2.4. Appearance. Contractor personnel shall present a clean and neat appearance. Contractor’s uniform must be easily recognized. This shall be accomplished by wearing distinctive clothing bearing the name of the company and/or by wearing badges that contain the company name and employee name.

5.3. Security Requirements:

5.3.1. Physical Security. The contractor shall be responsible for safeguarding all Government equipment, information and property provided for contractor use. At the close of each work period, Government facilities, equipment, and materials shall be secured.

5.3.2. Contractor Access to Air Force Installations. IAW AFMAN 31-113, Installation Perimeter Access Control. Vehicle registration, proof of insurance and a valid driver's license must be presented for vehicles.

(1) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.

(2) The contractor shall submit a written request on company letterhead to the CO listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor personnel needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The Contracting Officer will endorse the request and forward it to the issuing base pass and registration office or security police for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, and valid vehicle insurance certificate, to obtain a vehicle pass.

(3) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Volume 1, The Air Force Installation Security Program, and AFI 31-501, Personnel Security Program Management. Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor personnel are returned to the issuing office. Failure to comply with these requirements may result in withholding of final payment.

5.3.3. Contractor Access to Facilities. The Government is responsible for providing access to facilities for the contractor to accomplish all required work IAW the PWS.

APPENDIX 1

DEFINITIONS

Arrival Services. Services performed to prepare for and properly park transient aircraft in an assigned parking area.

Contract Administrator (CA). The individual within the contracting office who performs the day-to-day administration of the contract. The contract administrator may also be the contracting officer.

Contracting Officer (CO). The duly appointed Government agent authorized to award or administer contracts. The contracting officer is the only person authorized to contractually obligate the Government.

Corrective Action Report (CAR). Used to document unacceptable performance by the contractor.

Defect. Any non-conformance with requirements specified in the contract.

Defective Service. A service output that does not meet the standard of performance specified in the contract for that service.

Fair Wear and Tear. The deterioration of items attributed to normal usage.

Government Furnished Property (GFP). Facilities, equipment, tools, supplies, parts, or any other items furnished for the concessionaire's use by the Government.

Major discrepancy. A major finding is defined as a condition that would endanger personnel, jeopardize equipment or system reliability, affect safety of flight, or warrant discontinuing the process or equipment operation.

Minor discrepancy. A minor finding is defined as an unsatisfactory condition that requires repair or correction, but does not endanger personnel, affect safety of flight, jeopardize equipment reliability, or warrant discontinuing a process or equipment operation.

Performance Assessment. A process that measures success towards achieving defined performance objectives or goals defined within the performance thresholds in the services summary or the process of assessing progress towards achieving the objectives/goals developed in a performance plan or partnering agreement.

Performance Indicator. A measurable characteristic of an output of a work process. Generally synonymous with attribute.

Performance Management. The use of performance measurement information to effect positive change in organizational culture, systems, and processes, by helping to set agreed upon performance goals, allocating and prioritizing resources, informing managers to either confirm or change current policy or program directions to meet those goals, and sharing results of performance in pursuing those goals.

Performance Objective. The outcome associated with successful contract performance in a specific area. This is a critical success factor in achieving the organization’s mission, vision and strategy which, if not achieved, would likely result in a significant decrease in customer satisfaction or risk mission failure. Obtaining multi-services/sub-services performed at a certain measurable standard and consistently ensures success in achieving the objectives critical to the mission.

Performance Threshold. The minimum performance level of a performance objective required by the Government.

Quality Assurance (QA). Those actions taken by the Government to assure services meet the requirements of the Performance Work Statement (PWS).

Contracting Officer Representative (QA or COR). Individual who monitors a contractor on a daily basis and who is involved in every aspect of a contract to ensure the contractor is in compliance with that contract.

Quality Assurance Program Coordinator (CORC). The individual designated by the contracting squadron commander to manage the installation QA program.

Quality Control (QC). Those actions taken by a Concessionaire to control the performance of services so that they meet the requirements of the SOW.

Services Summary (SS). A summary of the performance objective and performance threshold required by the Government in concessionaire performance. Also known as a Performance Requirements Summary or Services Delivery Summary or Performance Requirements Document.

Standard. An exact value, a physical entity, or an abstract concept, established and defined by authority, custom, or common consent to serve as a reference, model, or rule in measuring quantities or qualities, establishing practices or procedures, or evaluating results. A fixed quantity or quality. A defined measure of comparison.

ACRONYMS. This is a list of commonly used, general acronyms, not inclusive of all identified elsewhere in this document.
