Statement of Work (SOW)
1. PROJECT TITLE:
Mobile Mammography Services at the Crow/Northern Cheyenne Hospital (CNCH), Crow Service Unit (CSU).
2. BACKGROUND/PURPOSE: The CSU does not have the equipment for mammograms and requires a mobile service to provide this important screening to our patients on site.
The mission of the Indian Health Services (IHS) is to raise the physical, mental, social and spiritual health to the highest level using the principles of "the patient comes first" with a focus on health care and health promotion, in a culturally sensitive manner while empowering the individual. Providing this service on site will remove the need for patients to travel for this service and directly affect health promotion services.
3. SCOPE: To provide mammography screenings for patients of the Crow Service Unit locations; CNCH, Lodge Grass Clinic, and Pryor Clinc through a mobile mammography vehicle. The contractor shall provide all labor, equipment, and transportation necessary to provide Mobile Mammography services.
4. APPLICABLE DOCUMENTS: Guidelines available include PHS, IHS, Billings Area, Service Unit, and Medical Staff/Radiology regulations, policies, procedures, and by-laws.
In addition, traditional and currently accepted radiology practices are available in the form of specialty consultation, journals, and textbooks.
5. TASKS: The primary task is to provide mammogram services for the CSU sites.
a. Task #1: The contractor shall provide mobile screening mammograms to the
Crow Service Unit sites; CNCH, Lodge Grass Clinic, and Pryor Clinic. A total of 24 visits annually.
b. Task #2: The contractor shall provide mobile screening mammograms on pre-set dates established by the site.
c. Task #3: The contractor shall provide the registered mammogram technologist and mobile mammogram vehicle, with driver.
6. DELIVERABLES AND DELIVERY SCHEDULE:
a. Location: Crow/Northern Cheyenne Hospital (CNCH)
One Hospital Loop, Crow Agency, MT 59022
Contractor shall provide Mammography services at CNCH once a month.
Every third month shall have two visits. The total annual visits for CNCH will be 16 visits. Mammography Services will be performed between the business hours of 8:30am to 4:00pm.
b. Location: Lodge Grass Clinic 418 Hardin Ave, Lodge Grass, MT 59050
Contractor shall provide Mammography services at the Lodge Grass Clinic once each month to the scheduled months of: January, April, July, and October. The total annual visits for Lodge Grass Clinic will be 4 visits.
Mammography Services will be performed between the business hours of 8:30am to 4:00pm.
c. Location: Pryor Clinic 2 Pryor Gap Road, Pryor, MT 59066
Contractor shall provide Mammography services at the Pryor Clinic once each month to the scheduled months of: February, May, August, and November.
The total annual visits for Pryor Clinic will be 4 visits. Mammography Services will be performed between the business hours of 8:30am to 4:00pm.
7. GOVERNMENT-FURNISHED EQUIPMENT AND INFORMATION: The Government will provide the facility at which the services will be provided.
8. PERIOD OF PERFORMANCE:
Base year: August 1, 2025 thru July 31, 2026 Option year one: August 1, 2026 thru July 31, 2027 Option year two: August 1, 2027 thru July 31, 2028 Option year three: August 1, 2028 thru July 31, 2029 Option year four: August 1, 2029 thru July 31, 2030
9. PERSONAL CONTACTS:
Personal contacts include patients, patient’s families, and IHS employees.
10. QUALITY CONTROL:
The primary agent/official responsible for monitoring the performance on this contract in a project officer capacity and who will be verifying that all services have been received and delivered will be Mr. James Reed, Imaging Supervisor Crow/Northern Cheyenne Hopsital, Crow Service Unit.
11. SECURITY: No specialty clearances are needed.
