About this file
This document contains survey questions for evaluating photovoltaic module quality and safety criteria for a federal contract opportunity to design and construct onsite photovoltaic systems at a facility in Hilo, Hawaii. The solicitation requires testing module components and completed modules according to standards from the National Renewable Energy Laboratory to verify compliance with quality management systems and pass accelerated testing requirements. Modules must undergo testing for UV exposure of encapsulants and backsheets, matched component coupons, UV exposure of cables and connectors, UV exposure and thermal testing of junction boxes, and thermal cycling, dynamic mechanical load, system voltage, and hot spot testing of completed modules. Responses to survey questions for each proposed module must include results of accredited laboratory tests verifying passes or failures for each test criterion.

Solar PV Module Quality and Safety Selection Criteria - Questions
The Solar Module Quality and Safety Selection Criteria is an adjectival evaluation factor. Modules and the associated module sub-components listed below to be reported on do not need to pass all tests in order to be viewed as acceptable.
The following module accelerated testing questions mirror the recommended testing steps detailed in the National Renewable Energy Laboratory (NREL) report “Photovoltaic Module Qualification Plus Testing” 2013 and is available online here:
http://www.nrel.gov/docs/fy14osti/60950.pdf
Each proposed module that is constructed of crystalline silicon cells laminated between a glass front sheet and polymeric backsheet and encapsulated in ethylene vinyl acetate (EVA) will be evaluated for quality and safety through the NREL Qualification Plus testing recommendations.
Contractors that propose module technologies other than that covered in the Qualification Plus Test recommendations are required to report the module manufacturer’s quality manufacturing programs in place as well as what accelerated tests have been conducted by
Answers to the following module quality questions must be based on tests done at an accredited laboratory, accredited by either an International Laboratory Accreditation Corporation (ILAC). When a manufacturer submits test data for the component testing, it must be accepted by the independent test lab according their data acceptance procedures. Both the certificate and test report shall indicate that submitted data were accepted by the test lab.
One survey form for each module proposed by contactor.

ISO 9001 Quality System Audit must be from an Certification Body approved by ANSI-ASQ National Accreditation Board (ANAB). A Certification body approved by a similar organization to ANSI such as International Accreditation Forum (IAF). An IAF accredited organization from either inside or outside the United States is acceptable.
