Performance Work Statement
FA4610-20-P-00XX
PERFORMANCE WORK STATEMENT (PWS)
FOR
583rd MMXS
INDUSTRIAL EQUIPMENT MAINTENANCE
14 May 2020
VANDENBERG AFB, CA
1.0. Objective:
For more than 50 years, the 583rd Missile Maintenance Squadron (583rd MMXS), a geographically separated unit assigned to the 309th Missile Maintenance Group at Ogden Air Logistics Complex, has provided depot level launch support for every Intercontinental Ballistic Missile test from Vandenberg AFB, CA to include all active missile silos in the Air Force (i.e. F.E. Warren, Malmstrom and Minot AFB). Every Minuteman III ICBM launch requires both the Missile Suspension System and the launcher closure door components to work perfectly for every launch. The launcher closure door components are replaced and rebuilt after every test launch and the Missile Suspension System is rebuilt after every four launches. 583rd MMXS provides on-base Depot Level support to the 576th Flight Test Squadron and the critical ICBM Test Launch Program they perform in support of National Security. The purpose of the resulting contract is to provide preventative maintenance and support for the industrial equipment utilized by the 583rd MMXS to support the Follow On Test & Evaluation (FOT&E) program & Programmed Depot Maintenance (PDM) program with Depot level fabrication and repairs of the Minuteman weapon system.
2.0. Scope of Work:
2.1. Periodic Maintenance. The Contractor shall furnish all labor, equipment, materials, supervision, tools, and transportation necessary to maintain the equipment listed Attachment 1, Equipment List.
2.1.1. The Contractors shall perform quarterly maintenance in accordance with (IAW )the equipment’s associated Commercial Off the Shelf (COTS) Manual Procedures or manufacturer’s recommendation (when applicable).
Calibration and certification shall be performed IAW industry standards by professionally trained/certified (if applicable) technicians who are fully qualified and have the technical expertise to perform the work required to maintain or repair the covered equipment.
2.1.2. Maintenance Tasks include:
Check oil levels and refill as necessary.
Inspect belts, motors, lubrication systems and way wipers.
Make minor adjustments and/or repairs/ Test and operate equipment, check all speed and gear ranges.
Measure the backlash on all screws and nuts.
Annotate Service, Inspection or repair performaed with date on the associated 244 Forms. (FORM 244 attached to all machines)(Example attachment 5)
Provide a written status report addressing each individual piece of equipment’s current condition and identifying any necessary repairs.
2.2. Emergency Repairs. The Contractor shall furnish all labor, equipment, materials, supervision, tools, and transportation necessary to diagnose and provide emergency repair (when applicable) of the equipment listed Attachment 1, Equipment List.
2.2.1. In the event a piece of equipment should require additional maintenance outside the periodic maintenance schedule or should a piece of equipment require emergency repair, the Contractor shall provide an on-site technician within 48 hours of notification by the customer.
2.2.2. The Contractor shall provide the Contracting Officer with a Diagnosis Report and a Modification Cost Proposal (Attachment 2) that itemizes the labor categories, man-hours, materials, and other associated costs required to perform the necessary repairs. The Contracting Officer shall evaluate the Modification Cost Proposal [with the advice and recommendation of the applicable Government Subject Matter Expert (SME)] and shall make a scope determination regarding the proposed work. Should the proposed work be determined to be within the scope of the terms & conditions of the contract and within the scope of the original competition, then the Contracting Officer shall negotiate with the Contractor to incorporate the proposed work via a contract modification.
The Contractor shall not perform additional work without an executed contract modification.
2.3. Equipment Moving/Equipment Removal. The Contractor shall have the ability to move equipment / incorporate new industrial machinery into / remove obsolete industrial machinery from the 583rd MMXS facilities. In the event equipment movement/removal is required, the Contractor shall furnish all labor, equipment, materials, supervision, tools, and transportation necessary to assess site/equipment conditions and provide necessary equipment movement/removal (when applicable).
2.3.1. In the event a piece of equipment should require movement / install / removal, the Contractor shall provide an on-site technician within a reasonable amount of time upon notification by the customer.
2.3.2. The Contractor shall provide the Contracting Officer with a Diagnosis Report and a Modification Cost Proposal (Attachment 2) that itemizes the labor categories, man-hours, materials, and other associated costs required to perform the necessary repairs. The Contracting Officer shall evaluate the Modification Cost Proposal (with the advice and recommendation of the applicable Government SME) and shall make a scope determination regarding the proposed work. Should the proposed work be determined to be within the scope of the terms & conditions of the contract and within the scope of the original competition, then the
Contracting Officer shall negotiate with the Contractor to incorporate the proposed work via a contract modification. The Contractor shall not perform additional work without an executed contract modification.
3.0. QUALIFICATIONS
The Contractor shall ensure the assigned technicians performing maintenance and repair work on the covered equipment are professionally trained, certified (if applicable), fully qualified technicians with the technical expertise to perform the work required.
4.0. Hours of Operation
4.1. The Contractor is responsible for conducting business, during normal hours of operation between the hours of 6:00 a.m. and 4:30 p.m. Pacific Standard Time (PST), Monday through Thursday excluding weekends and recognized federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. The Contractor is required to perform services during normal hours of operation (standard hours) unless provided specific written authorization by the Contracting Officer to work Special Hours. The Contractor must submit a request and justification for working non-standard hours to the Contracting Officer at least 3 working days in advance. Inclement weather, operational delays, and security related delays might affect the contractor’s work periods. The Contractor may be required to temporarily stop work for operational requirements, special events, installation exercises, or while Distinguished Visitor (DV) familiarization visits/tours are conducted. Notification to the Contractor to shall be provided by the Contracting Officer as required. The Contractor must make allowances for these events and resulting lost time will not be reimbursable.
4.2. Recognized Federal Holidays. The Contractor is not required to perform services on recognized federal holidays unless provided specific written authorization by the Contracting Officer to work Special Hours. The following is a list of recognized federal holidays:

5.0. Environmental Requirements
The Contractor shall adhere to all environmental requirements in addition to any other environmental coordination and laws applicable. Refer to PWS Attachment 3 – Vandenberg AFB Environmental Specifications Section 01 57 20
5.1. All materials to be removed and/or disposed of by the Contractor shall remain the property of the Government.
5.1.1. The Contractor shall make every effort to recycle materials generated by this effort to include but not limited to woods and metals. The Contractor shall contact the Vandenberg Recycling Center (805-605-1143/0102) to coordinate disposal of generated waste or recycleable materials.
5.1.2. The Contractors shall coordinate with DLA Disposition Services (previously “DRMO”) at 805-606-9852 for proper disposal of items noted in PWS Attachment 4 – 30 CES GovStuff Instructions.
6.0. Hazardous Material Requirements
6.1. The contractor shall first obtain approval from the HAZMART for all HAZMAT
(including pesticides) usage on VAFB except for the commodities exempted on the Installation Management Flight approved exemption list. The contractor with access to government computer must attend applicable training for the installation’s hazardous material authorization and tracking processes. The contractor must submit an AF Form 3952, Chemical/Hazardous Material Request Authorization (or an electronic equivalent) with current manufacturer’s Safety Data Sheets (SDSs) for all materials purchased outside of the HAZMART supply system. Notify HAZMART to establish a shop account code and 30 CES/CEIEC, Environmental Support at (805) 605-3870 or (805) 605-7573 for authorization and approval process before using any hazardous materials on VAFB. The HAZMART Environmental Support staff will process, review and approve all submitted HAZMAT once in compliance. All HAZMART registered HAZMAT shall have appropriate bar codes issued and must be attached to each HAZMAT container.
Upon contract termination, contractor must remove all excess HAZMAT on the job site and clear or return all unused and issued bar codes to the HAZMART located in Building 5500, Bay “C” Room 10, and can be reached at (805) 605-3870. Refer to PWS Attachment 3 – Vandenberg AFB Environmental Specifications Section 01 57 20
6.2. Contractor The contractor shall provide a monthly hazardous materials usage report with corresponding container’s bar code number to the HAZMART Environmental Support to include electronic reporting NOTE: Contractors reporting all of their monthly hazardous materials usage through the HAZMART should meet the air emissions, Emergency Planning and Community Right-to-Know Act Section 313, Toxic Chemical Release Inventory (TRI), solvent usage, and HAZMAT VAFB database/report requirements. Refer to PWS Attachment 3 – Vandenberg AFB Environmental Specifications Section 01 57 20
6.3. Asbestos Containing Material (ACM).
6.3.1. In the event the Contractor should encounter ACM during performance, the Contractor should immediately cease work and shall immediately contact Mr. Daniel Carson (Hazardous Waste Program Manager, 805-606- 2359) in Bldg. 11146. The Contractor shall also immediately notify the Contracting Officer. Refer to PWS Attachment 3 – Vandenberg AFB Environmental Specifications Section 01 57 20
7.0. Historical Drawings & Site Condition Data
7.1. The 30th Civil Engineering Squardron (30 CES) will provide the Contractor access to any existing, available and allowable engineering plans, drawings, diagrams, aerial photographs, digitized map files, etc. to facilitate evaluation of the site(s), as needed for this effort. However, responsibility for field verification and accuracy shall remain with the Contractor.
7.2. Drawings (if provided) shall not be construced as complete or accurate records of existing site conditions. The Contractor shall perform research into the available historical records (only those records pertinent to this project) located in the 30 CES Vault (if applicable). Use of available historical records provided by the Government does not relieve the Contractor of responsibility for verification and validation of existing site conditions, coordination of existing conditions in parallel with proposed requirements, and above-grade/below-grade condition assessment.
7.3. Failure to verify/validate existing site conditions does not relieve the Contractor of the responsibility to provide a complete, operating, and acceptable system.
8.0. Government Owned Property
8.1. All materials gathered and/or developed in the performance of these tasks listed shall be returned to and become property of the Government; and shall not be used and/or distributed by the contractor without specific written permission of the Contracting Officer (CO).
9.0. Government Furnished Items & Services
9.1. Equipment/Material. The Contractor shall provide all equipment, materials, and tools required to perform work.
9.2. Utilities. Electrical outlets are available in most areas. The Contractor shall not change or modify any utility system or component; or connect any Government property, equipment or system without prior Contracting Officer review and writing approval authorizing change/modification. The Contractor shall practice energy/resource conservation in its use of Government provided utilities.
9.3. Facilities. The Contractor shall provide its own portable toilet and sanitary facilities.
9.4. Security, Fire, And Medical Response Services. The Government shall provide police and fire protection. In the event of a medical emergency, base ambulance service for transporting an injured employee to a local hospital is available on a cost reimbursement basis. Emergency services may be obtained by calling 9-1-1 from a base phone or 805-606-3911 on a cellular phone. The Government will provide initial medical treatment for life-threatening emergencies. It shall be the responsibility of the Contractor or Contractor employee to seek continued medical treatment if needed. The Contractor or employee shall reimburse the Government for all Government-provided medical services.
10.0. Work Coordination
10.1. Work Clearance Request. The 30 CES Customer Service (Bldg. 11439) will assist the Contractor with efforts to fully complete the Base Civil Engineering Work Clearance Request (AF 103). It is the Contractor’s responsibility to have a completed and approved AF 103 prior to beginning any activities including clearing, laydown, mobilization, grubbing, exploratory geophysical work, etc. It is also the Contractor’s responsibility to maintain any markings provided by Base agencies.
10.2. AF 103 Coordination. Coordination of the AF 103 requires a physical presence to carry the document around to multiple agencies on VAFB for coordination. AF 103s will require the Environmental Protection Plan (EPP) to be completed before the final signatures can be obtained. While assistance may be granted towards completion of the AF 103, all responsibility is on the Contractor to coordinate and accomplish. Multiple AF 103s may be accomplished for a given project, within reason. The Contractor may wish to accomplish separate AF 103s for different aspects of work. The Contractor must provide concise maps of the areas involved in the work including major aspects of the work locations.
10.3. Staging Area. The Base CE will identify space on the jobsite or in a base contractor staging area where the contractor may deliver non-hazardous materials within the installation. An approval letter from Base Planning is required for laydown areas. These areas must be included on a map and coordinated via an AF 103.
11.0. Applicable Documents/Permits. The Contractor shall identify and comply with all applicable federal, state, and local statutes; Air Force/ Military instructions, manuals, handbooks, regulations, guidance, and policy letters; Executive Orders; American Petroleum Institute Codes; National Association of Corrosion Engineers; National Fire Protection; Steel Structures and Painting Council; National Electrical Code; Uniform Fire Code; Occupational Safety and Health Administration (OSHA) regulations; Environmental Protection Agency regulations; and International Building Code including all changes and amendments in effect on the date of issuance of each contract. It is the Contractor’s responsibility to identify and comply with all applicable and relevant requirements.
It is the responsibility of the Contractor to identify all permits, clearances and other documents required for this project.
12.0. Security Requirements
12.1. At all times, while on Vandenberg AFB (VAFB) premises, Contractor employees shall comply with VAFB security requirements imposed by the Installation Commander. The Contractor shall obtain all necessary passes, decals, badges, or other items required for access to perform contract services at VAFB. The Contractor shall be responsible for accountability of all such media issued to Contractor personnel and these media shall be surrendered to the 30th Security Forces Squadron (30 SFS) Pass and Registration Section upon completion or termination of the contract or upon termination of an individual’s employment with the Contractor. The Contractor shall comply with all VAFB policies regarding entry to the installation, rules of the road, contingency operations compliance, and photography prohibitions. Contractor employees are authorized entry to the installation only for the purpose of work under this contract, and are not authorized in other facilities or areas without prior Government authorization. Due to safety/security requirements, Contractor personnel shall be required to fluently speak, understand, read, and write English.
12.2. Main Base Access. Entry requirements are subject to change, without notice, due to installation and world situations. For base access all contractor employees are subject to a background check through the Federal Bureau of Investigation. Current security access restrictions at the Santa Maria Gate (Main Base Entry Gate) require all Contractor vehicles (which are the size of a standard bread delivery truck or larger) to enter only through the Lompoc Gate (located at intersection of Santa Lucia Road and Pine Canyon Road). There is a vehicle check pull-off area at that gate and the Contractor will be required to submit to a vehicle search prior to gaining entry to the base. Contractor should plan and allow for delays as a result of in-place security procedures.
10.2.1 The Contractor shall coordinate access to facilities with the 583rd MMXS
POC.
10.2.2 The Contractor shall brief all personnel that they shall immediately leave any area upon notification by the Contracting Officer, constituted authority, or the Security Forces, when directed, as occasional operational requirements, real world activities, or emergencies dictate.
10.2.3 The Contractor shall not employ persons for work on this contract if such employee is identified to the Contractor by the 30th Security Forces Squadron (30 SFS) as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population. Due to safety/security, all Contractor personnel shall be able to fluently speak, read, and write English.
10.3 Contractor Vehicles. Each Contractor vehicle shall have the name of the
Contractor exhibited on each side of the vehicle. The Contractor shall provide company identification tags and vehicle identification at no additional cost to the Government.
10.4 Physical Security. The Contractor shall be responsible for safeguarding all government equipment, information and property provided for Contractor use.
10.5 Contractor Personel.
10.5.1 Contractor personnel shall present a neat appearance and be easily recognizable. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges that contain the company name and employee name.
10.5.2 Contractor personnel shall comply, and ensure that Sub-Contractor personnel comply, with current federal, state, local, Department of Defense (DoD), and Air Force regulations, plans, and policies including all applicable parts of the Air Force Occupational Safety and Health Standard (AFOSH) and United States Department of Labor Occupational Safety and Health Administration (OSHA).
10.5.3 The Contractor shall not employ any person who is an employee of the
United States Government if the employment of that person would create a conflict of interest, nor shall the Contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with (IAW)
DoD 5500.7-R, "Joint Ethics Regulation (JER)", "Standards of Conduct," Dated August1993.
11.0. Contractor Manpower Reporting
11.1. The Contractor shall report ALL Contractor labor hours (including Subcontractor labor hours) required for performance of services provided under this via a secure data collection site. The Contractor is required to completely fill in all required data fields at http://www.ecmra.mil.
11.2. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September.
While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.
12.0. Deliverables
12.1. Meeting Minutes. The Contractor shall take meeting minutes to include the recording of the attendance list. These shall be submitted via email to the Contracting Officer within one (1) business day of the meeting being held.
12.2. Equipment Status Report. After each periodic maintenance visit, the Contractor shall provide a written status report addressing each individual piece of equipment’s current condition and identifying any necessary repairs.
13.0. Invoicing. The Contractor shall invoice electronically to the Defense Finance Accounting Service (DFAS) utilizing the Wide Area Work Flow (WAWF) application on the Procurement Integrated Enterprise Environment website (www.wawf.eb.mil) within five (5) working days of completion of work. The Contractor shall notify the 583rd MMXS POC identified in the WAWF clause of the contract upon submittal of invoice for processing.
The Contractor shall provide supplemental documentation with the invoice to be uploaded in iRAPT. Supplemental files shall include work orders issued and other documentation that was generated for each task (inspection records). Supplemental files are used to validate invoice pricing.
