About this file

This Statement of Work (SOW) details the Phoenix Area Indian Health Service's (PAIHS) requirements for media destruction and IT asset disposition services. The scope involves securely collecting and destroying approximately 1,067 hard disk drives, 365 laptops/desktops, 182 mobile phones, 243 tape/data cartridges, 50 SD cards, and two boxes of computer memory from multiple office floors. The contractor must be a certified eWaste recycler, FACTA and HIPAA compliant, and responsible for providing personnel, containers, secured vehicles, and destruction services following NIST Special Publication 800-88 Revision 1 Guidelines for Media Sanitization.

Key requirements include secure chain-of-custody, destruction methods like disintegration and incineration, providing a comprehensive Certificate of Destruction with each invoice, and ensuring all materials are completely destroyed before recycling. Pickup will occur at the Renaissance II building in Phoenix, Arizona during regular work hours (8am-5pm), with the contractor responsible for loading, transporting, and securely destroying all media. The destruction process must render data unrecoverable, with particular emphasis on solid-state drives being disintegrated or incinerated due to the age and potential data risk of the existing hardware.
