SUBJECT: Request for Past Performance Information for Solicitation W912LP-21-R-0008 Technical Instruction Services for a Detecting Danged/Armed Suspect course

1. The USPFO-IA Purchasing & Contracting Office is in the process of selecting a contractor to provide the subject services for the Midwest Counterdrug Training Center (MCTC). All Offerors submitting proposals in response to this solicitation are required to provide references for similar services in order to assist the government in evaluating past performance.

2. Please complete the attached Past Performance Questionnaire (PPQ) and submit it directly to the individual identified in paragraph 4 below. The PPQ should completed by the individual within your organization who is most familiar with the Offeror’s performance on the identified contract(s). The PPQ should reflect an honest evaluation of the Offeror’s performance based on objective facts.

3. As your knowledge is crucial to our evaluation of the Offeror’s past performance, we request that you provide responses to all questions. A “Not Observed” response may be appropriate when no evidence is available to evaluate a specific question. Please provide a rating for each question, and include narrative comments to support your ratings. Typed or legible handwritten responses are acceptable.

4. Please submit the completed PPQ via email to kelsey.l.letcher.civ@mail.mil or mail to USPFO- P&C, Attn: Ms. Kelsey Letcher, 7105 NW 70th Avenue, Bldg. 3475, Room 104, Johnston, IA 50131-1824.

Email responses are preferred. PPQ submissions are due no later than 11:00 a.m. central time on August, 12, 2021. The contract specialist may contact you to discuss questionnaire responses. As this questionnaire relates to an ongoing source selection, once completed it should be handled as Source Selection Sensitive Information in accordance with FAR 3.104. We will safeguard the information, to include respondent’s name, from disclosure outside the Source Selection Evaluation Board (SSEB).
