CUI

Submission Requirements. Offerors shall provide the following submissions for each course being proposed.

(a) Technical, Past Performance and Price Submissions. Offerors shall submit an original (1) and two (2) hard copies of complete proposals, PLUS one (1) electronic copy of the complete proposal on CD/DVD(s).

Electronic copies may not be submitted on flash drives/thumb drives, as their use is prohibited on the government’s computers. Hard copy submissions shall be provided in binders, with a separate divider for each Volume, and binder/divider/tab titles as indicated in the table below. CD/DVD(s) shall include folders for each Volume; with folders/sub-folders/file names as indicated in the second column in the table below. All information that is included within the hard copy binders MUST also be included on the CD/DVD(s). Offerors shall clearly indicate the solicitation number on all volumes of the proposals. Failure to provide complete proposals in both hard copies and electronic copy will result in the proposal being found non-responsive and it may not be considered for award.

Volume I: Course Content

1a Course Overview, Performance

Objectives, and Training Approach

Narrative of course overview, performance objectives;

and training approach, to include, but not be limited to, the number of instructors provided for each iteration, and method(s) of instruction (lecture, practical exercise, etc.)

to be utilized in conducting the course.

1b Course Agenda Course Agenda shall be broken down by day and hour.

1c Topic Overview A brief overview of what will be covered for each topic listed in the course agenda.

1d Slide Sets (if used) Slide sets shall be numbered in order of presentation.

Presentation order should correspond with Course Agenda. Slide sets may be printed OR provided on a CD/DVD (NOT flash drives) placed in the appropriate location within the hard copy binders.

1e Videos (if used) Provide electronic copies of videos and identify where they are used within the course agenda (or indicate which slides they’ll be embedded in). All videos must be viewable using Windows Media Player™. Videos may be provided on a CD/DVD (NOT flash drives) placed in the appropriate location within the binder.

1f Practical Exercises (if used) Provide a list of the practical exercises. For each practical exercise identify: the course topic(s)/learning objective(s) it corresponds to, and a thorough explanation of how the practical exercise will be conducted.

1g Pre-test and post-test Pre-tests and post-tests may be written, practical exercise, or a combination of the two. Provide a copy of all written tests or segments thereof. If all or part of the test is practical exercise, provide an explanation of how the test will be conducted. If graded, indicate the minimum passing score.

1h Student handbook (required) and other reference materials (optional)

Student handbook (required): Shall be specific to this course, and a hard copy must be provided with the proposal.

Other reference materials (optional): Offeror shall list any commercially-published references that will be provided as part of student take-home materials, including ISBN #, Title, Publisher. Offer is not required to provide hard copies of commercially-published references with the proposal. For each reference, indicate if it will be provided to students in hard copy or electronic format.

1i Contractor-provided materials/equipment Offeror shall provide list of materials and equipment they will provide to facilitate the course – do NOT merely indicate that “materials listed in PWS will be provided” or similar statement.

1j Student-provided materials/equipment

Offeror shall provide list of materials and equipment that students must bring to participate in the course, if applicable.

Volume II: Offeror/Instructor Qualifications

Volume II of the proposal shall identify the Offeror (prime contractor and subcontractors/teaming partners)

2a: Offeror Qualifications 2a1: Offeror Qualifications Prime Contractor Qualifications Provide a narrative of the Prime Contractor’s qualifications to provide the requested training. If PWS states that a specific certification, license, or other formal accreditation is required, provide a copy of that certificate, license, or accreditation document.

2a2: Offeror Qualifications Subcontractor/Teaming Partner Qualifications

Provide a List of Subcontractors and/or Teaming Partners, and their roles/responsibilities. Provide a narrative to support their qualifications to perform their areas of responsibility. If their area of responsibility requires a specific certification, license, or other formal accreditation as stated in the PWS, provide a copy of that certificate, license, or accreditation document.

2b: Instructor Qualifications

2b1: Instructor Qualifications List of proposed instructors Offerors shall submit for consideration only instructors who meet all mandatory qualification requirements. The following documents shall be submitted for each proposed instructor.

Each proposed instructor shall complete and sign the Instructor Qualification Checklist, and obtain the signature of an official qualified to validate the information provided.

2b3: Instructor Qualifications List of Courses Taught in the past 3 years (for each proposed instructor)

List only courses that are similar in duration (training hours/days), content and target audience to the course being proposed. Include course titles, dates, locations, and hosting agencies/organizations.

2b4: Instructor Qualifications Qualification Documents (for each proposed instructor)

If PWS states that a specific certification, license, or other formal accreditation is required, provide a copy of that certificate, license, or accreditation document.

Instructor(s) will NOT be credited with the qualification in the evaluation if required documents are not provided.

Volume III: Past Performance Offeror’s Past Performance

References Provide a list of companies/agencies that Offeror has requested to submit Past Performance Questionnaires (PPQs), including:

a. Company/Agency Name (Offeror)

b. Point of Contact (POC) Name (Offeror)

c. Company/Agency address (Offeror)

d. Company/Agency phone number and POC direct number or extension, if applicable (Offeror)

e. POC email address

f. Brief description of services (courses) provided by Offeror to the Company/Agency

g. Date(s) of services provided by Offeror to the Company/Agency

h. Indicate whether Offeror was prime contractor or subcontractor for referenced services

Offeror shall send PPQs to at least three (3) but no more than five (5) companies or agencies, other than MCTC, for whom they have provided training that is similar in content, length, and target audience to the proposed training. Offeror shall complete the “Offeror” information on the PPQs before sending to the references. The referenced company/agency shall send the completed PPQs directly to the Contract Specialist.

Offeror is responsible for following up to ensure that the reference companies/agencies have submitted their completed PPQs prior to the PPQ due date/time.

Volume IV: Price Proposal 5a: Price Proposal Price Proposal Offeror shall provide a Firm Fixed Price (FFP), per course iteration, for each Contract Line Item (CLIN) in Section B of this solicitation. The FFP shall be inclusive of all labor, materials, equipment, mailing/shipping, and travel required to perform the training.

5b: Representations and Certifications

Representations and Certifications

Offeror shall complete all representations and certifications within their System for Award Management (SAM) profile at http://www.sam.gov. Any provisions listed within the “Full Text Provisions and Clauses” document which are not completed within SAM shall be printed, completed and included with the proposal.

5c: Section 889 Certifications Section 889 Certifications Offeror shall complete FAR provisions 52.204-24 and 52.204-26 within the “Section 889 Certifications” document, and return the signed document with the proposal.

Offeror shall complete FAR provision 52.229-11 document and return the signed document with the proposal.

(b) Technical proposals shall clearly document, in the Offeror’s own words, how the Offeror will perform the services/training required by the PWS. Proposals that merely mimic language from the solicitation or PWS will result in a negative technical rating.

(c) Offeror is responsible for ensuring that all proposal materials (including Past Performance Questionnaires) have been delivered to the Contract Specialist prior to the proposal due date/time. Late proposals will NOT be accepted. Recommendation: Ship proposals using a trackable delivery method and email tracking number(s) and delivery confirmation(s) to the Contract Specialist.

(d) Proposal materials will NOT be returned.
