1.0. DESCRIPTION OF SERVICES. The Contractor shall provide all personnel, equipment, tools, materials, supervision, transportation, and other items and services necessary to perform installation of Contractor-Furnished Luxury Vinyl Plank (LVP), carpet tile, and matching cove base defined in this Performance Work Statement (PWS).
1.1. The Government will ensure all facilities that require removal of existing flooring/cushion under a specific work order will be free of asbestos hazards.
1.1.1. Asbestos Containing Material Identification (ACM). The Contractor shall not start any work that may disturb any materials in a facility until the Government has identified all materials as asbestos-free. The Contractor shall not commence work without the Contracting Officer’s Representative (COR) approval that the performance area is asbestos-free.
1.2 Materials

1.2.1.1. Provide product data for LVP. Samples of material shall be submitted to the COR for color selection prior to installation.
1.2.1.2. Maintenance data for LVP. Contractor shall submit maintenance data to the COR upon job completion.

1.2.2. Carpet Tile

1.2.2.1 Dimensions. The Contractor shall perform an on-site inspection of the work area to determine requirements for carpet installation and confirm dimensional requirements. It is the responsibility of the contractor to notify the COR, within two (2) working days prior to the performance start date, of any discrepancies in the dimensions between the data provided by the Government and the contractor’s survey.
1.2.2.2. Carpet Sourcing Procedures. Contractors shall abide by the mandated Carpet II program. Information, including the Ordering Guide, Mandatory Use Policy Letter, and Waiver Request form can be found at:

1.2.3. Vinyl Cove Base. The Contractor shall furnish and install vinyl cove base as directed.
Removal of existing materials shall be as required to support the work.
1.2.3.1 Materials. 120 feet coils: 1/8-inch thick, homogeneous vinyl or rubber composition, tapered or bullnose edge, color to match existing, or as selected from standard colors available.

1.2.3.2. Cove Base Adhesives. Waterproof, stabilized type as recommended by wall base manufacturer to suit material and wall conditions.
1.3. Furniture. Moving of furniture is the responsibility of the facility occupants. The Contractor shall not be required to move furniture before or after installation.

1.4. Flooring Removal. The Contractor shall remove all existing flooring/cushion and cove base as specified per task order and dispose of it properly only at authorized off base locations.
1.5. Prepare Floor. The Contractor shall ensure concrete and tile floor surfaces are smooth and level to the eye to provide a proper surface for installation of LVP or carpet tile. The Contractor shall fill all cracks, holes, and crevices with material compatible to the floor as directed by the manufacturer’s specifications. Contractor shall comply with manufacturer's installation specifications to prepare substrates indicated to receive flooring. Apply concrete slab primer, if recommended by flooring manufacturer, prior to applying adhesive. Apply according to manufacturer's directions. The contractor’s responsibility for such floor repair preparation shall be limited to the use of no more than one gallon of patching compound per ten square yards of floor area to be covered. The contractor must notify the contracting officer if floor repairs in excess of this amount are identified and request further instruction
1.6. Door Clearances. The Contractor shall ensure clearance between the bottom edges of doors and the level floor surface of newly installed LVP or carpet tile. All doors shall open freely or close without dragging the newly installed LVP. If the door is dragging, the Contractor shall notify the facility manager and the COR to have the Civil Engineers correct the problem.
1.7. Unaffected Areas. Protect all unaffected areas with plastic sheeting, drop cloths, periodic vacuuming, or sweeping from work debris.
1.8. Remove Rubbish. The Contractor shall remove all rubbish, wrapping paper, salvages and scrap daily. The Contractor shall not leave removed waste material inside the facility over-night.
Waste materials generated by the Contractor shall be disposed of to an authorized off base facility daily.
1.9. Final Cleaning. The Contractor shall remove spots, smears, and other foreign matter with an approved solvent. Use a solvent, which will not discolor or damage tile surface to include cove bases.
1.10. The Contractor shall provide and install T-cap edge strip, using materials consistent with industry practices, at all transitions.

3.0. GOVERNMENT FURNISHED PROPERTY/SERVICES/EQUIPMENT
3.1 Available utilities: Subject to availability, the Government, without charge to the contractor and from existing outlets, will furnish electricity, gas, water, and sanitary sewer capacity. The contractor at their own expense, shall install, maintain, and remove any necessary temporary connections and distribution lines. If temporary lines are associated with a specific task order, they shall be removed prior to final acceptance.
3.2. Conservation of Utilities. The Contractor shall adhere to all base level utility conservation practices. The Contractor shall be responsible for operating under conditions, which preclude the waste of utilities.
3.3. CONTRACTOR-FURNISHED ITEMS AND SERVICES.
3.3.1. Accessories. The Contractor shall furnish flooring, edge strips, glue, adhesive, filler, and transitional strips and like items for a complete installation and finished appearance. If furnishing carpet, contractor must abide by paragraph 1.2.2.2, Carpet Sourcing Procedures.
3.3.2. Notification to Commence Work. Upon receipt of task order the Contractor shall begin work installation within five (5) working days after service notification is received from the contracting officer. The contractor will receive work order information from the COR by means of face-to-face communication, e-mail, fax or phone and acknowledgment of job order must be submitted to COR. The Government will provide the contractor with the facility number and floor plan.
4.0. GENERAL INFORMATION.
4.1. Quality Control. The contractor shall develop and maintain a quality control program/plan (QCP) to ensure the Flooring Installation services are performed in accordance with commonly accepted commercial practices and services identified in this PWS. The program/plan shall also include how the contractor will implement and maintain quality-accepted practices/procedures to identify, prevent, and ensure nonperformance and continual repeat of defective service does not occur. The written QCP shall be submitted to the CO for review within 30 days after award. The CO will review and either accept the QCP as written or provide feedback within ten (10) working days. If QCP is returned, contractor shall resubmit within five (5) working days, until plan is accepted. The plan shall specifically address the items listed above as well as the contractor's strategy to provide quality workmanship, continual process improvement and for correcting deficiencies as required.
4.1.1. Quality Assurance. The Government will conduct surveillance on the contractor’s performance under this contract. The Government has developed a contract specific Quality

Assurance Surveillance Plan (QASP). The Government will record all surveillance observations.
When an observation indicates defective performance, the COR will require the contract manager or representative to initial the observation indicating acknowledgment of deficiency. The initialing of the observation does not necessarily constitute contractor concurrence with the observation, only acknowledgment that the contractor has been made aware of the defective performance.
Government surveillance of tasks not listed in the Service Summary (SS) or by methods other than those listed in the SS may occur during the performance period of this contract. Such surveillance will be done according to standard inspection procedures of other contract provisions. Any action taken by the CO as a result of surveillance will be in accordance with the terms of this contract.
4.1.2. Government Remedies. The CO shall follow FAR 52.212-4, Contract Terms and Conditions-Commercial Items, for contractor’s failure to perform satisfactory services or failure to correct non-conforming services.
4.1.3. Performance Evaluation Meetings. The contract manager may be required to meet at least weekly with the COR and the Contracting Officer during the first month of the contract.
Subsequent meetings will be on a quarterly basis and/or as needed. The written minutes of all performance evaluation meetings shall be prepared by the Government. Should the contractor non-concur with the minutes; the contractor shall so state any areas of non-concurrence in writing to the CO within three (3) working days of receipt of the signed minutes. The minutes will be included in the contract file.
4.2. Hours of Operation. The Contractor shall perform the on-site services required under this contract during the following hours: 7:30 am - 4:30 pm, Monday through Friday, except on U.S.
holidays specified in section 4.2.1. of this PWS. No on-site work shall be scheduled or performed on holidays unless previously approved or directed by the CO. If the contractor desires to work on non-scheduled workdays, he/she must submit a written request to the CO for approval.
4.2.1. Holidays. All federal holidays are listed at https://www.opm.gov/policy-data-oversight/pay-leave/federal-holidays/. The contractor shall observe all federal holiday, except where the Service Contract Act specifies differently.
4.3. Security Requirements. HAFB is a closed military installation. The Air Force reserves the right to establish criteria for entry of all personnel, vehicles, and equipment onto HAFB. Access to the base may be further restricted as deemed necessary by the 49th Wing Commander (49 WG/CC) or his/her designee. Delays resulting from restricted access to HAFB will not be the basis for a claim by the contractor when the 49 WG/CC or his/her designee deems that such changes are necessary to protect the security of HAFB.
4.3.1. The contractor shall not hire an illegal alien to work or perform any service on HAFB.
Employees who are not U.S. citizens can only be persons possessing a current resident alien immigrant status (Green Card) or if a non-resident immigrant, must possess a current Employment Authorization Document. While on HAFB, non-U.S. citizen employees must be able to produce for inspection the appropriate credential: Green Card or Employment Authorization Document.
Any illegal alien found on the installation shall be immediately detained by the base security forces and barred from the installation.

4.3.2. Listing of Employees. The contractor shall maintain a current listing of employees that require access to HAFB. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the sponsoring agency unit security manager within 5-duty/business days after award. A redacted (no Personal Identifiable Information [PII]) copy of the list shall be provided to the COR and contracting officer. An updated listing shall be provided when an employee's status or information changes (i.e., terminated for cause, reassignment, retirement, etc.) or when a new employee is added.
4.3.3. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-Government owned vehicles. Contractor employees shall only be granted access to the installation to perform functions directly related to their respective contract.
4.3.4. The contractor shall ensure all employees are identified on a HAFB Visit Access Request (VAR). The contractor shall submit VARs through the contracting officer to the 49th Security Forces Squadron (49 SFS) Pass and Registration Office; VARs must be submitted no later than 7-duty/business days, and for foreign nationals, 14-duty/business days, prior to requiring access to HAFB. Recommend putting foreign nationals on a separate VAR.
4.3.5. Retrieving Identification Media. The contractor shall retrieve all identification media and/or visitor passes from employees who depart for any reason before the contract expires, i.e., terminated for cause, reassignment, retirement, etc. The contractor will notify the Government within 24 hours when unable to retrieve identification media from contract employees.
4.3.6. Traffic Laws. The contractor and its employees shall comply with base traffic regulations.
Traffic regulations include, but are not limited to:
4.3.6.1. Contractor employees shall wear a seatbelt when operating or riding in any military, company, or privately-owned vehicle while on the installation.
4.3.6.2. Speed limits on HAFB are 30 MPH unless otherwise posted. Be aware of reduced speed limits when driving in the housing and school zones.
4.3.6.3. All vehicle operators shall not use cell phones unless the vehicle is safely parked or unless they are using a hands-free device. The wearing of any other portable headphones, earphones, or other listening devices (except for hands-free cellular phones) while operating a motor vehicle is prohibited. Penalty: Privileges to operate a motor vehicle while on the installation may be suspended for 5 days, effective the first day after receipt of notice.
4.3.7. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their government, company, or private-owned vehicle while on HAFB.

4.3.8. For Official Use Only (FOUO). The contractor shall comply with DoDM 5200.01-V4/E3, DoD Information Security Program: Controlled Unclassified Information (CUI), requirements.
This manual sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
4.3.8.1. Records Management (RM) (2017). Records Management (RM) (June 2017). The contractor shall provide accountability and ensure all records and documents generated during the period of this contract are made available to the government at all times. All information may be maintained electronically. All publications and forms will be available for contractor's use through the Holloman Publications Web Site. Any publications or forms that are required for the performance of this contract and are not available electronically are required to be maintained manually.
4.3.8.2. Freedom of Information Act (FOIA) and Privacy Act (PA) (2017). The contractor shall comply with DoDM 5400.7, DOD Freedom of Information Act (FOIA) Program, DoD5400- R_AFMAN 33-302 and AFI 33-332, Air Force Privacy and Civil Liberties Program. These regulations set policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding Personally Identifiable Information (PII) material.
4.3.8.3. Contractor Records (June 2017). The contractor shall maintain all government records IAW AFMAN 33-363, Management of Records, paragraph 6.4., Contractor Support, in order to support the COR who is responsible for all lifecycle record management and all duties assigned to the organization's Record Professional (RP) (Tier-1 (T-1)). [AFI 33-360, Publications and Forms Management, Table 1.1 outlines description of the authorities for T-1.]
4.3.9. Reporting Requirements. Contractor personnel shall report the appropriate authority any information or circumstances of which they are aware which may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Employees shall be briefed by their immediate supervisor upon initial on-base assignment.
4.3.10. Security Training. Contractor personnel are required to attend security and OPSEC orientation under Homeland Security Presidential Directive (HSPD) Title 12. The Contractor shall schedule and ensure all assigned contractor personnel receive initial and annual refresher training through the unit Security Assistant in accordance with DoDM 5200.01 V3_AFMAN 16-1404V3 Encl. 5, DODM Information Security Program Protection of Classified Information. Contractors are also required to take the Controlled Unclassified Information (CUI) training.
4.3.11. Employee Background Checks. All contractor employees who do not have a current security investigation acceptable to the Wing Information Protection Office (IPO), and who require a Tier 1 (T1) investigation for the performance of their duties, shall contact the unit Security Assistants to complete an Electronic Questionnaires for Investigation (e-QIP), SF 85, Questionnaire for Non-Sensitive Positions, and an AF Form 2583, Request for Personnel Security Action. When required for contract performance, the Government will provide unclassified network access. Contractor employees will be held strictly accountable for actions they initiate on the network and will conduct business in accordance with USAF, MAJCOM, and HAFB
Attachment 1 Page 10 of 14 instructions and policies. The unit Security Assistants will ensure contractor employees have a favorable T1 investigation or favorable employment suitability determination from the unit Commander (for “No Determination Made” investigation results), and complete Information Assurance training prior to network access. Interim access may be granted in accordance with DODM5200.02_AFMAN16-1405, Air Force Personnel Security Program.
4.3.12. Physical Security. The contractor shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.
4.3.13. Key Control. The Contractor shall establish and implement methods of making sure all keys and/or combinations issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The Contractor shall not duplicate any keys issued by the Government. The Government requires keys/combinations to the contractor's area of responsibilities. The Contractor shall supply a key or combination to the Fire Department in the event of an emergency.
4.3.13.1. The Contractor shall immediately report to the COR any occurrences of lost or duplicated keys, compromised lock combinations, or use by unauthorized persons.
4.3.13.2. In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the contracting officer, to re-key or replace the affected lock or locks without cost to the Government. The Government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government and the total cost deducted from the monthly payment due the contractor.
4.3.13.3. The Contractor shall prohibit the use of keys or combinations, issued by the Government, by any other persons other than the contractor's employees and the opening of locked areas by contractor employees to permit entrance of persons other than contractor employees engaged in performance of contract work requirements in those areas.
4.4. Contractor Personnel Requirements.
4.4.1. Contract Manager. The Contractor shall provide a point of contact who shall be responsible for the performance of the work. The name of this person, and alternate(s), who shall act for the contractor when the contract manager is absent, shall be designated in writing to the CO five (5) working days prior to project start day. The contract manager or alternate shall have full authority to act for the contractor on all contract matter relating to the daily operation of this contract. The contract manager or alternate shall be available during normal duty hours within two (2) hours to meet on the installation with the designated personnel to discuss problem areas.
After normal duty hours, the manager and alternate shall be available within two (2) hours. They must be able to read, write, speak, and understand English.
4.4.2. Contractor Personnel. Contractor personnel shall present a neat appearance and be easily
Attachment 1 Page 11 of 14 recognized as contractor employees. This shall be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contains the company and employee name in English. Contractor personnel shall use protective clothing whenever they are performing tasks that could cause harm to the employees or damage to property.
4.4.3. The Contractor shall not employ persons to work on this contract if such person is identified to the contractor by the Government as a potential threat to the health, safety, security, general wellbeing or operational mission of the installation and its population.
4.4.4. Performance of Services during crisis declared by the National Command Authority. IAW DoDI 3020.37 Continuation of Essential DoD Contractor Services During Crisis and aforementioned Air Force implementation, and unless otherwise directed by an authorized Government representative, it is determined that services as identified in this PWS are non-essential for performance during a crisis.
4.5. Completion Time. The Contractor shall be required to notify COR when job is completed.
4.. Retreat. Retreat will be sounded, via the installation “giant voice” audio system, every Monday through Friday at 4:30 pm to signal the official end of the duty day and to pay respect to the U.S.
Flag. All vehicle operators shall come to a complete stop until the last note of retreat. Contractor personnel outside of a vehicle or facility shall face toward the direction of the sounding of retreat or towards the base flagpole (if visible) and remain silent until the last note of retreat is played.

Appendix C
Invoicing Instructions Utilizing the Government Purchase Card (GPC)
GPC will be the primary payment method for all Task Orders. The Contractor shall accept payment via GPC for any work as defined in this PWS, accomplished below $20,000.00.
Procurement Integrated Enterprise Environment (PIEE) (formally known as Wide-Area Workflow (WAWF):
Vendors will use PIEE WAWF to create and submit invoices to ensure compliance with the Prompt Payment Act.
Mandatory: Use of PIEE WAWF is mandatory when the GPC is used as a method of payment against a contract. In order to ensure proper routing of contract transactions paid with the GPC, use the code “CRCARD” in the Pay Official/DoDAAC field. The Government acceptor (COR) is required to review invoices WAWF. The Card Holder is required to verify acceptance has occurred in WAWF prior to submitting the invoice to the Certifying Officer (usually the A/BO) for certification.
