About this file
This document is a Quality Assurance Surveillance Plan (QASP) for a sole source contract to be awarded to Phillips Corporation, Federal Division, to provide and install a 5-axis spindle drive assembly, ensure proper ball bar calibration, and validate 5-axis cutting operability for a Haas VR-11 machine with serial number 1179903.
The key details are:
- The contractor shall provide spindle repair kit, remove existing spindle, install new spindle assembly, perform ball bar calibration, and validate 5-axis travel and software operability.
- Spindle assembly and replacement will be monitored by a qualified government operator, and operability will be verified through 100% inspection and 5-axis travel testing.
- Performance metrics include proper spindle alignment, successful ball bar calibration, tool changes, 2-axis rotational and 1-axis linear operation, and tool offset capabilities.
- The current hardware should be 100% operable upon service completion.

Quality Assurance Surveillance Plan
SPINDLE ASSEMBLY KIT SERVICE, REPLACEMENT, AND CALIBRATION
1. Description:
The Technology Development Directorate (TDD) requests a sole source contract be awarded to Phillips Corporation, Federal Division, hereby referred to as Phillips Federal, to provide and install a 5-axis spindle drive assembly, ensure proper ball bar calibration, and operability demonstrated with 5-axis cutting.
2. Purchase Requirements:
The contractor shall provide the following for the base contract for Haas VR-11, S/N 1179903:
a. Shipment of spindle repair kit part number 93-3388 to BLDG 5400
b. Removal of existing spindle, 93-3388
c. Installation of new spindle assembly part number 93-3388
d. Ball bar calibration of new spindle
e. Validation of all 5-axis travel for the machine
f. Validation of software operability with new spindle assembly
3. Method of Surveillance:
Spindle assembly and replacement will be monitored by a qualified and trained government operator to ensure proper installation and setup. Operability of equipment will be verified through a 100% inspection and 5-axis travels with machine controller.
4. Performance Metrics:
Performance metrics are met when:
a. Spindle demonstrates proper alignment
b. Spindle passes ball bar calibration
c. Spindle successfully changes tools from the rotary tool changer
d. Spindle operates in two rotational axis, one vertical linear axis
e. Spindle can successfully perform tool offsets
5. Acceptable Quality Level:
The current hardware should be 100% operable upon the service completion date.
