About this file
This document is an Appendix C form for Assurance of Compliance with Section 504 of the Rehabilitation Act of 1973, related to the Department of Health and Human Services (DHHS) regulations. The form requires contractors to affirm their commitment to complying with accessibility regulations for individuals with disabilities, specifically addressing employment and service provision requirements.
The form provides two options for contractors: those employing fewer than 15 persons and those employing 15 or more persons. For organizations with 15 or more employees, there is a requirement to designate a specific person to coordinate efforts to comply with DHHS regulations. The document includes blank fields for the contractor to provide details such as the name of the 504 compliance person, contractor name, address, and an authorized official's signature. An exception note indicates that smaller organizations (under 15 employees) may refer disabled individuals to alternative accessible service providers if significant facility alterations would be required to accommodate their needs.
