About this file

This document is Appendix C, an Assurance of Compliance form related to Section 504 of the Rehabilitation Act of 1973, which requires organizations to demonstrate their commitment to non-discrimination and accessibility for individuals with disabilities. The form is designed for contractors to affirm their compliance with federal regulations, specifically those imposed by the Department of Health and Human Services (DHHS).

The form allows contractors to indicate whether they employ fewer than 15 persons or 15 or more persons, with the latter category required to designate a specific coordinator responsible for ensuring compliance with DHHS accessibility regulations. By signing this document, contractors agree that their contracts will be extended based on the representations made, and the assurance is binding on the contractor, its successors, transferees, and assignees. An exception note provides guidance for smaller organizations (fewer than 15 employees) that may find facility modifications challenging, allowing them to refer disabled persons to alternative accessible service providers.
