About this file
This file is Appendix K – Bid Opportunity List.xlsx from the Wisconsin Department of Transportation, which serves as a compliance document required under 49 Code of Federal Regulations Part 26.11. The document establishes a standardized bid opportunity list that must be completed and submitted with all bids and proposals on DOT-assisted contracts, capturing information about all firms participating or attempting to participate in these projects, including prime contractors, consultants, subcontractors, subconsultants, and material suppliers regardless of their Disadvantaged Business Enterprise (DBE) status.
The form requires comprehensive information from all participating entities including bidder name, complete contact information (address, phone, email), DBE certification status, firm age, type of work performed, and annual gross receipts categorized into five tiers ranging from less than $500,000 to $5 million and above. Prime contractors and consultants must complete all data fields, while subcontractors and subconsultants need only provide name and contact information plus any available information for the remaining categories. The document includes multiple identical forms for capturing information on various subcontractors and subconsultants, ensuring comprehensive tracking of all firms involved in DOT-assisted transportation projects for federal compliance purposes.
