About this file

This file is a bid opportunity list form template issued by the Wisconsin Department of Transportation (WisDOT) in compliance with 49 Code of Federal Regulations Part 26.11. The form serves as an appendix to bidding documents and requires all firms participating in or attempting to participate on DOT-assisted contracts to submit completed documentation. The form mandates inclusion of all entities bidding on prime contracts, subcontracts, materials, and supplies on DOT-assisted projects, including both disadvantaged business enterprises (DBEs) and non-DBEs. For consulting companies, subconsultants expressing interest in teaming on specific DOT-assisted projects must also be listed.

Prime contractors and consultants are required to complete all form columns, including bidder name and contact information, DBE status, firm age, type of work, and annual gross receipts categorized into brackets ranging from less than $500,000 to $5 million and above. Subcontractors and subconsultants require prime contractors to complete columns for name and contact information plus any available information on the remaining data fields. The form includes multiple rows accommodating multiple subcontractor and subconsultant entries, each with identical annual gross receipts bracket options. This standardized template ensures consistent data collection across all DOT-assisted contract opportunities and maintains a comprehensive registry of participating firms for federal oversight and compliance purposes.
