About this file
Appendix J is a bid opportunity list form required by the Wisconsin Department of Transportation for all DOT-assisted contracts in compliance with 49 Code of Federal Regulations Part 26.11. The form is designed to track all firms participating or attempting to participate on DOT-assisted projects, including both Disadvantaged Business Enterprises (DBEs) and non-DBEs. Prime contractors and consultants must complete all columns of the form, including bidder name, contact information, DBE status, firm age, type of work, and annual gross receipts. Subcontractors and subconsultants require prime contractors to complete at minimum the bidder name and contact information columns, along with any available information regarding DBE status, firm age, type of work, and annual gross receipts. The form must be submitted with all bids and proposals related to DOT-assisted projects.
Annual gross receipts are categorized into five ranges: less than $500,000; $500,000 to $1 million; $1 million to $2 million; $2 million to $5 million; and $5 million and above. For consulting companies specifically, the bid opportunity list must include all subconsultants who contact the prime and express interest in teaming on a specific DOT-assisted project. The form serves as a comprehensive documentation tool to maintain transparency and tracking of all business participation in federally-assisted transportation projects throughout Wisconsin.
