About this file

This is a Vendor Disclosure Statement form required by Colorado Revised Statute 24-102-206 for the Thermal Opportunity Mapping contract opportunity in Colorado. The form must be completed and returned to the contracting agency by any vendor whose contract or subcontract services will be performed outside the United States or Colorado. The disclosure requirement applies to all state contracts and solicitations for services executed after August 3, 2007. Vendors are required to identify the specific country or state where services will be performed, explain the necessity or advantage of performing work outside the United States or Colorado, and provide the contract routing number and vendor name.

The form exempts certain contracts from the disclosure requirement, including those related to Medicare, the Colorado Medical Assistance Act, the Children's Basic Health Plan Act, and the Colorado Indigent Care Program. No pricing information, contract term, or specific service details are contained within this disclosure statement form itself, as it serves solely as a compliance document for vendors to declare whether work will be performed outside the specified geographic boundaries.
