One form must be submitted per entity, with asterisked information required for the Prime/Lead/Principal entity and all other legal entities.
In addition to individual entity certifications, the Prime Offeror or Sole Offeror must submit an explanation demonstrating satisfaction of 10 USC 4022(b)(d) Appropriate Use of Authority by selecting one of three compliance pathways: (1) at least one NDC or NRI participating to a significant extent in the prototype project with detailed explanation of contributions across seven specified factors and total estimated cost and percentage; (2) all significant participants other than the Federal Government are small businesses or NDCs; or (3) at least one-third of total prototype project costs are paid from non-federal sources. The form includes a sample text box demonstrating how to document NDC contributions with specific examples and cost/percentage data.
