About this file

This document is a Past Performance Questionnaire (PPQ) for a vector-borne infection surveillance contract for NAVSUP FLC Sigonella, Detachment Naples. The questionnaire is designed to evaluate a contractor's previous performance by gathering detailed information about their past work, including the quality, timeliness, customer satisfaction, and compliance with contract terms. The specific opportunity involves conducting vector-borne infection surveillance in the Ashanti Region of southern Ghana, requiring personnel, equipment, and laboratory capabilities in Kumasi for vector speciation and molecular analysis.

The PPQ includes sections for rating the relevance of previous work, assessing confidence levels across multiple performance dimensions, and providing an overall performance rating. The rating scale ranges from "Substantial Confidence" (high expectation of successful performance) to "No Confidence" (no expectation of successful performance). The document is marked as Source Selection Information and is part of a competitive solicitation process, with contact points provided for Sebastiano Paciello and Ricardo Alberto Lopez from the Navy's contracting team.
